Thematic review of family therapy journals 2009

In this article the contents of the principal English‐language family therapy journals published in 2009 are reviewed under these headings: narrative therapy, child‐focused problems, adult‐focused problems, substance abuse across the lifespan, illness across the lifespan, family violence, couples, diversity, developments in systemic practice, training and research.     

Positioning theory as a discursive approach to understanding same‐sex intimate violence

This study focuses on same‐sex intimate violence, recognizing that violence in gay and lesbian relationships is as severe and prevalent as in heterosexual relationships. Positioning theory as a relational approach is used as an alternative to the dominant individual and structural accounts. Intimate violence is seen as produced through the assignment of rights and duties or positioning in couples' conversations. Two gay and two lesbian couples were interviewed. A basic discursive pattern was found from 25 accounts of episodes involving physical violence. Violence ensued only after the initiator of violence had claimed innocence or blamelessness and attributed guilt or blame on the receiver of violence. The findings are discussed in relation to the discursive production of power and violence.     

Personality,spirituality, suicide,and self-injury proneness among lesbian,gay, and bisexual adults

The present study sought to clarify gaps in current knowledge integrating personality, spirituality, and risk for suicide/self-harm among sample of 336 lesbian, gay, and bisexual community members. It was hypothesised that Neuroticism would positively predict, and Extraversion and Agreeableness would negatively predict, measures of suicide and self-injury proneness. Additionally, it was predicted that spirituality, defined as Spiritual Life Integration (SLI) and Social Justice Commitment, would interact with personality traits to attenuate risk for suicide and self-injury. Results supported the role of Neuroticism, and identified an unexpected predictor of Conscientiousness, at the main effect level. Moderation patterns were observed such that Agreeableness and Extraversion interacted with SLI to attenuate risk, such that high levels of each trait and high levels of spirituality were protective against suicide and self-injury proneness. Theoretical and practical implications with emphasis on counselling intervention implementation and future research are discussed.     

“I’m Not Sure I Trust the System Yet”: Lesbian Service Member Experiences With Mental Health Care

The purpose of this research was to understand lesbian service member experiences with mental health care. Individual and organizational factors were explored, including the influence of military policy (e.g., “Don’t Ask, Don’t Tell”) on service member utilization of mental health services. Thirty-seven participants responded to a survey containing 16 open-ended items regarding the impact of “Don’t Ask, Don’t Tell” on one’s professional life, relational life, identity, and willingness to access mental health services. Data were analyzed through an open- and axial-coding and constant comparative method. The findings indicated a lesbian service woman’s likelihood of accessing mental health services was impacted by confidentiality concerns, fear of repercussions, and a sense that military culture lags behind policy changes. Recommendations for therapists included renewed focus on safety through affirmative practices, need for competency in military and lesbian/gay culture, and sensitivity to the effects of systemic oppression on self-esteem. Implications and future research are discussed.     

Prejudice Events and Traumatic Stress among Heterosexuals and Lesbians,Gay Men,and Bisexuals

This mixed-methods study examined associations between prejudice events and post-traumatic stress disorder (PTSD) among 382 lesbians, gays, and bisexuals (LGB) and 126 heterosexuals. Using the Composite International Diagnostic Interview, we assessed PTSD with a relaxed Criterion A1; that is, we allowed events that did not involve threat to life or physical integrity to also qualify as traumatic. We first assessed whether exposure to prejudice-related qualifying events differed with respect to participants' sexual orientation and race. We found that White LGBs were more likely than White heterosexuals to encounter a prejudice-related qualifying event, and among LGBs, Black and Latino LGBs were no more likely than White LGBs to experience this type of event. We then used qualitative analysis of participants' brief narratives to examine prejudice events that precipitated Relaxed Criterion A1 PTSD among 8 participants. Two themes emerged: (a) the need to make major changes, and (b) compromised sense of safety and security following exposure to the prejudice event.     

Dual Marginality: The Impact of Auditory Status and Sexual Orientation on Abuse in a College Sample of Women and Men

This exploratory study examined the relationship between the combination of auditory status and sexual orientation, and experiencing interpersonal violence, as measured by sexual, physical, and psychological abuse. A cross-sectional examination of 1,023 undergraduate students including 222 Deaf and hard-of-hearing students provided the data. Findings revealed that gay, lesbian, and bisexual students who also were Deaf or hard of hearing were more likely to experience sexual, physical, and psychological abuse than those in mainstream majority groups. The combination of auditory minority with sexual minority status for students had the strongest effect for physical and psychological abuse. Implications for policy and future research are provided.     

Internalized Homophobia and Intimate Partner Violence in Young Adult Women’s Same-Sex Relationships

This study examined the relationship between intimate partner violence (IPV), internalized homophobia, and psychological maladjustment in a convenience sample of young adult women. Rates of psychological and physical abuse in women’s same-sex relationships were shown to be commensurate to previously reported rates of violence in the IPV literature; rates of sexual coercion were also ascertained. A relationship was found between Moral and Religious Attitudes Toward Lesbianism (MRATL), a dimension of internalized homophobia, and the perpetration of sexual coercion. As expected, internalized homophobia was connected to psychological maladjustment. In addition, both the perpetration of psychological aggression and physical violence were associated with emotional instability. The study findings are discussed within the current body of literature on IPV.     

In God’s House

Abstract

By naming the ways silence and taboo structure the documentary, In God’s House, the article traces the ways personal narratives are deployed as: (1) a cultural practice to consolidate an “Asian” ethnicity; and as (2) a pedagogical strategy to address the silence of sexual discourse—particularly around LGBTQ issues—in Asian American contexts. As cultural practices, silence and taboo undergird the framework of community—protecting individual honor, securing public face and fortifying webs of relationships that sustain individual and social flourishing. More than a façade for the “inscrutable Asian,” silence illuminates a practice of communal relationality that reconfigures “American” significations of community, sexuality, race and identity.     

Using Contextual Therapy to Treat Depression with Couples

Abstract

Though depression is one of the most common presenting problems treated by systemic therapists, there have been relatively few attempts to describe how specific systemic therapy models can be applied to treat couples dealing with depression. In this paper, we discuss how contextual therapy can be used to treat depression in couples therapy. We identify multidirected partiality as an intervention and as a characteristic. We introduce letter writing and internalized other interviewing as contextual interventions that help to facilitate partiality and understanding between clients. A clinical vignette illustrates our discussed concepts.
