Stroke remains a leading cause of disability, with survivors experiencing long-term decrements in independence and quality of life. Occupational therapists (OTs) employ numerous neurorehabilitative treatment approaches to remediate impairments that are impeding performance. OTs also use physical agent modalities to facilitate increased participation and success in therapy. One such modality is transcranial direct current stimulation (tDCS), a form a noninvasive brain stimulation that can be overlaid onto task practice and delivers a constant, low intensity current into the brain. tDCS is safe, portable, and efficacious in remediating a variety of deficits, yet is not consistently incorporated into clinical practice. The authors discuss the mechanisms, safety, evidence, and potential applications of tDCS to enhance outcomes for this growing population.  相似文献

To allow different views on motor learning to inform rehabilitation research, the authors aimed to explicate a frequently missed yet fundamental difference in starting point of such views. By considering how rehabilitation in practice answers the question of what parts an activity consists of, reductive and emergent approaches to motor learning are identified and traced throughout rehabilitation practice. The authors show that when a task is cut up along reductive dimensions while also apparently relying on emergent components, this unequally favors the reductive approach and acts to limit the views on motor learning available. By showing the approaches in practice, the authors hope to inspire an awareness that brings both approaches the opportunity to independently inform research so that new theories and practices can proliferate.  相似文献

In four experimental studies, we explored the effect of consistency between central group values and the content of group‐directed praise on group‐based esteem, group identification, and willingness to express attitudes and intentions (in)consistent with the content of praise. Study 1 used pre‐existing groups with clearly defined central values, Study 2 relied on individual differences in perceptions of central group values within the same social group, and Studies 3 and 4 manipulated the centrality of group values experimentally. The results demonstrated that identity‐inconsistent praise resulted in lower group‐based esteem (Studies 1–4), and reduced group identification (Study 4), as compared to identity‐consistent praise. In addition, in all studies, identity‐inconsistent praise led to stronger willingness to reaffirm (the ignored) central group characteristics than identity‐consistent praise. The results are consistent with extensions of the self‐verification approach to the collective self.  相似文献

ObjectiveTo evaluate the effect of low intensity recorded maternal voice on the physiologic reactions of healthy premature infants in the neonatal intensive care unit (NICU).MethodsPhysiologic responses of 20 healthy preterm infants in the NICU of Shariati Hospital, Tehran, were obtained during a 15 min intervention including three 5 min periods (no-sound control, audio recorded playback of mother’s voice, no-sound post-voice). The intervention was presented three times a day for three consecutive days. During each intervention, oxygen saturation (%, OSPR), heart rate (HR), and respiratory rate (RR) were recorded at 1 min intervals over the 15 min and then averaged over each 5 min period, resulting in 3 averages for each variable for each intervention.ResultsRepeated Measures Analysis of Variance were employed to examine each variable separately. Over the three days, comparison of oxygen saturation over each of the three periods (before, during voice, after) revealed an increase in oxygen saturation during the voice period, compared to the pre-voice period, which persisted over the post-voice period; there were no differences between the voice and post-voice periods. Analyses of the HR and RR data showed a decrease in both variables during the voice period compared to the pre-voice period which persisted over the post-voice period. Again, there were no differences between the voice and post-voice periods.ConclusionExposure to low intensity recorded maternal voice has positive effects on the preterm infants ‘physiologic responses.  相似文献

IntroductionPhysical therapists should implement practice conditions that promote motor skill learning after neurological injury. Errorful and errorless practice conditions are effective for different populations and tasks. Errorful learning provides opportunities for learners to make task-relevant choices. Enhancing learner autonomy through choice opportunities is a key component of the Optimizing Performance through Intrinsic Motivation and Attention for Learning (OPTIMAL) theory of motor learning. The objective of this study was to evaluate the interaction between error opportunity frequency and OPTIMAL (autonomy-supportive) practice conditions during stepping sequence acquisition in a virtual environment.MethodsForty healthy young adults were randomized to autonomy-supportive or autonomy-controlling practice conditions, which differed in instructional language, focus of attention (external vs internal) and positive versus negative nature of verbal and visual feedback. All participants practiced 40 trials of 4, six-step stepping sequences in a random order. Each of the 4 sequences offered different amounts of choice opportunities about the next step via visual cue presentation (4 choices; 1 choice; gradually increasing [1-2-3-4] choices, and gradually decreasing [4-3-2-1] choices). Motivation and engagement were measured by the Intrinsic Motivation Inventory (IMI) and the User Engagement Scale (UES). Participants returned 1–3 days later for retention tests, where learning was measured by time to complete each sequence. No choice cues were offered on retention.ResultsParticipants in the autonomy-supportive group outperformed the autonomy-controlling group at retention on all sequences (mean difference 2.88s, p < .005, t[6835] = 3.42). Participants in both groups had the most difficulty acquiring the decreasing choice (4-3-2-1) sequence (p < .001, t[6835] = −4.26) and performed most poorly on the errorful (4 choice) sequence (p < .034, t[6835] = 2.65) at retention. Participants in the autonomy-supportive group performed best at retention on the increasing choice (1-2-3-4) sequence (p < .033, t[6835] = −2.7). Participants in both groups who reported greater attention to the task on the UES Average Focused Attention subscale during acquisition had poorer retention performance, particularly for the decreasing choice (4-3-2-1) sequence (p < .005, t(6835) = 3.39). Participants in the autonomy-supportive group reported significantly higher overall motivation (p = .007, t(38) = 0.728, d = 0.248) on the IMI as compared to the autonomy-controlling group.ConclusionIndividual benefits of errorless learning and autonomy-supportive practice conditions, with an interaction effect for practice that begins errorless but adds increasing error opportunities over time, suggest that participants relied on implicit learning strategies for this full body task and that feedback about successes minimized errors and reduced their potential information-processing benefits. Subsequent work will continue to examine how assigning a positive versus a negative quality to error provision influences the benefits of errorful learning in a variety of tasks.  相似文献

