A residential treatment program has been developed specifically for substance-abusing pregnant and parenting women in Finland, focusing on simultaneously supporting maternal abstinence from substances and the mother-baby relationship. The aims of the study are to explore maternal pre- and postnatal reflective functioning and its association with background factors, maternal exposure to trauma, and psychiatric symptoms, postnatal interaction, child development, and later child foster care placement. Participants were 34 mother-baby pairs living in three residential program units during the pre- to postnatal period. We employed self-report questionnaires on background, trauma history, and psychiatric symptoms (Brief Symptom Inventory: L.R. Derogatis, 1993; Edinburgh Postnatal Depression Scale: J.L. Cox, J.M. Holden, & R. Sagovsky, 1987; Traumatic Antecedents Questionnaire: B. Van der Kolk, 2003), videotaped mother-child interactions coded for sensitivity, control, and unresponsiveness (Care Index for Infants and Toddlers: P. Crittenden, 2003); a standardized test of child development (Bayley Scales of Infant Development-II: N. Bayley, 1993); and semistructured interviews for maternal reflective functioning (Pregnancy Interview: A. Slade, E. Bernbach, J. Grienenberger, D.W. Levy, & A. Locker, 2002; Parent Development Interview: A. Slade et al., 2005). Pre- and postnatal maternal reflective functioning (RF) was on average low, but varied considerably across participants. Average RF increased significantly during the intervention. Increase in RF level was found to be associated with type of abused substance and maternal trauma history. Mothers who showed lower postnatal RF levels relapsed to substance use more often after completing a residential treatment period, and their children were more likely to be placed in foster care. The intensive focus on maternal RF is an important direction in the development of efficacious treatment for this very high risk population.  相似文献

We examined the associations between attachment‐related symptoms (symptoms of reactive attachment disorder (RAD), symptoms of disinhibited social engagement disorder (DSED), and clinging) and later psychological problems among international adoptees. The study population comprised internationally adopted children (591 boys and 768 girls, 6–15 years) from the ongoing Finnish Adoption (FinAdo) study. Data were gathered with self‐administered questionnaires both from adoptive parents and from adoptees aged over 9 years. Attachment‐related symptoms were measured using of a short (8‐item) questionnaire and later behavioral/emotional problems were assessed using the Child Behavior Checklist (CBCL) and the Five to Fifteen (FTF) scale for attention‐deficit/hyperactivity disorder (ADHD) symptoms. RAD and DSED symptom subscales were associated with an increased risk of emotional and behavioral problems and ADHD. Especially the mixed type of attachment‐related symptoms was strongly associated with later emotional and behavioral problems.  相似文献

This study investigated the potential association between symptoms of reactive attachment disorder and language difficulties among internationally adopted children in Finland (the FinAdo study). The language difficulties were assessed using a standardised Five to Fifteen (FTF) parental questionnaire and the symptoms of reactive attachment disorder using a FinAdo questionnaire. The study sample consisted of 689 6–15 year old children (49.2% boys, response rate 48%). Twenty-nine percent of the children were reported to have language difficulties and 8% severe language difficulties (10% and 2%, respectively, in the general population). A child's symptoms of reactive attachment disorder were associated with language difficulties and severe language difficulties, OR 2.15, 95% CI [1.39, 3.31] and OR 4.33, 95% CI [1.57, 11.98], respectively, the associations being robust to adjustments for background factors.  相似文献

Students’ suggestions for how to eliminate bullying at universities were gathered as part of an e-questionnaire sent to each university student (N = 10,551) at a Finnish university. The suggestions (n = 2804) regarding how to address bullying at universities were divided into the following four classes: support (944), punishment (78), support and punishment (65), and no suggestions (1717). Further analysis concerning support revealed the following five categories according to Houses’ (1981) classification: instrumental/material support (n = 265), informational support (n = 263), emotional support (n = 139), and appraisal support (n = 69). In addition, there were 208 inexact suggestions regarding how to stop bullying. Analysis concerning resources for help revealed the following five categories: Finnish Students Health Service/student association/police/university priest (120), university teachers (49), other students/peer support (41) and administration (36). In this classification, there were 698 inexact suggestions. This is a sign of the difficulty in addressing bullying situations.  相似文献

