重症手足口病的早期识别与处理

手足口病(hand-foot-mouth disease,HFMD)是由肠道病毒[以柯萨奇A组16型(Coxsackievirus A16,CoxA16)、肠道病毒71型(Enterovirus 71,EV71)多见]引起的急性传染病,而重症多由EV71感染引起。由于EV71的高度嗜神经性,重症者可合并中枢神经系统(central nervous system,CNS)损害、急性肺水肿/出血等。现就重症手足口病的早期识别与处理进行简要综述。

从早期止痛的意义,早期止痛的依据以及传统止痛观念的成因等三个方面论述急腹症早期止痛的必要性和合理性,并探讨早期止痛的策略。提醒医务人员要更新观念,对急腹症患者应尽早实施止痛干预。     

Parallelism in the Early Moist Texts

Parallelism is present everywhere in the early Moist texts: at the syntactic level, at the semantic level, between sentences, between sets of sentences, between argumentative structures. The present article gives many examples of the phenomenon: parallelism of insistence, insistence from top to bottom, insistence from bottom to top, parallelism with symmetry, parallelism involving negation, subcontraries and negation at deeper levels, parallelism of the argumentative structures. Logic is particularly applied to the study of parallelism involving negation. From the point of view of argumentation, it is shown that many of those constructions have an important role in supporting arguments such as: arguments of generalization, a fortiori arguments, arguments of exemplarity, consequentialist arguments, arguments by comparison. This study draws the attention to the importance of argumentation in the study of Moism and gives a new light on the argument by parallelism (mou 侔) in the “Xiaoqu”: It is a natural extension of what we call “parallelism involving negation,” already very common in the early Moist texts.     

早期应激对情绪调节的影响及其神经机制

在个体生命早期遭遇的长期或重大的应激经历会显著增加个体罹患抑郁、焦虑等心理疾患的风险, 而情绪调节能力的损伤是引发这些心理疾患的重要因素之一。以人类为对象的行为实验和调查研究表明, 早期应激不仅会影响日常生活中情绪调节策略的使用, 还会对情绪调节能力造成影响, 其影响方向可能与早期应激的严重程度有关。目前, 大部分研究表明严重的早期应激会损伤情绪调节能力, 但中度的早期应激也可能提高情绪调节能力。更为整合性的研究表明情绪调节能力中介了早期应激和各类疾患之间的关系。进一步, 我们从神经层面上阐述了早期应激对情绪调节相关脑区和神经环路的影响。未来研究应当注意控制无关变量, 进一步探究不同早期应激对于情绪调节的影响。     

The language of absence

This paper will focus on the concept of 'absence', which describes a continuum of non-responsiveness and misattunement of the environment in the stage of absolute dependence; it refers to concepts like lack, failure, non-recognition, impingement, neglect, tantalizing, ranging to mental, physical and sexual abuse. An extreme external absence causes shock and fear. The automatic survival response is an inner absence, an intrapsychic absence, a dissociation of parts of the self. The external and the inner absence are the negative image of each other. The concept of absence points to the synchronicity of outer and inner reality and portrays the non-responded-to needs of the self. This point of view of the development of psychopathology of the self on the basis of massive dissociation is inherently an intersubjective-field-theory. As the inner absence is created as a reaction to an absence of the other, in analysis - the analyst has an active role in reviving it. This paper will explore the language of absence, that is, the derivatives and consequences of these situations in the inner realm, and in the relations with the analyst. It is the author's contention that understanding and speaking this language has important clinical and technical implications. Understanding the language of absence enables the analyst to recognize its intersubjective and its intrapsychic presence, to provide an environment that allows for its revival, and to facilitate and regulate the annihilation anxiety that awakens when dissociated self-states are experienced. When the absence is present, i.e. when the traumatic experience and the dissociated reactions to it are experienced in an attuned relationship, it is rendered with meaning, symbolization, and validation, and enables the survival mode of dissociation to be relinquished.

Influenced by the methods and practices of Hippocrates, the Asclepius cult used herbal formulae and medicinal applications intricately connected with Asclepius cult rituals and worship. An understanding of the types of herb and medical applications surrounding their use by the cult aids understanding of the inner world and symbolism of ancient dreams.Jeffrey B. Pettis teaches in the field of New Testament Studies at Fordham University, New York City. His focus is the interpretation and use of ancient dreams in early Christianity and the Greco-Roman world. He has lived and traveled in Greece, Egypt and Turkey, studying the design and culture of ancient temples and the sanctuaries of Asclepius, the deity of the ancient dream cult. He received his Ph.D. from Union Seminary in 2004.

Creativity Research Overlooks the Study of Resilience among Young Children: A Bibliometric Network Review

Creativity researchers are increasingly interested in understanding when, how, and for whom creativity can be beneficial. Previous reviews have demonstrated that creativity research largely ignores the study of its impact on factors that promote health, and well-being among populations of adults. It is unclear, in fact, whether this gap in research also extends to creativity research among young children. This paper addresses this issue. Early childhood is a crucial stage for the cognitive development of young children who remain highly sensitive to stress, and adversity. It is therefore essential to identify and promote factors that are beneficial to early childhood resilience, thereby contributing to documenting more of the effects of creative activities on positive outcomes. This paper presents a review with a bibliometric analysis of 1000 randomly selected articles from the Web of Science, without bias towards any specific peer-reviewed journal. The analysis of 454 included articles shows that approximately 80% of the included studies focus on creativity as an outcome (replicating previous findings with a larger sample), with only 3.78% investigating creative activities as predictors among young children. In this small percentage, most of the studies addressed creative activities in young children related to resilience outcomes.

既往研究已证实早期经历与个体成年后的行为密切相关,然而早期经历对个体成年后的行为产生长期影响的分子生物机制并不清楚。近年来越来越多的研究表明表观遗传因素在早期经历调控成年个体行为中发挥了重要功能。表观遗传是研究没有DNA序列变化,但是可遗传和可逆转的基因组功能的调控。本文试图从早期经历参与调控个体成年后行为表现以及基因表达的相关表观遗传学修饰进行综述。
