ABSTRACT There are increasing concerns globally about the mental health of students. In the UK, the actual incidence of mental disturbance is unknown, although university counselling services report increased referrals. This study assesses the levels of mental illness in undergraduate students to examine whether widening participation in education has resulted in increases as hypothesised by the UK Royal College of Psychiatrists. Patterns of disturbance across years are compared to identify where problems arise. Students (N=1197) completed the General Health Questionnaire-28 either on the first day at university or midway through the academic year for first, second and third year students. Rates of mental illness in students equalled those of the general population but only 5.1% were currently receiving treatment. Second year students reported the most significant increases in psychiatric symptoms. Factors contributing to the problem are discussed. 相似文献

The study explored and described the lived experiences of single mothers and described guidelines for psychiatric nurses to facilitate the mental health of single mothers subjected to aggression by their adolescent sons. Participants were a purposive sample of 8 (single mothers, aged between 40 and 50 years, 7 mothers were employed and 3 were professionally trained, 4 mothers were domestic workers and 1 earned living by selling vegetables). The mothers attended the outpatient department at the National Psychiatric Hospital in Swaziland because of being exposed to aggression by their adolescent sons and had adolescent sons living with them 6 months prior to as well as during the study. Data were collected using individual in-depth phenomenological interviews, direct observation and field notes. Analyses used thematic coding. Results reflected that single mothers experienced their adolescent sons as disrespectful and violent, they had negative experiences about their adolescent sons which resulted in anxiety related symptoms, they had negative experiences of self and experienced poor relationships with their neighbours. Guidelines concerning intra- and interpersonal challenges were formulated for the psychiatric nurse to facilitate the mental health of these mothers. 相似文献

Hegemonic mental health service approaches stress the need for ‘accurate’ diagnosis in order to understand what the correct ‘treatment’ should be. I review the evidence on how far diagnosis in psychiatry has helped advance scientific knowledge and clinical practice and conclude that it hasn’t. I then examine how National Institute for Health and Care Excellence (NICE) guidelines in relation to children’s behaviour problems reflects the lack of progress that results from adopting a diagnostic approach. Taking mental health practice beyond reliance on narrow non-evidence-based diagnostic algorithms can be developed by adopting existing projects such as the Partners for Change Outcome Management Systems project. 相似文献

AbstractCentralized Off-Unit Programing With Extended hours (COPE) is a recovery-oriented model of service that aims to address limited occupational engagement and social isolation among inpatient clients with serious mental illness. This study explored the self-reported outcomes of 12 clients using semi-structured interviews and pre- and post- administration of the Personal Recovery Outcome Measure. These outcomes were supported by significant improvements in personal recovery and included sense of choice, occupational engagement, socialization, change in environment, skill learning, and improved sense of wellness. These findings provide preliminary evidence on the COPE model of service and its potential role in promoting personal recovery. 相似文献

ABSTRACT Tobacco addiction and obsessive-compulsive symptoms (OCS; intrusive thoughts or impulses that cause distress and rituals) are both mediated by compulsivity and negative reinforcement. Little evidence exists to guide theory, research, treatment, and population-based prevention of this co-occurrence. We propose a conceptual model of OCS-smoking co-occurrence in which smokers with elevated OCS capitalize on smoking to assuage OCS. This secondary analysis examined associations between OCS levels and self-reported smoking motives that are model-concordant: i) negative reinforcement—smoking for relief; ii) sensorimotor—benefits of behaviors and sensations of the tobacco self-administration ritual; and iii) habitual—smoking without conscious control. A community sample of cigarette smokers enrolled in a cessation trial (N = 458; 47.2% female; M age = 36.9; SD = 13.6) completed pre-quit self-report measures of OCS levels and smoking motives. Regression models adjusted for sociodemographic covariates and non-OCS psychopathologies indicated that OCS level was positively associated with each model-concordant motive. OCS level was also correlated with tobacco dependence severity and greater withdrawal symptom severity experienced during previous quit attempts. Those with higher OCS report greater motivation to smoke for negative reinforcement, sensorimotor behavioral-ritualistic, habit/automaticity, and stimulation reasons. Further examination of the proposed model of OCS-smoking co-occurrence is warranted to guide theory and intervention for this population. 相似文献

At the onset of World War II, both military and civilian psychiatrists were keen on designating internal factors, such as, cowardice, an overbearing mother, or a henpecked father as determinants of war neuroses. By the end of the war, the notion that anyone could break down under extreme pressure displaced most other explanations of war neuroses. In this paper, using feminist emotional geographies as a framework, I look at how love contributed to this shift. I read three types of texts created through the practices engaged by military psychiatrists in the Canadian Army during World War II at three different sites—in units treating only exhaustion, at a convalescent depot, and at a field dressing station. These texts as both the outcome and record of Canadian military psychiatric practices in World War II form the basis upon which I read how love as a machine (a lá Gilles Deleuze and Félix Guattari) passes through both psychiatry and the military as it contributes to enacting a reality in practice (a lá Annemarie Mol). 相似文献
