Two cases of Gilles de la Tourette syndrome from India are presented. The symptomatology of Tourette syndrome is the same as that documented in western populations which suggests biological factors in the aetiology of the syndrome.  相似文献

We reply to the Ioannidis's paper "Effectiveness of antidepressants; an evidence based myth constructed from a thousand controlled trials." We disagree that antidepressants have no greater efficacy than placebo. We present the efficacy from hundreds of trials in terms of the percentage of patients with a substantial clinical response (a 50% improvement or more symptomatic reduction). This meta-analysis finds that 42-70% of depressed patients improve with drug and 21%-39% improve with placebo. The response benefit of antidepressant treatment is 33%-11% greater than placebo. Ioannidis argues that it would be vanishingly smaller because systematic biasing in these clinical trials would reduce the drug-placebo difference to zero. Ioannidis' argument that antidepressants have no benefit is eroded by his failures of logic because he does not present any evidence that there are a large number of studies where placebo is substantially more effective than drug. (To reduce to zero, one would also have to show that some of the unpublished studies find placebo better than drug and have substantial systematic or methodological bias). We also present the empirical evidence showing that these methodological concerns generally have the opposite effect of what Ioannidis argues, supporting our contention that the measured efficacy of antidepressants likely underestimates true efficacy. Our most important criticism is Ioannidis’ basic underlying argument about antidepressants that if the existing evidence is imperfect and methods can be criticized, then this proves that antidepressant are not efficacious. He presents no credible evidence that antidepressants have zero effect size. Valid arguments can point out difficulties with the data but do not prove that a given drug had no efficacy. Indeed better evidence might prove it was more efficacious that originally found. We find no empirical or ethical reason why psychiatrists should not try to help depressed patients with drugs and/or with psychotherapeutic/behavioral treatments given evidence of efficacy even though our treatment knowledge has limitations. The immense suffering of patients with major depression leads to ethical, moral, professional and legal obligations to treat patients with the best available tools at our disposal, while diligently and actively monitoring for adverse effects and actively revising treatment components as necessary.  相似文献

In the first exposition of the doctrine of indeterminacy of translation, Quine asserted that the individuation and translation of truth-functional sentential connectives like ‘and’, ‘or’, ‘not’ are not indeterminate. He changed his mind later on, conjecturing that some sentential connectives might be interpreted in different non-equivalent ways. This issue has not been debated much by Quine, or in the subsequent literature, it is, as it were, an unsolved problem, not well understood. For the sake of the argument, I will adopt Quine’s background assumption that all the semantic features of a language can be reduced to the speakers’ dispositions toward assent and dissent, as far as only the truth-conditional core of the meaning of sentences is concerned. I will put forward an argument to the effect that the speech dispositions of most, if not all, English (French, Italian, etc.) speakers constrain a unique translation of their connectives. This argument crucially relies on an empirical conjecture concerning the behaviour of these operators.  相似文献

Introduction: Evidence from phenomenological, family, genetic, and treatment studies from Western centers have suggested that tic-related obsessive-compulsive disorder (OCD) could be different from non-tic-related OCD. This study from India investigated the differences in OCD with and without tics, with respect to sociodemographics, symptom profile, and comorbidity, including obsessive-compulsive spectrum disorders, to examine whether the clinical profile of tic-related OCD is similar to that reported previously. Methods: Fifty subjects with OCD and tics (chronic motor tics and Tourette syndrome) were compared with 141 OCD subjects without tics. Results: Subjects having OCD with tics tended to be males, and had an earlier onset of illness. They had more of symmetry/aggressive and religious obsessions, and cleaning, ordering/arranging, hoarding, and repeating compulsions and were associated with trichotillomania and hypochondraisis. Stepwise backward (Wald) regression analysis showed that an early age of onset, male gender, aggressive obsessions, cleaning compulsions, and trichotillomania were significantly associated with tic-related OCD. Conclusion: The findings of this study from India are broadly similar to those reported previously from the West indicating the universality of differences in tic- and non-tic-related OCD. Our findings also support the existing evidence that tics contribute to the heterogeneity of OCD.  相似文献

The motion aftereffect is a robust illusion of visual motion resulting from exposure to a moving pattern. There is a widely accepted explanation of it in terms of changes in the response of cortical direction-selective neurons. Research has distinguished several variants of the effect. Converging recent evidence from different experimental techniques (psychophysics, single-unit recording, brain imaging, transcranial magnetic stimulation, visual evoked potentials and magnetoencephalography) reveals that adaptation is not confined to one or even two cortical areas, but occurs at multiple levels of processing involved in visual motion analysis. A tentative motion-processing framework is described, based on motion aftereffect research. Recent ideas on the function of adaptation see it as a form of gain control that maximises the efficiency of information transmission at multiple levels of the visual pathway.  相似文献

Why do faces become easier to recognize with repeated exposure? Previous research has suggested that familiarity may induce a qualitative shift in visual processing from an independent analysis of individual facial features to analysis that includes information about the relationships among features (Farah, Wilson, Drain, & Tanaka Psychological Review, 105, 482–498, 1998; Maurer, Grand, & Mondloch Trends in Cognitive Science, 6, 255–260, 2002). We tested this idea by using a “summation-at-threshold” technique (Gold, Mundy, & Tjan Psychological Science, 23, 427–434, 2012; Nandy & Tjan Journal of Vision, 8, 3.1–20, 2008), in which an observer’s ability to recognize each individual facial feature shown independently is used to predict their ability to recognize all of the features shown in combination. We find that, although people are better overall at recognizing familiar as opposed to unfamiliar faces, their ability to integrate information across features is similar for unfamiliar and highly familiar faces and is well predicted by their ability to recognize each of the facial features shown in isolation. These results are consistent with the idea that familiarity has a quantitative effect on the efficiency with which information is extracted from individual features, rather than a qualitative effect on the process by which features are combined.  相似文献

Is it Push or Pull? Recent Evidence from Migration into Bangalore, India   总被引:1,自引:0,他引:1  
Given urban areas contribute to more than 65 % of India’s gross domestic product (GDP), the sustainability of the rapid 8–9 % GDP growth India has experienced in the last decade, is dependent to a large extent on urban areas. With migration being one of the important factors contributing to the growth of urban population, we attempt to understand whether it is push (out of the rural area) or pull (toward the urban area due to its perceived benefits) which explains migration in India, taking the case of Bangalore, which has one of the largest proportions of in-migrants to total population. An examination of these factors is done using a primary survey of migrants in Bangalore. Using a probit model, we find that the lower the level of education of the migrant, the greater the importance of the push factors whereas with increasing level of education of the migrant, pull factors become more important in migration. Women are more likely to be “pulled” toward urban areas. We find migrants from within Karnataka are “pushed”. This suggests that nonfarm employment opportunities have to be increased, rural infrastructure improved, and the development of small and medium towns encouraged.  相似文献

The present paper analyses the impact of organizational communication on job satisfaction and job performance. Data were collected from 380 employees working at different managerial levels in various organizations in India by using Organizational Communication Scale (Roberts & O’Reilly, 1974), Job Satisfaction Survey scale (Spector, 1985), and Job Performance scale (Rodwell, Kienzle & Shadur, 1998). It was found that organizational communication had a significant effect on job satisfaction and job performance of the employees. The analysis further indicated that the employees at different levels perceived job satisfaction differently. Thus, it can be inferred that in Indian organizations, job satisfaction and performance are very much dependent on the communication behavior of the organization.  相似文献
