AbstractOver 90% of mental health providers encounter suicidal clients. However, the majority of marriage and family therapists (MFTs) are not exposed to suicide-specific trainings within their clinical coursework. Due to the lack of consistent training protocols for suicide assessment and management among MFTs, this qualitative study identified common MFT responses to suicidal clients, including assessment of risk severity and intervention modalities. Responses were then compared to best practices for suicidal clients. Participants inconsistently adhered to best practices identified in the larger field of mental health, revealing the need for increased MFT training on suicide to ensure appropriate and effective care. 相似文献

This article discusses the authors' experiences of the transition from trainee to trainer on a psychotherapeutic training programme, and some of the challenges involved. Using the framework offered by Rizq (2009), the article examines the many parallel processes implicit in the roles of trainee, trainer and therapist. We explore these using examples from personal experience and psychodynamic theory and concepts. We believe our experience illustrates one of the transitions that are inevitable throughout the process of training, qualifying and practising as therapeutic practitioners. These transitions, whilst often fraught with difficulty, are invariably paramount to the process of reflection and development as a practitioner and teacher. Authors have argued that various forms of interventions may be more effective in younger children. Is cognitive training also more effective, the earlier the training is applied? We review evidence suggesting that functional neural networks, including those subserving attentional control, may be more unspecialised and undifferentiated earlier in development. We also discuss evidence suggesting that certain skills such as attentional control may be important as ‘hub’ cognitive domains, gating the subsequent acquisition of skills in other areas. Both of these factors suggest that attentional training administered to younger individuals ought to be relatively more effective in improving cognitive functioning across domains. We evaluate studies that have administered forms of cognitive training targeting various subcomponents of attention and the closely related domain of working memory, and we contrast their reported transfer to distal cognitive domains as a function of the age of the participants. Although negative findings continue to be common in this literature we find that cognitive training applied to younger individuals tends to lead to significantly more widespread transfer of training effects. We conclude that future work in this area should concentrate on understanding early intensive training, and discuss a number of practical steps that might help to achieve this aim. 相似文献

Counseling in Ireland has experienced rapid growth in the past 30 years. Public attitudes toward counseling have become more positive, especially with the increasing secularization of a once strongly religious Catholic society. Licensure is nonexistent but there are certification bodies that attempt to ensure qualified practice. There is no third‐party payment, so access to counseling for impoverished individuals is a problem. Training programs are quite varied. Reliability of scores from psychological or educational assessments provides important information regarding the precision of measurement. The reliability of scores is however population dependent and may vary across groups. In item response theory, this population dependence can be attributed to differential item functioning or to differences in the latent distributions between groups and needs to be accounted for when estimating the reliability of scores for different groups. Here, we introduce group-specific and overall reliability coefficients for sum scores and maximum likelihood ability estimates defined by a multiple group item response theory model. We derive confidence intervals using asymptotic theory and evaluate the empirical properties of estimators and the confidence intervals in a simulation study. The results show that the estimators are largely unbiased and that the confidence intervals are accurate with moderately large sample sizes. We exemplify the approach with the Montreal Cognitive Assessment (MoCA) in two groups defined by education level and give recommendations for applied work. 相似文献

An Introduction to Core Competencies for the Use of Mobile Apps in Cognitive and Behavioral Practice

Mobile technologies have become increasingly pervasive and integral in our daily lives. A growing number of mobile applications are focused on health applications with many specifically focused on helping the prevention, treatment, and management of mental health issues. These apps are slowly being introduced into clinical settings, either being brought in by clients who themselves are using these tools or by health systems or providers. Unfortunately, most practicing providers obtained their clinical training before the advent of mobile health tools and methods. Thus, a critical need exists to develop and disseminate resources to develop the essential skills needed to adopt mobile health techniques. We discuss the five core competencies in mobile health care: evidence, integration, security and privacy, ethics, and cultural considerations. Although this paper will not fully prepare a provider to use mobile apps in their practice, it provides an introduction that will helpfully guide a provider to additional resources and additional ways to develop these competencies. Finally, we discuss the future directions of the field, including a growing differentiation of the impact of mobile apps on clinical care and the need for training models and experiences to match this differentiation. 相似文献

Practitioners face three challenges in delivering evidence-based practice: limitations in the evidence available to guide routine clinical decisions; limitations in clinical judgment that are hard to remedy with typical work routines; and the practical difficulties of training and sustaining the breadth of skills relevant to meet diverse patient needs in a generalist practice. We recommend designing practice environments that support development of excellent clinical judgment with use of standardized work routines that help detect relationships between clinical decisions, interventions, and patient outcomes.
