DSM-IV-TR defines four subtypes of bipolar disorder (BP): bipolar I, bipolar II, cyclothymic disorder and bipolar not otherwise specified (NOS). However, cyclothymic disorder in children is rarely researched, or often subsumed in an “NOS” category. The present study tests the replicability of findings from an earlier study, and expands on the criterion validity of cyclothymic disorder in youth. Using the Robins and Guze (1970) framework we examined the validity of cyclothymic disorder as a subtype of BP. Using a youth (ages 5–17) outpatient clinical sample (N?=?894), participants with cyclothymic disorder (n?=?53) were compared to participants with other BP spectrum disorders (n?=?399) and to participants with non-bipolar disorders (n?=?442). Analyses tested differences in youth with cyclothymic disorder and bipolar disorder not otherwise specified who do, and those who do not, have a parent with BP. Compared to youth with non-bipolar disorders, youth with cyclothymic disorder had higher irritability (p?<?0.001), more comorbidity (p?<?0.001), greater sleep disturbance (p?<?0.005), and were more likely to have a family history of BP (p?<?0.001). Cyclothymic disorder was associated with a younger age of onset compared to depression (p?<?0.001) and bipolar II (p?=?0.05). Parental BP status was not significantly associated with any variables. Results support that cyclothymic disorder belongs on the bipolar spectrum. Epidemiological studies indicate that cyclothymic disorder is not uncommon and involves significant impairment. Failing to differentiate between cyclothymic disorder and bipolar NOS limits our knowledge about a significant proportion of cases of bipolarity.  相似文献

Since the mid 1990s, early-onset bipolar spectrum disorders (BPSDs) have received increased attention in both the popular press and scholarly press. Rates of diagnosis of BPSD in children and adolescents have increased in inpatient, outpatient, and primary care settings. BPSDs remain difficult to diagnose, particularly in youth. The current diagnostic system makes few modifications to accommodate children and adolescents. Researchers in this area have developed specific BPSD definitions that affect the generalizability of their findings to all youth with BPSD. Despite knowledge gains from the research, BPSDs are still difficult to diagnose because clinicians must: (1) consider the impact of the child’s developmental level on symptom presentation (e.g., normative behavior prevalence, environmental limitations on youth behavior, pubertal status, irritability, symptom duration); (2) weigh associated impairment and course of illness (e.g., neurocognitive functioning, failing to meet full DSM criteria, future impairment); and (3) make decisions about appropriate assessment (differentiating BPSD from medical illnesses, medications, drug use, or other psychiatric diagnoses that might better account for symptoms; comorbid disorders; informant characteristics and assessment measures to use). Research findings concerning these challenges and relevant recommendations are offered. Areas for further research to guide clinicians’ assessment of children with early-onset BPSD are highlighted. Deep semantic encoding of verbal stimuli can aid in later successful retrieval of those stimuli from long-term episodic memory. Evidence from numerous neuropsychological and neuroimaging experiments demonstrate regions in left prefrontal cortex, including left dorsolateral prefrontal cortex (DLPFC), are important for processes related to encoding. Here, we investigated the relationship between left DLPFC activity during encoding and successful subsequent memory with transcranial magnetic stimulation (TMS). In a pair of experiments using a 2-session within-subjects design, we stimulated either left DLPFC or a control region (Vertex) with a single 2-s train of short theta burst stimulation (sTBS) during a semantic encoding task and then gave participants a recognition memory test. We found that subsequent memory was enhanced on the day left DLPFC was stimulated, relative to the day Vertex was stimulated, and that DLPFC stimulation also increased participants’ confidence in their decisions during the recognition task. We also explored the time course of how long the effects of sTBS persisted. Our data suggest 2 s of sTBS to left DLPFC is capable of enhancing subsequent memory for items encoded up to 15 s following stimulation. Collectively, these data demonstrate sTBS is capable of enhancing long-term memory and provide evidence that TBS protocols are a potentially powerful tool for modulating cognitive function.  相似文献

I argue that there is a distinction to be drawn between two kinds of mental realism, and I draw some lessons for the realism-antirealism debate. Although it is already at hand, the distinction has not yet been drawn clearly. The difference to be shown consists in what realism is about: it may be either about the interpretation of folk psychology, or the ontology of mental entities. I specify the commitment to the fact-stating character of the discourse as the central component of realism about folk psychology, and from this I separate realism about mental entities as an ontological commitment towards them. I point out that the two views are mutually independent, which provides the possibility of considering folk psychology as not being in cognitive competition with scientific psychology. At the end I make a tentative suggestion as to how to interpret the former in order to avoid this conflict.  相似文献

In this paper, I introduce a prominent classical scholar, József Balogh, whose work can be read as a significant contribution to the historiography of ancient, and in some sense modern, philosophy. Following a summary biography, I sketch the relevance of Balogh's interpretation of Augustine. I draw some analogies between his and Eric Havelock's treatment of the problems in ancient philosophy, and argue that the obvious similarities between them have a common origin, namely the perspective of the orality/literacy chasm which both treated, in connection with their research into Augustine and Plato, as crucial. Subsequently, I show how the problem of reading aloud, which Balogh was the first to treat systematically, has acquired significance in some current debates in philosophy.  相似文献

In this paper I reconstruct the central concept of the young Lukács??s and Mannheim??s sociology of knowledge, as they present it in their writings in the early decades of the twentieth century. I argue that this concept, namely Weltanschauung, is used to refer to some conceptually unstructured totality of feelings, which they take to be a condition of possibility of intellectual production, and this understanding is contrasted to an alternative construal of the term that presents it as logically structured, quasi-theoretical background knowledge. This concept has Kantian reminiscences: it is a condition of possibility of intellectual production in general. The young Mannheim and Lukács rely on ??Weltanschauung?? so understood as a phenomenon mediating between the facts of society and individual intellectual production and reception: it is seen as being conditioned by sociological facts and therefore as a historical and sociological category through which, and therefore indirectly, society enters into intellectual production.  相似文献

The present paper offers a narrative of the post-World War II development of Hungarian philosophy, and argues that it is characterized by a double, historical and anthropological orientation under Marx’s influence. The resulting amalgam is an intellectual history that looks beyond the ideas themselves, searching for underlying images of man which are represented as ideological backgrounds to theories of nature, society, cognition, etc. The most important works of this approach interpret ideas and anthropologies within a Marxist framework, and see them as closely linked to the social–historical circumstances in which they develop; yet, these approaches represent an alternative attitude quite different from the official ideology of dialectical materialism.  相似文献
