医联体中实施老年患者中期照护模式的实践探索

Practice of Intermediate Care of Elderly Patients in Medical Treatment Alliance

The definition of intermediate care, its market demand and the difficulty of domestic implementation are introduces. This paper discusses the background, basic condition, evaluation index, medical insurance settlement, performance distribution and expert subsidy of the medical treatment alliance in Northern Jiangsu People's Hospital. The results are compared before and after the intermediate care, such as the rehabilitation effects of the elderly patients with acute rotator cuff injury and the elderly patients without intermediate care. The average length of stay and the unplanned readmission rate of patients, the satisfactions of patients and medical staff, all of which data are significantly optimized. It is suggested that the intermediate care mode in the medical treatment alliance ward is more suitable for the elderly patients who need rehabilitation after operation, so as to improve the working quality of the hospital, speed up the operation efficiency of the third level hospital, and promote the further promotion of hierarchical diagnosis and treatment.
