The emergence of the ethics consultation as a means to resolve moral crises in clinical medicine has revealed the need for a worksheet that would facilitate intake and analysis. The author developed the Bioethics Consultation Form as an attempt to remedy this need. The form is arranged in an outline format and is a useful asset to ethics committee discussions and record keeping. The first section covers basic intake data concerning the patient's medical and personal information, advance directives, and values, as well as the values of the physician and family. After the intake section is completed with the above data, the ethics consultant then turns to the analysis section. This second section allows for (1) the discussion of conflicting values, (2) the identification of priorities, and (3) the elucidation of ethical norms relevant to the case.The Bioethics Consultation Form was adopted by the Patient Care Advisory committee of the Franklin Square Hospital Center in Baltimore, Maryland in 1986. The methodology in the use of the form will be discussed. Further, the potential spectrum of consultative cases that can be analyzed using the form will be highlighted.  相似文献

A combined multi-attribute utility and expectancy-value model has repeatedly been found to yield a worse fit to choices than to preference ratings. The present study investigated two possible explanations for this finding. First, people's belief-value structures may change in the choice task as they try to find the best alternative. Second, a difficult choice task may cause the decision maker to use simplifying heuristics. In the first of two experiments, subjective belief-value structures were measured on two occasions separated by about one week. Immediately before the second measurement, different groups of subjects performed a choice task, gave preference ratings, or performed a control task. The results did not support an interpretation of the greater difficulty of predicting choices in terms of changes in belief-value structures. However, the notion of simplifying heuristics received support by the finding that adopting simpler versions of the original model improved the predictions of the choices. In the second experiment, beliefs were measured immediately before or after each of a series of choices or preference ratings. The results indicated that although temporary changes in beliefs may occur, they can hardly provide a full account of the differential predictability of preferences and choices.  相似文献

The use of spelling-sound information in both reading and spelling was evaluated by having children read and spell nonwords and five types of words that differed in terms of their regularity for reading and spelling. The subjects were grade 3 children who had been psychometrically defined as good readers and good spellers (“good”), good readers and poor spellers (“mixed”), or poor readers and poor spellers (“poor”). Results indicated that all children attempted to use spelling-sound correspondences in both reading and spelling, although children in both the mixed and the poor groups had weaker knowledge of these correspondences and were less systematic in their use of them. Furthermore, even though the children in the mixed group had been matched with children in the good group on reading comprehension, the number and type of errors made by the mixed subjects on both the reading and spelling tasks were more similar to those of the poor subjects than to those of the good subjects.  相似文献

Four experiments examined the control of observing responses by information feedback during visual discrimination learning. Second-grade children participated in Experiment 1; kindergarten, second-, and fifth-grade children were subjects in Experiments 2 and 3, and grade 5 and adult subjects were tested in Experiment 4. In order to view the stimuli, subjects in Experiments 1, 2, and 3 activated lights in viewing boxes; in Experiment 4, stimulus fixations were measured using a corneal reflection technique. Fifth graders and adults observed the discriminative stimuli for longer times on trials following negative feedback than on trials following positive feedback; in contrast, kindergartener's observing was not affected by type of feedback. Second graders showed smaller and less reliable reactions to type of feedback than did older subjects. These results support the view that visual observing is controlled by cognitive processes associated with hypothesis testing.  相似文献

In the framework of a robustness study on maximum likelihood estimation with LISREL three types of problems are dealt with: nonconvergence, improper solutions, and choice of starting values. The purpose of the paper is to illustrate why and to what extent these problems are of importance for users of LISREL. In a conditioned inhibition paradigm (A+, B+, AX?_, pigeons received either of two keylight stimuli reliably followed by food (A+, B+). However, when one of these keylights was accompanied by another stimulus, food did not follow (AX?). For some groups, the putative inhibitor was a tone, whereas for others it was illumination of a red houselight. The birds pecked the A and B stimuli at a high rate. When X was red houselight, the birds pecked A at a much lower rate in the presence of X. When X was a tone, discrimination between A and AX was much poorer. Moreover, in a transfer test, red houselight inhibited responding to the other keylight, B, but tone did not. These results indicate that red houselight becomes a conditioned inhibitor more quickly than tone in appetitive situations, just as red houselight becomes a conditioned excitor more quickly in those situations. These results contradict the assertion that the latter outcome occurs because red houselight is a stronger appetitive excitor than tone at the start of the experiment (the “head start” hypothesis).  相似文献
