Sleep disturbance is a core component in posttraumatic stress disorder (PTSD). Although cognitive-behavioral treatments for PTSD reduce the severity of sleep symptoms, they do not lead to complete remission. The present study examines the impact of Cognitive Processing Therapy (CPT) and Prolonged Exposure (PE) on subjective measures of sleep disturbance from treatment randomization through long-term follow-up (LTFU). Participants were 171 female rape victims with PTSD who were randomly assigned to CPT, PE, or Minimal Attention (MA). After 6-weeks, the MA group was randomized to CPT or PE. Sleep symptoms were assessed at baseline, post-MA, post-treatment, 3-months, 9-months and LTFU using the Pittsburgh Sleep Quality Index (PSQI) and nightmare and insomnia items from the Clinician Administered PTSD Scale. Change in sleep during MA, from pre- to post-treatment for CPT and PE, and from post-treatment through LTFU was assessed using piecewise hierarchical linear modeling with the intent-to-treat sample. Controlling for medication, sleep improved during CPT and PE compared to MA, and treatment gains were maintained through LTFU. CPT and PE were equally efficacious and improvements persist over LTFU, yet, neither produced remission of sleep disturbance. Post-traumatic stress disorder (PTSD) is of great interest to public health, due to the high burden it places on both the individual and society. We meta-analyzed randomized-controlled trials to examine the effectiveness of early trauma-focused cognitive-behavioral treatment (TFCBT) for preventing chronic PTSD. Systematic bibliographic research was undertaken to find relevant literature from on-line databases (Pubmed, PsycINFO, Psyndex, Medline). Using a mixed-effect approach, we calculated effect sizes (ES) for the PTSD diagnoses (main outcome) as well as PTSD and depressive symptoms (secondary outcomes), respectively. Calculations of ES from pre-intervention to first follow-up assessment were based on 10 studies. A moderate effect (ES = 0.54) was found for the main outcome, whereas ES for secondary outcomes were predominantly small (ES = 0.27–0.45). The ES for the main outcome decreased to small (ES = 0.34) from first follow-up to long-term follow-up assessment. The mean dropout rate was 16.7% pre- to post-treatment. There was evidence for the impact of moderators on different outcomes (e.g., the number of sessions on PTSD symptoms). Future studies should include survivors of other trauma types (e.g., burn injuries) rather than predominantly survivors of accidents and physical assault, and should compare early TFCBT with other interventions that previously demonstrated effectiveness. 相似文献

In 2 experiments, the effects of mental stress on limb stiffness were investigated. The relative contribution to arm stiffness of individual muscle activity, co-contraction, muscle reflexes, and postural adjustments were examined. In each experiment, participants (N = 24, Experiment 1; N = 16, Experiment 2) held their supinated hand under a tray that they were required to return to horizontal after it had been suddenly released. Electromyographic activity in the biceps and triceps muscles was recorded, as were elbow and wrist angles and tray displacement. In Experiment 1, mental arithmetic stress was shown to lead to decreased tray displacement (i.e., increased resistance) compared with displacements under the control, unstressed condition, as well as to increased elbow flexion before tray release. In Experiment 2, the increased resistance to perturbation caused by mental stress was found to be independent of initial elbow angle, but to vary as a function of the amount of upward force exerted before tray release. The authors conclude that stress-induced increases in limb stiffness result from changes in the initial position of the elbow, specified by its angle, together with the initial force exerted by participants to counteract the mechanical perturbations. 相似文献

The relation of self-compassion to physical and psychological well-being was investigated among 182 college students. The self-compassion scale was delineated into three composites, following the proposition by Neff that self-compassion consists of three main components: self-judgment versus self-kindness (SJ–SK), a sense of isolation versus common humanity (I–CH), and over-identification versus mindfulness (OI–M). Findings support the association between self-compassion and psychological and physical well-being, but the composites demonstrate different influences. SJ–SK and I–CH were predictive of both depressive symptomatology and physical well-being, and SJ–SK and OI–M were predictive of managing life stressors. The results of this study support and expand prior research on self-compassion. 相似文献

