The purpose of this study was to test the hypothesis that social demand could substantially affect reports of self-monitored blood glucose (BG) in adolescents with insulin-dependent diabetes mellitus. Of 34 patients initially enrolled in the study, 10 were excluded because they did not bring any BG records with them to an outpatient clinic appointment. The remaining 24 patients were randomly assigned to either a low or high social demand condition that provided instructions for monitoring of BG for the week following the appointment. The subjects' BG records were quantified to provide frequency of measurement and mean reported BG for the week prior to and after the clinic visit. Five subjects did not return their BG records for the week following the intervention. The analyses were therefore based on the 19 subjects from whom complete records were obtained. The 12 subjects in the low social demand group and 7 subjects in the high social demand group were equivalent with regard to age, duration of diabetes, socioeconomic status, and glycosylated hemoglobin. Frequency of BG measurement was similar in both groups during both weeks. The mean BG value reported in the week prior to intervention was similar for the groups. However, analyses of the post-intervention BGs revealed that subjects in the low-demand group reported significantly higher BGs compared to pre-intervention and to subjects in the high-demand group. These findings suggest that self-monitoring and reporting of BG is a social behavior that is affected by the demand characteristics of the interpersonal patient-health provider relationship. Because optimal treatment planning for individuals with diabetes requires accurate BG records, care must be taken to interpret them in light of the social demand characteristics associated with clinical assessment.  相似文献

Effective family management of type 1 diabetes in childhood is critical to maintaining optimal glycemic control. The purpose of this study was to provide preliminary evidence for a reduced form of the Diabetes Self-Management Profile (DSMP) using Rasch modeling techniques. The study was a secondary analysis of DSMP data drawn from a previous study on patterns of self-management from 239 preadolescents with type 1 diabetes. Rasch modeling strategies were used to identify the most informative items and then a reduced score composite was correlated with hemoglobin A1c (A1c) and blood glucose monitoring (BGM) frequency. A short form of the DSMP was obtained using seven items that comprised all five subscales of the DSMP. The DSMP Short-Form (DSMP-SF) composite score correlated significantly with child’s HbA1c and BGM frequency. The DSMP-SF may be considered a valid and effective screening interview alternative to the longer, original, DSMP, particularly when attempting to identify high-risk patients.  相似文献

This paper reviews the behavioural and neuroscience literatures on extinction in Pavlovian conditioning with a view towards finding possible points of contact between these two often independent lines of investigation. Recent discoveries at the behavioural level indicate (1) that conditioned stimulus (CS)-unconditioned stimulus (US) associations specific in their sensory content are fully preserved during extinction, (2) that inhibitory stimulus-response associations appear to be learned during extinction, (3) that extinction is influenced by the level of activation of the US representation during nonreinforced trials, (4) that decreases in attention can influence conditioned performance during extinction, and (5) that contexts acquire an ability to modulate learning during both conditioning and extinction. Recent discoveries at the neural systems level suggest (1) that the hippocampus is important in context-specific learning during extinction, (2) that the prefrontal cortex is possibly important in long-term memory for extinction, (3) that the basolateral amygdala may be important in sustaining attention to a CS during extinction, (4) that NMDA receptors are important either in neural plasticity during extinction or by affecting the value of the US representation during extinction, and (5) that the GABAergic system may partially mediate inhibitory learning during extinction. It is concluded that both of these levels of analysis can benefit the other in the pursuit of a more comprehensive understanding of extinction.  相似文献

Four experiments examined the effects of varying the conditioned stimulus-unconditioned stimulus (CS-US) interval (and US density) on learning in an appetitive magazine approach task with rats. Learning was assessed with conditioned response (CR) measures, as well as measures of sensory-specific stimulus-outcome associations (Pavlovian-instrumental transfer, potentiated feeding, and US devaluation). The results from these studies indicate that there exists an inverse relation between CS-US interval and magazine approach CRs, but that sensory-specific stimulus-outcome associations are established over a wide range of relatively long, but not short, CS-US intervals. These data suggest that simple CR measures provide different information about what is learned than measures of the specific stimulus-outcome association, and that time is a more critical variable for the former than latter component of learning.  相似文献

