腹腔镜胆囊切除术中不同麻醉方式对眼内压的影响

摘    要:探讨全麻喉罩复合硬膜外阻滞麻醉对腹腔镜胆囊切除患者眼内压(IOP)的影响,60例患者行腹腔镜胆囊切除患者根据麻醉方式的不同随机分为全麻喉罩复合硬膜外阻滞组(A 组)与气管插管全麻组(B 组),每组各30例。记录麻醉前(T0)、气管插管(置入喉罩)前(T1)、气管插管(置入喉罩)后即刻(T2)、CO2气腹后10min(T3)、术毕患者清醒拔除气管导管(喉罩)前(T4)时的 IOP 。结果显示:喉罩全麻复合硬膜外阻滞用于腹腔镜胆囊切除,与单纯气管插管全麻比较,可有效降低患者在诱导插管、CO2气腹与患者清醒吸痰、拔管所引起的 IOP 升高反应(P<0.05)。

Abstract:To investigate the effect of general anesthesia(GA) with laryngeal mask airway (LMA) combined with epidural block on intraocular pressure (IOP) in patients undergoing laparoscopic cholecystectomy (LC) ,and compared with those who are under GA with tracheal intubation only ,60 patients scheduled for LC were randomly divided into 2 groups :GA with LMA combined with epidural block (group A) and GA with tracheal intubation group (group B) ,there are 30 cases in each group .The IOP was recorded at :before anesthesia (T0 ) ,tracheal intubation or LMA insertion (T1 ) ,immediately after tracheal intubation or LMA insertion (T2 ) ,10 min after CO2 pneumoperitoneum (T3 ) ,and before extubation or LMA removal (T4 ) after awareness .The results indicated that the general anesthesia with laryngeal mask airway combined with epidural block could effectively reduce the high IOP reaction caused by intubation ,CO2 pneumoperitoneum ,sputum aspiration and extubation after awareness ,compared to GA with tracheal intubation only (P< 0 .05) .