The present study extends the feedback-seeking behavior literature by investigating how supervisor-related antecedents (i.e., supervisors’ expert power, reflected appraisals of supervisors, and supervisors’ emotional intelligence) influence subordinates’ negative feedback-seeking behavior (NFSB) through different cost/value perceptions (i.e., expectancy value, self-presentation cost, and ego cost). Using data collected from 216 supervisor-subordinate dyads from various industries in Taiwan, we employ structural equation modeling analysis to test our hypotheses. The results show that expectancy value mediates the relationship between supervisor expert power and subordinates’ NFSB. Moreover, self-presentation cost mediates the relationship between reflected appraisals of supervisors’ and subordinates’ NFSB. Theoretical and practical implications of this study are also discussed. 相似文献

In this study, we investigated the relationship of cognitive tempo to psychological differentiation and locus of control. The 103 male and female undergraduate students who participated in this study were administered the adolescent and adult version of the Matching Familiar Figures Test, the Group Embedded Figures Test, and the Adult Nowicki-Strickland Internal-External Scale. Using multiple regression analyses, we found that field dependence and independence were significantly related to both cognitive tempo latency and cognitive tempo errors. Contrary to expectation, no relationship was found between locus of control and cognitive tempo. We concluded that psychological differentiation is highly useful in the direct prediction of cognitive tempo, although locus of control is not. Overall, the value of examining cognitive tempo latency and errors separately was upheld. The importance of these relationships is discussed. 相似文献

We investigated the relation between the Rotter (1966) locus of control concept and involvement in automobile accidents. Subjects were 184 college students who completed a survey measuring accident involvement, the Rotter scale, and scales featuring beliefs and behaviors in traffic situations that would be expected from internally oriented persons. No significant relation was found between the Rotter scale and traffic accidents. Accident involvement was best explained by internal beliefs about accident control, and the reported number of near-miss accidents per week. A path diagram relating survey variables is included. 相似文献

The authors investigated the psychological adjustment of 37 British women whose adolescent children survived the cruise ship Jupiter's sinking in 1988, about 6 years previously. They compared these women with a group of widows (N = 18) and a group of women who had suffered no major negative life event (N = 15). Psychological adjustment of the 37 women was assessed with the Schedule for Affective Disorders and Schizophrenia Lifetime Version (SADS-L; R. L. Spitzer & J. Endicott, 1975) and various standard questionnaires. The women whose children had been involved in the disaster were found to have suffered a greater number of incidents of psychological distress in the period since the disaster than the women who had suffered no major negative life events but fewer incidents of psychological distress than the widows. This experiment tested the hypothesis that there would be a significant increase in powerlessness scores over a 10 year period. The sample was drawn from a population of college students originally tested in 1964. At the 10 year follow-up, all Ss had graduated from college and were either professionally employed (mostly as teachers) or engaged in child-rearing/home activities. The average age of the 1974 sample was 29 years and consisted of 70 females and 30 males. The test instrument was the adult version of Rotter's Internal-External (I-E) Locus of Control Scale which consists of 23 forced-choice items. There were no significant differences on total I-E scores over the 10 year period. However, with use of the Mirels two-factor scoring system, significant increases in social-political control were found. No significant differences in personal control were found. It was concluded that care should be taken in utilizing cross-sectional findings and that utilizing multidimensional scores suggests different results for previously published reports. 相似文献

The authors assessed connections among adolescents’ emotional dispositions, negative academic affect, coping strategies, academic stress, and overall grade point average (GPA). A total of 119 ninth through 12th-grade students completed assessments for (a) overall positive and negative moods, (b) GPA, and (c) academically related variables involving stress, negative emotions, and engaged and disengaged coping strategies. Greater negative academic affect and disengaged coping were related to lower GPAs, and disengaged coping mediated the connection between negative academic affect and GPA. By contrast, higher academic stress was related to students’ overall moods, negative academic affect, and disengaged coping; disengaged coping mediated the connection between academic stress and negative overall moods. Discussion focused on the especially problematic nature of disengaged academic coping. 相似文献
