There is a debate regarding the use of the white coat, a traditional symbol of the medical profession, by students. In a study evaluating final-year South African medical students' perceptions, the white coat was associated with traditional symbolic values (e.g., trust) and had practical uses (e.g., identification). The coat was generally perceived to evoke positive emotions in patients, but some recognized that it may cause anxiety or mistrust. Donning a white coat generally implied a responsibility to the profession. For a few, without the coat, patients would not cooperate, resulting in some perceiving no need to be distinguished from qualified practitioners. There was thus some evidence of entitled (vs. earned) respect. In the light of the underresourced health care setting in which these students learn clinical medicine, we recommend that students be able to recognize the potential for unprofessional or unethical behavior. The purpose of this qualitative, ethnographic study was to explore the impact of a chronic disease on low socio-economic women with RA in South Africa. Ten women participants with a mean age of 48 years and a mean duration of RA of 10 years were obtained through purposive sampling from a public hospital RA clinic. In-depth individual interviews were conducted as a method of data collection. The interviews were tape recorded and transcribed for the data analysis. Grounded theory analysis was used which produced 3 primary categories around the impact of RA: (1) Physical impact, (2) Social and economic impact, and (3) Psychological impact; and 5 sub-categories, namely, Pain and poor Functional Status, Change in life-style following a weakened economic position, Change in the constitution of the “Self”, Change in the constitution of the “Self” of the spouse/partner, and Depression. Depression, pain and poor functional status are well known health outcomes for RA patients. Perceived change in the psychological concept of the “Self” is less frequently reported. The value of this study is that the concept of the self is further explored in chronically ill individuals as a ‘fluid’ one, with the potential for undergoing a complete transformation as a consequence of RA. 相似文献

This study explored South African adolescents’ perceived risk and protection for HIV infection. Adolescents (N=28) with a mean age of 14.6 years receiving mental health care participated in Focus Group Discussions (FGDs) to engage in an in-depth exploration of their perceived risk and protective mechanisms for HIV prevention. The data were analysed using thematic content analysis. Themes that emerged from the FGDs associated with risk for HIV infection were: Alcohol and illicit drug use; Non-condom use; Prostitution and risky sexual activity with multiple partners; and Non-disclosure of HIV status. Themes that emerged as protective mechanisms included: HIV knowledge and appropriate HIV risk reducing behaviour; and Proactive parental attitudes and behaviour. These findings highlight the importance of ascertaining the perceptions of adolescents about risk and protection from HIV infection in order to inform future programs for HIV prevention among adolescents. 相似文献

Previous research has investigated the complex association between religious beliefs and racism. Many studies have found that fundamentalist religious beliefs are positively associated with racial prejudice among European and European American populations. However, few studies have examined whether this association is found in other cultures or whether the association also characterizes spiritual beliefs. Data from 493 South African university students from three racial backgrounds revealed significant differences among the groups. A positive association between fundamentalism and racial prejudice was found among participants, but general spiritual beliefs were negatively associated with racist attitudes. The results emphasize the need to address contextual factors that influence the association between religious beliefs and racism within a given culture. 相似文献

The association between psychosocial factors and disability is less clear. This study investigated the biological and psychosocial (employment and psychological distress) factors associated with level of disability in an adult sample in South Africa. Data were analysed from a cross-sectional survey among adults aged 18–64 (n = 4974). Multiple linear regression was used to investigate the associations of the selected variables with disability. The mean percentage score on the WHODAS scale of disability was 5.31% (95% CI: 4.74–5.88). Age (p < 0.001) and race (p = 0.0002) were significantly associated with disability, and history of stroke (β = 7.19, 95% CI: 3.19–11.20) and heart-related conditions (β = 2.08, 95% CI: [0.23–3.93) showed positive associations. Of the psychosocial variables, psychological distress (β = 10.49 [8.63–12.35]) showed a strong positive association while employment (?1.62 [?2.36 to ?0.88]) showed a negative association with disability. The association between demographic factors, medical conditions and increased disability confirms the findings in the literature. The finding that psychological distress is associated with increased disability has not been frequently reported. This study highlights specific psychosocial targets that may be usefully addressed by health policies and interventions in order to improve disability management. 相似文献

Burnout is typically viewed as resulting from situational factors (e.g., workload). However, as burnout is partly a reaction to frustrated goal striving, dispositional goal orientation may also be important. We used Hobfoll's ( 1989 ) conservation of resources model to develop hypotheses for relationships between goal orientation and 3 dimensions of burnout. Self‐report questionnaire measures of the 2 × 2 model of goal orientation (Elliot & McGregor, 2001 ) and burnout were administered to undergraduate students. Structural equation modeling revealed that all 4 dimensions of goal orientation were related to burnout. Avoidance‐related dimensions were positively associated and approach‐related dimensions were negatively associated with burnout. The findings suggest the importance of considering the role of motivational dispositions in development, prediction, and prevention of burnout. 相似文献
