Out of all the natural phenomena surrounding us, the phenomena Life, as a matter of course, is that which is the most mysterious and least understood of all. In many aspects, it seems as though it would be possible to investigate, study, and explain Life, like so many of the other subjects that fall within the category of the natural sciences: time, space, mass, movement, and so on. However, in its essence, Life is organized and managed differently than all other natural phenomena, operating, as it were, according to its own laws. Life certainly has certain clearly recognizable characteristics, such as: metabolism, homeostasis, continuity, and more. However, up until the present time, no definition for Life has been provided, which unifies it as an entity, and differentiates it unmistakably from other natural phenomenon. The current article offers a definition of the true nature of the phenomenon of Life, which in turn helps to explain the big questions of life—evolution, the mind–body problem, consciousness, and more.  相似文献

Somatic symptom disorder (SSD) and illness anxiety disorder (IAD) are two new diagnoses introduced in the DSM-5. There is a need for reliable instruments to facilitate the assessment of these disorders. We therefore developed a structured diagnostic interview, the Health Preoccupation Diagnostic Interview (HPDI), which we hypothesized would reliably differentiate between SSD, IAD, and no diagnosis. Persons with clinically significant health anxiety (n = 52) and healthy controls (n = 52) were interviewed using the HPDI. Diagnoses were then compared with those made by an independent assessor, who listened to audio recordings of the interviews. Ratings generally indicated moderate to almost perfect inter-rater agreement, as illustrated by an overall Cohen’s κ of .85. Disagreements primarily concerned (a) the severity of somatic symptoms, (b) the differential diagnosis of panic disorder, and (c) SSD specifiers. We conclude that the HPDI can be used to reliably diagnose DSM-5 SSD and IAD.  相似文献

The author investigates the oneiric representation of somatic states and the diagnostic capacity of dreams. He draws on Freud's hypotheses on the procedures by which somatic stimuli insert themselves in oneiric elaboration and restructures them according to the recent neurobiological discoveries and to analytical experiences. In the representations of certain dreams, with a psychic interpretation agreed upon by the patients, somatic alterations unknown to the analytical couple were discriminated and confirmed by radiological investigations. These representations were linked to the manifestation of one aspect of the bodily Self, neglected in the precocious maternal relation, that entered the organization of the Self consolidated in the relation with the paternal figure. This conjunction gave origin to the double meaning (somatic and psychic) of the dream. The entering of the somatic representation in the oneiric one did not appear to be the figurative effect, but of a condensation of diagnostic capacity into the meaning of the dream. This characteristic manifested itself in the particular styles of the dreamers, interpretable by an analyst countertransferentially oriented. The perception or scotomization of the condensation in the interpretation of the dream and of the moment had an effect on the evolution of the analysis.  相似文献

Objective: Individuals with chronic obstructive pulmonary disease (COPD) exhibit low physical and mental health-related quality of life (HRQL) and high susceptibility to disability. We investigated the influence of psychological factors on HRQL and disability in COPD individuals recruited from the general population. In line with Leventhal’s common sense model, we expected psychological factors to be associated with HRQL and disability even after controlling for medical status.

Methods: Individuals with COPD (n = 502; 59.7 years old; GOLD grades were I: 3%, II: 17%, III: 34%, IV: 46%) were assessed through an online survey administered via COPD patient organisations in Germany. Individuals filled in the Short Form Health Survey (SF-12), COPD Assessment Test, Patient Health Questionnaire (modules: GAD-2, PHQ-15, PHQ-9), Brief Illness Perception Questionnaire, a questionnaire that assesses causal illness attributions, and the internal illness-related locus of control scale of the ‘KKG questionnaire for the assessment of control beliefs about illness and health’. Multiple linear regressions were calculated.

Results: The investigated factors explained high variances (disability = 56%, physical HRQL = 28%, mental HRQL = 63%, p ≤ .001). Better mental health, more optimistic illness perceptions, attribution to psychological causes, and stronger internal locus of control were associated with lower disability and better HRQL. Comorbid somatic symptoms contributed to high disability and low quality of life.

Conclusion: Psychological factors, such as illness perception, attribution and internal locus of control, were associated with disability and HRQL. These factors should be considered when designing treatments for individuals with COPD, and adequate interventions should be provided to enhance illness understanding and self-management skills.  相似文献

Depression is typically treated as a homogeneous construct despite evidence for distinct cognitive, affective, and somatic symptom dimensions. Anxiety sensitivity (AS; the fear of consequences of anxiety symptoms) is a cognitive risk factor implicated in the development of depressive symptoms. However, it is unclear how lower order AS dimensions (i.e. physical, cognitive, and social concerns) relate to depressive symptom factors. Confirmatory factor analysis, followed by structural equation modeling, were conducted to examine the factor structure of depression and to then examine the relations between these factors and the lower order factors of AS. This study was conducted in a sample of 374 adults (M age = 35.5, 54.3% female) with elevated levels of psychopathology (89.2% meeting criteria for at least one DSM-5 diagnosis, 25.6% primary depressive disorder). In this study a two-factor model of depression, composed of Cognitive and Affective/Somatic factors, was superior to one- and three-factor solutions. AS cognitive concerns were related to both cognitive and affective/somatic symptoms of depression. Neither of the other AS dimensions was related to depression symptom dimensions. These findings provide a better understanding of the relations between AS and depression symptoms.  相似文献

