Genetic Counselors’ Current Use of Personal Health Records-Based Family Histories in Genetic Clinics and Considerations for Their Future Adoption

Given the widespread adoption of electronic medical records and recent emergence of electronic family history tools, we examined genetic counselors’ perspectives on the emerging technology of the personal health record (PHR)-based family history tool that links to an electronic medical record (EMR). Two-hundred thirty-three genetic counselors responded to an on-line survey eliciting current use of electronic family history (EFH) tools and familiarity with PHR-based family history tools. Additionally, after being shown a series of screen shots of a newly developed PHR-based family history tool based on the U.S. Surgeon General’s My Family Health Portrait (United States Department of Health and Human Services 2009), participants were surveyed about the perceived usefulness, ease of use, and impact on current workflow that this kind of tool would have in their practices. Eighty-three percent reported that their institution has an EMR, yet only 35 % have a dedicated space for family history. Eighty-two percent reported that less than 5 % of their patients have a PHR, and only 16 % have worked with patients who have a PHR. Seventy-two percent or more agreed that a PHR-based family history tool would facilitate communication, increase accuracy of information, ensure consistency in recording information, increase focus on actual counseling, reduce repetitive questions, improve efficiency, and increase the legibility and clarity. Our findings suggest that participants were familiar with existing EFH tools, but that the majority did not use them in practice. Genetic counselors’ adoption of such tools is limited due to non-existence of this kind of technology or inability to integrate it into their clinics. They are also strongly in favor of adopting a PHR-based family history tool in genetics clinics, but have practical concerns that must be addressed before the tool can be implemented.     

Who is David and who is Goliath? The Eddie Long scandal and the black mega-church

The recent scandal involving Bishop Eddie Long of New Birth Missionary Baptist Church has led millions of individuals to evaluate the guilt or innocence of Bishop Long as well the significance of the black mega-church. This study examined the possible effects of a well-publicised scandal involving a black mega-church pastor on the mental health of African Americans within and outside of the church. To address this paucity in the research, over 3000 comments made by anonymous contributors on CNN's public website were analysed using a grounded theory methodology. The following five themes were identified: (1) Keep an Open Mind: “Most People Thrive on Negativity;” (2) Hypocrisy as Part of Religion: “They aren’t real Christians;” (3) Greed as Part of Religion: “Just in it for the money;” (4) Religion and/or Christianity as Problematic: “Christians scare the Jesus out of me;” and (5) Religious Leaders Actions are separate from Individuals’ Personal Faith and Their Relationship with God: “You don’t put your faith in a man.” Narratives will be offered to support and illustrate each of these themes, and the implications for the mental health of African Americans will be discussed.     

LGBTQ+ Addiction Research: An Analysis of the Journal of Addictions & Offender Counseling

Articles published in the Journal of Addictions & Offender Counseling (JAOC) from 1980 to 2018 were examined to see how LGBTQ+ issues are represented. This study also investigated whether sexual orientation and gender identity were reported in empirical studies in JAOC.     

Marital Expectations in Strong African American Marriages

The current exploratory study utilized a family strengths framework to identify marital expectations in 39 strong African American heterosexual marriages. Couples reflected on their marital expectations over their 10 or more years of marriage. Three themes emerged through qualitative analysis and the participants' own words were used in the presentation of the themes. African Americans indicated that there was growth in marital expectations over time, with marital expectations often beginning with unrealistic expectations that grew into more realistic expectations as their marriages progressed. Participants also indicated that core expectations in strong African American marriages included open communication, congruent values, and positive treatment of spouse. Finally, participants explained there is an “I” in marriage as they discussed the importance of autonomy within their marital relationships. Results are discussed in association with existing research and theory.     

Addressing Classism,Ableism, and Heterosexism in Counselor Education

As the counseling profession charts its future course, issues related to classism, ableism, and heterosexism remain fully incorporated within the multicultural/social justice curriculum. The authors define each of these forms of oppression, explicate their intersections with race, and summarize the resulting implications for counseling education and training.     

Asthma-specific cognitions,self-focused attention,and fear of negative evaluation in adolescents and young adults diagnosed with childhood-onset asthma

The present study examined the impact of asthma-specific thought intrusion (TI) and thought suppression (TS) on two cognitive-affective variables (self-focused attention and fear of negative evaluation) among adolescents and young adults (AYAs) diagnosed with childhood-onset asthma. Participants were 290 AYAs who completed assessment questionnaires and participated in a written exercise electronically. Asthma-TI and TS were reported by participants following participation in a writing assignment. Asthma-TI was associated with increased private, public, and social anxiety self-focused attention, and greater fear of negative evaluation. Interestingly, asthma-TS was not associated with these same outcome variables. Findings suggest illness-specific cognitions are associated with cognitive-affective variables and it may be important to assess for illness-specific intrusive thoughts following asthma-focused medical appointments. Additionally, findings suggest the importance of assessing asthma-TI and TS separately in order to better understand thoughts about health and psychological functioning.