This study investigated attachment‐related behavioural problems and their change over time among internationally adopted toddlers (N = 171, boys 40%) in Finland. The parents filled in questionnaires posted in 2007 and again in 2009. At the baseline (on average, 1.9 years from adoption), the mean age of the adoptees was 2.8 years. The comparison group (N = 244, boys 46%) was collected through well‐baby clinics. Results from the paired t‐test and mixed models showed that the adoptees' attachment‐related behavioural problems decreased during the 2‐year follow‐up and that they did not manifest more problems than their age‐mates any more 4 years after adoption. The decrease in behavioural problems was associated with the child's female gender, country of origin and adoptive parents' positive experience about adoption. A child's stereotypic or self‐injurious behaviour was not associated with the change in attachment‐related behaviour over time, although both were associated with behavioural problems. People's threat perceptions play a role in influencing foreign policies towards perceived adversary countries. Earlier research has identified multiple components shaping mass-level threat perceptions including military power, adversary country's perceived intentions, and national identities. On the individual level, education, use of media, and interest in politics have been shown to influence threat perceptions. However, most studies on perceptions of security threats fail to include both contextual and individual-level explanatory factors and to consider that different national threats may be constructed differently. This research bridges formation of threat perceptions on the individual level to wider societal processes and provides an empirical perspective to understanding threat perceptions among the educated section of the Chinese population. To analyze threat perceptions, students from leading Chinese universities (N = 771) took part in a survey in the autumn of 2011 and spring of 2012. Respondents who followed conventional media were more likely to perceive both the United States and Japan as threatening, and the effect of media consumption was particularly strong with regards to perceived threat from Japan. In addition, each threat perception was significantly associated with threat-specific explanatory factors. Potential explanatory factors of threat perceptions were explored with linear regression models.  相似文献

International adoptees need to cope with stressful transitions and to develop secure attachment with their caregivers at the same time. Although most children adopted from abroad adjust fine, they are at increased risk of psychological problems. We investigated whether both child and family‐related factors are associated with later psychological problems and whether the length of time spent at home after adoption before daycare moderates these associations among internationally adopted children in Finland (FinAdo, Finnish Adoption Study). The sample consisted of 1,265 children (708 girls, 557 boys) who arrived in Finland before they started school (mean age 2 years at arrival). Later behavioral problems were measured using the Child Behavior Check List (CBCL). According to our results, male gender, older age, child's early clinical symptoms (problems of sensory processing) and single parenthood were associated with later behavioral problems measured by CBCL scores. Longer stay at home before the start of daycare or school modified these results. Longer stay at home was associated with less later behavioral externalizing problems in girls but not in boys compared to those who spent a shorter time at home.  相似文献

Maternal mental health and the contents of her representational world are important determinants of early parent–child relationship. We examined, first, the role of prenatal and postnatal depressive symptoms and maternal attachment style in predicting the quality of mother–child interaction. Second, we analysed whether the secure‐autonomous attachment style can protect the dyadic interaction from the negative effects of maternal depression. The participants were 59 mother–infant pairs examined during pregnancy (T1), 4–5 months postpartum (T2) and when the children were approximately 14 months old (T3). Maternal attachment style was assessed with a modified Adult Attachment Interview ‐procedure, depressive symptoms with Edinburgh Postnatal Depression Scale, and observed mother–child interaction with Care Index. The results show that autonomous mothers were more sensitive and responsive and their children more co‐operative than dyads with dismissing maternal attachment style. As hypothesized, mothers with the combination of both prenatal and postpartum depressive symptoms were highly unresponsive in their dyadic interaction. Further, prenatal depressive symptoms had a stronger impact on maternal unresponsiveness than postnatal symptoms. As hypothesized, mother's autonomous attachment style protected the mother–child interaction from the negative impact of maternal postnatal depressive symptoms, whereas dyads with preoccupied mothers were especially at risk for interaction problems when mothers had postpartum depressive symptoms.