Abstract.— A retroactive interference paradigm was employed for studying qualitative memory changes. Sentences, containing an ambiguous word that was defined differently by succeeding words, were used as stimulus material. Such retroactive definitions of meaning were found to lead to qualitative changes in reproduction. It is suggested that the issue of permanence and change in memory may be related to the choice of lists of independent items or contextual units as stimulus material. A contextual theory of memory, implying the conception of memory in a stream, is advocated.  相似文献

The conventional wisdom regarding the impact of discrimination on political behavior is that the perception of prejudiced treatment motivates individuals to take political action. This study challenges this common conception by demonstrating that the source of discrimination can play a significant role in whether perceived or experienced injustice leads to activism or withdrawal from sociopolitical life. Drawing from political science and social psychology literature, this study provides a new perspective on the potential effects of discrimination on a relatively new marginalized group in the United States. Specifically, an important distinction is drawn between political (systematic) and societal (interpersonal) discrimination in analyzing the sociopolitical behavior of American Muslims in the aftermath of the 9/11 terrorist attacks. The results will hopefully encourage scholars to take a deeper look at the nexus between discrimination and democratic engagement, which is an important, complex, multidimensional, and understudied topic.  相似文献

The significant role of psychosocial factors in the management of type 1 diabetes in youth has been well documented. The International Society for Pediatric and Adolescent Diabetes (ISPAD) therefore published the Clinical Practice Consensus Guidelines for psychological care of young patients. However, it is unclear if and how these guidelines are being implemented. A questionnaire was created to assess implementation of the guidelines and directed to physicians through the ISPAD listserve via a web-based survey. One hundred fifty-five participants from 47 countries completed the survey. Ninety-six percent of respondents reported that they work in a team with other professionals, and 95 % of teams discuss psychological difficulties associated with diabetes management. Seventy-two percent of respondents reported having “easy access” to a mental health specialist (MHS). In 56 % of practice settings, the MHS is considered to be part of the team; 43 % participate in routine clinic visits and 26 % see all patients. Seventy percent screen for psychological problems and 57 % assess family functioning. Psychosocial or behavioral interventions addressing psychosocial and regimen adherence difficulties are offered by 79 % of teams. Psychological care is available for many children with diabetes worldwide. Yet, nearly 30 % of teams do not have access to a MHS. More training in the recognition of psychosocial problems and counseling skills is warranted. More advocacy is needed to increase availability and utilization of psychological services in routine diabetes care.  相似文献

The present experiments explored the importance of training with a common antecedent or consequence for common coding in symbolic matching-to-sample tasks with human subjects using reversal and transfer tests. Experiment 1 assessed common coding following training in many-to-one (MTO) and one-to-many (OTM) symbolic matching-to-sample tasks by comparing learning of partial and total reversals of these contingencies. Experiment 2 assessed common coding in MTO and OTM tasks by comparing performances in transfer tests when the common coding relations in the transfer phase were either consistent or inconsistent with those present during initial training. Evidence for common coding in the MTO and OTM discriminations was obtained in both experiments, although the amount of common coding was greater in the MTO discrimination in Experiment 1. These data are discussed in terms of associative mediational approaches to common coding phenomena. It is suggested that a basic requirement of an associative theory is that common coding should be more likely to occur when the stimuli are trained with a common consequence than when they are trained with a common antecedent, and that this requirement has been at least partly satisfied in the present studies.  相似文献

The interpersonal effects of assertive and unassertive behavior on females who scored high and low in self-reported assertion were examined. Subjects from each of the two assertion categories individually interacted with a female confederate trained to act either assertively or unassertively in a structured manner. In analyzing the subjects' impressions of the confederate, no significant differences between high and low assertives on the dimensions of competence, task attraction, social attraction, likability, and desirability were found. Assertive responding appeared less salient to the observer assertee in more naturalistic conditions and its social impact seemed more attenuated when the assertive responding took on a relatively mild form that posed little or no direct social or personal risk to the receiver of the assertive behavior.  相似文献