Persistent somatic symptoms of varying etiology are very common in emerging adults and can lead to distress and impairment. Internet-delivered interventions could help to prevent the burden and chronicity of persistent somatic symptoms. This study investigated the impact of therapist guidance on the effectiveness of a cognitive-behavioral Internet intervention for somatic symptom distress (iSOMA) in emerging adults, as a secondary analysis of a two-armed randomized controlled trial. We included 149 university students (83.2% female, 24.60 yrs) with varying degrees of somatic symptom distress who were either allocated to the 8-week intervention with regular, written therapeutic guidance (iSOMA guided) or to the control group (waitlist), which was afterwards crossed over to receive iSOMA with guidance-on-demand (iSOMA-GoD). Primary outcomes were somatic symptom distress (assessed by the PHQ-15) and psychobehavioral symptoms of the somatic symptom disorder (assessed by the SSD-12) at pre- and post-treatment. Secondary outcomes included depression, anxiety, and disability. Both treatments showed statistically significant pre-post improvements in primary (iSOMA-guided: d = 0.86–0.92, iSOMA-GoD: d = 0.55–0.63) and secondary outcomes. However, intention-to-treat analysis revealed non-significant between-group effects for all outcomes (ps ≥ .335), after controlling for confounding variables, and effect sizes were marginal (d = −0.06 to 0.12). Overall, our findings indicate that Internet-delivered cognitive behavioral therapy with regular guidance is not unequivocally superior to guidance-on-demand in alleviating somatic symptom distress and associated psychopathology in emerging adults. As a next step, non-inferiority studies are needed to test the robustness of these findings and their impact on clinical populations.  相似文献

This article tries to explain, in the light of some neuroscientific and psychoanalytical considerations, the repetitive pattern of panic attacks. Freud considered the panic attack as an ‘actual neurosis’ not involving any conflictual process. Recent neuroscientific findings indicate that psychosomatic reactions, set off by a danger situation, depend on the primitive circuit of fear (including the amygdala) characterised by its speed, but lack accurate responses and may also be activated by harmless stimuli perceived erroneously as dangerous. The traumatic terror is stored in implicit memory and may be set off by a conditioned stimulus linked to a previous danger situation. In the panic attack, the traumatic event is created by the imagination and this construction (a micro‐delusion), built in loneliness and anxiety, has the same power as the real trauma. A mutual psychosomatic short‐circuit between body and psyche, in which terror reinforces the somatic reactions and the psychic construction, is established. Therefore, it is important to highlight these constructions in order to analyse and transform them. In the second part of the article the author reviews the main psychoanalytical theories about panic attacks, stressing how, in his opinion, panic attack is a consequence of the breakdown of the defence organisation at various levels and may appear during periods of life crisis. Two patients suffering from a deficit of personal identity are presented. The various organisations and the different levels (biological, neuroscientific, associative, traumatic) of the panic attack determine different kinds of therapeutic approaches (pharmacological, cognitive and psychoanalytical). While the psychopharmacological treatment is aimed at reducing the neurovegetative reaction and the cognitive method is attempting to correct the associative and perceptive processes of fear signals, psychoanalytical therapy represents both a specific means to free patients from panic attacks as well as an indispensable route for their emotional growth.  相似文献

This article contributes to our understanding of popular thinking about happiness by exploring the work of David E Kelley, the creator of the television program Ally McBeal and an important philosopher of happiness. Kelley's major points are as follows. He is more ambivalent than is generally the case in popular philosophy about many of the traditional sources of happiness. In regard to the maxim that money can't buy happiness he gives space to characters who assert that there is a relationship between material comfort and happiness, as well as to those that claim the opposite position. He is similarly ambivalent about the relationship between loving relationships and happiness; and friendships and happiness. In relation to these points Kelley is surprisingly principled in citing the sources that he draws upon in his thinking (through intertextual references to genres and texts that have explored these points before him). His most original and interesting contributions to popular discussions of the nature of happiness are twofold. The first is his suggestion that there is a lot to be said for false consciousness. He presents situations where characters choose wilfully to ignore reality and instead to live in fantasy worlds where they are happy. Rather than condemning such behaviour, Kelley presents it as understandable, attractive and perhaps even heroic. The second is his proposal that happiness should be seen as an effect of bodily performance rather than an expression of the authentic inner self -- if one forces oneself to smile, happiness may follow. Ultimately Ally McBeal presents a multifaceted popular account of the nature of happiness, where the various positions explored cannot all be reconciled; and where ironic ambivalence is the key tone.  相似文献
