Abstract

The main point of this paper is that a client's suitability for brief or time-limited therapy is determined by various factors, including context. There is still agreement on the validity of Malan's selection criteria, which included ‘mild illness, recent onset, high motivation and response to trial interpretation’, yet many additional issues have since emerged and need to be considered. For instance, the counsellor's suitability, training and work experience, the counsellor's assessment skills and ability to establish a dynamic focus, the clients' capacity for self-reflection, their ego strength and their response to a trial therapy in the first session. Then there is the importance of the various contexts in which nowadays much brief counselling is offered free to clients, whether in education, at the workplace, in primary healthcare settings or by charitable organizations. This means that issues of money and markets have come to the fore and an initial differential assessment needs to be carried out in order to decide which method or model of therapy is best for the client. Matching the counsellor's personality to the needs and to the pathology of the client, and matching the treatment to the client's developmental stage or life stage crises are other aspects of the work, which determine the issue of suitability. In the end as always there is much that remains unknown about what works for whom and how the client's decision to take up help is made.  相似文献

This article discusses why it is crucial in the cross-cultural clinical encounter to practice from both Jessica Benjamin's intersubjective frame as well as relational theory when the therapist is from the dominant White culture and the client is African American. Intersubjective and relational theories are utilized to illustrate how it is in this clinical situation, however, that some therapists may find themselves avoiding the use of a more relational practice. A case illustration is presented from the lens of Winnicott's theory of object relations to convey the dynamic complexities that may impede a therapist from practicing out of these (more relational) frames and then the case is reconceptualized from both a relational and intersubjective perspective.  相似文献

Abstract. This paper examines two views of free will. It looks first at the fourteenth‐century religious insights of John Duns Scotus, one of history's seminal thinkers about free will. It then examines what current neuroscience tells us about free will. Finally, it summarizes the past and present views and concludes by answering two questions: Does free will refer to an absence of external constraint, or does it refer to a human ability to decide in an acausal manner?  相似文献

In this paper, I explore how we might link ideas about clinical facts to current issues in child psychotherapy research. I consider what our understanding of clinical facts might contribute to our research methods and how our research methods might better represent the clinical facts. The paper introduces a selection of psychoanalytic writers' formulations of the concept and describes some of the debates about the shortcomings of the traditional style of case reporting. The importance of keeping emotional experience central in our research is discussed. I describe a research method that I believe has the potential to capture and describe some of the complicated processes of change in psychotherapy. This is the concept of the ‘turning point’ session. Introducing the paradigm of complexity theory, I briefly explore how we might think about and understand the relationship between processes revealed through detailed analysis of a single session and change over a longer period of therapy.  相似文献

This paper focuses on the divergence behaviour of the successive geometric mean (SGM) method used to generate pairwise comparison matrices while solving a multiple stage, multiple objective (MSMO) optimization problem. The SGM method can be used in the matrix generation phase of our three‐phase methodology to obtain pairwise comparison matrix at each stage of an MSMO optimization problem, which can be subsequently used to obtain the weight vector at the corresponding stage. The weight vectors across the stages can be used to convert an MSMO problem into a multiple stage, single objective (MSSO) problem, which can be solved using dynamic programming‐based approaches. To obtain a practical set of non‐dominated solutions (also referred to as Pareto optimal solutions) to the MSMO optimization problem, it is important to use a solution approach that has the potential to allow for a better exploration of the Pareto optimal solution space. To accomplish a more exhaustive exploration of the Pareto optimal solution space, the weight vectors that are used to scalarize the MSMO optimization problem into its corresponding MSSO optimization problem should vary across the stages. Distinct weight vectors across the stages are tied directly with distinct pairwise comparison matrices across the stages. A pairwise comparison matrix generation method is said to diverge if it can generate distinct pairwise comparison matrices across the stages of an MSMO optimization problem. In this paper, we demonstrate the SGM method's divergence behaviour when the three‐phase methodology is used in conjunction with an augmented high‐dimensional, continuous‐state stochastic dynamic programming method to solve a large‐scale MSMO optimization problem. In 2 experiments, dynamic systems theory predictions concerning intrinsic dynamics and variability of bimanual coordination were examined at different developmental stages. In Experiment 1, ten 4-, 6-, 7-, 8-, and 10-year-old children and adults performed unimanual dominant, unimanual nondominant, and bimanual continuous circle drawing. All tasks were performed at the participants' preferred rate, size, and mode of coordination. The 4-, 6-, and 7-year-old children produced larger circles with longer durations than those of the 8- and 10-year-olds and the adults. That finding demonstrates that younger children display different intrinsic dynamics than older children and adults. The 4-, 6-, and 7-year-old children also displayed more variability in bimanual coordination (more time in less stable patterns of coordination, higher standard deviation in relative phase) and produced more transitions between coordination patterns than the 8- and 10-year-olds and the adults. In Experiment 2, the same participants performed bimanual circles at increasing rates. Consistent with predictions of the HKB model (H. Haken, J. A. S. Kelso, & H. Bunz, 1985), the number of transitions decreased as speed increased. Some support was found for the notion that age-related variables of attention and rate contribute to the increased variability in young children's bimanual coordination.  相似文献

In this commentary, the authors elaborate on the issue of going beyond curve fitting to the drawing of inferences about motor learning. They argue that the agenda of A. Heathcote and S. Brown (2004) is largely a theory-free, curve-fitting enterprise that can be useful for certain aspects of describing behavior change, but that its gold standard of percentage of variance accounted for can also be misleading in its relevance to the theory of learning. Clearly, analysis methods are necessary and some are better than others, but the researcher can more fully exploit the relevance of methods to the construct with a priori theorizing than with a data-driven strategy of maximizing percentage of variance accounted for in curve fitting.  相似文献

Backward slope walking was considered as a practical rehabilitation and training skill. However, its gait stability has been hardly studied, resulting in its limited application as a rehabilitation tool. In this study, the effect of walking direction and slope grade were investigated on the local dynamic stability of the motion of lower extremity joints and trunk segment during backward and forward upslope walking (BUW/FUW). The local divergence exponents (λS) of 16 adults were calculated during their BUW and FUW at grades of 0%, 5%, 10%, and 15%. Mean standard deviation over strides (MeanSD) was analyzed as their gait variability. Backward walking showed larger λS for the abduction-adduction and rotational angles of knee and ankle on inclined surface than forward walking, while λS for hip flexion-extension angle at steeper grades was opposite. No grade effect for any joint existed during BUW, while λS increased with the increasing grade during FUW. As to the trunk, walking direction did little impact on λS. Still, significant larger λS for its medial-lateral and vertical motion were found at the steeper grades during both FUW and BUW. Results indicate that during BUW, the backward direction may influence the stability of joint motions, while the trunk stability was challenged by the increasing grades. Therefore, BUW may be a training tool for the stability of both upper and lower body motion during gait.  相似文献

Changes to intersegmental locomotor control patterns may affect body stability. Our study aimed to (a) characterize upper body dynamic stability in response to the unilateral addition of mass to the lower extremity and (b) evaluate the efficacy of 2 different stability measures commonly used in the literature to detect resulting symmetrical step pattern modifications across the weighted segments (spatial) and between epochs of the gait cycle (temporal). Young adults walked on a treadmill while unloaded or with weights applied unilaterally to their foot, shank, or thigh. Both margin of stability and local dynamic stability (LDS) estimates detected similar trends of distal segment weighting resulting in more unstable upper body movement compared to proximal weighting; however only LDS detected anteroposterior changes in upper body stability over time.  相似文献