Performance of fine motor skills (FMS) assessed by a clinical test battery has been associated with reading achievement in school-age children. However, the nature of this association remains to be established. The aim of this study was to assess FMS in children with reading difficulties using two experimental tasks, and to determine if performance is associated with reduced binocular function. We hypothesized that in comparison to an age- and sex-matched control group, children identified with reading difficulties will perform worse only on a motor task that has been shown to rely on binocular input. To test this hypothesis, motor performance was assessed using two tasks: bead-threading and peg-board in 19 children who were reading below expected grade and age-level. Binocular vision assessment included tests for stereoacuity, fusional vergence, amplitude of accommodation, and accommodative facility. In comparison to the control group, children with reading difficulties performed significantly worse on the bead-threading task. In contrast, performance on the peg-board task was similar in both groups. Accommodative facility was the only measure of binocular function significantly associated with motor performance. Findings from our exploratory study suggest that normal binocular vision may provide an important sensory input for the optimal development of FMS and reading. Given the small sample size tested in the current study, further investigation to assess the contribution of binocular vision to the development and performance of FMS and reading is warranted.  相似文献

Infants' differential vocal response (DVR) towards their mother and a female stranger at 3 months of age has been predominantly investigated as an index of early cognitive functioning. The present study explored the relationship between DVR and different infant and mother indicators of the developing relationship quality in a sample of 23 mother–infant dyads. Mother–infant interactions and stranger–infant interactions were videotaped during home visits when the infants were 3 months old. At the age of 12 months, infants' behaviour was assessed in the Ainsworth's strange situation. In both assessments, mothers noted the infants' behavioural states on 3 successive days. Results revealed a specific relation between DVR and the emotional atmosphere of the early mother–infant interaction. Particularly, DVR was less pronounced in favour of the mother when the mother displayed emotional expressions defined as rejection. DVR is discussed as an index that may indicate both infant emotional engagement and infant cognitive competencies. Copyright © 2005 John Wiley & Sons, Ltd.  相似文献

In the present study the effects of a wide range of doses of the dopamine release inhibitor CGS 10746B were evaluated in spontaneous activity and in aggressive behaviour using the paradigm of isolation‐induced aggression. The two higher doses (8 and 16 mg/kg) produced a decrease in spontaneous motor activity. Antiaggressive effects were observed after administration of doses from 4 mg/kg upwards. At this dose, CGS 10746B diminished threat and attack, and although an increase in immobility was observed, no impairment of other motor behaviours was presented. With higher doses, aggression was practically abolished but with a concomitant effect on many other behaviours. When animals were separated depending on their latency of attack, those that showed a long attack latency (LAL) presented a stronger response to 4 mg/kg than those that had a short attack latency (SAL), which were not affected in their aggression by this dose. We can conclude that presynaptic dopamine function is necessary for the normal expression of aggressive behaviours, since CGS 10746B reduces aggression at doses that do not affect spontaneous motor activity. The purpose of this longitudinal study was to describe object‐centred interactions between mothers and their 2–4‐month‐old infants, before and during the emergence of reaching and grasping movements. We hypothesized that when reaching movements emerge at around 3 months, mothers alternate between attention stimulation and reaching stimulation, before joint actions between mother and infant develop around objects. Twelve dyads were recorded when infants were 2 months, 3 months and 4 months. The interactive sessions lasted 5 min. Three age‐appropriate toys the infant could handle were available to the mother. A principal component analysis (PCA) was performed on verbal and non‐verbal maternal behaviours, motor infant behaviours and co‐occurrences of those behaviours. The developmental course of prehension in infants when playing with their mother follows similar pathways, as was described when they are observed alone. Mothers appeared to early scaffold prehension skills by verbal and non‐verbal means. Moreover, maternal behaviours change according to the infant's behaviour, and conversely, infant's behaviours influence maternal behaviours: mother plays first an active part in joint action, while later on, the infant achieves joint action when motor skills develop. The control exerted by a stimulus associated with an extinction component (S−) on observing responses was determined as a function of its temporal relation with the onset of the reinforcement component. Lever pressing by rats was reinforced on a mixed random-interval extinction schedule. Each press on a second lever produced stimuli associated with the component of the schedule in effect. In Experiment 1 a response-dependent clock procedure that incorporated different stimuli associated with an extinction component of a variable duration was used. When a single S− was presented throughout the extinction component, the rate of observing remained relatively constant across this component. In the response-dependent clock procedure, observing responses increased from the beginning to the end of the extinction component. This result was replicated in Experiment 2, using a similar clock procedure but keeping the number of stimuli per extinction component constant. We conclude that the S− can function as a conditioned reinforcer, a neutral stimulus or as an aversive stimulus, depending on its temporal location within the extinction component.  相似文献
