BackgroundIntensive task-specific training is promoted as one approach for facilitating neural plastic brain changes and associated motor behavior gains following neurologic injury. Partial body weight support treadmill training (PBWSTT), is one task-specific approach frequently used to improve walking during the acute period of stroke recovery (<1 month post infarct). However, only limited data have been published regarding the relationship between training parameters and physiologic demands during this early recovery phase.ObjectiveTo examine the impact of four walking speeds on stride characteristics, lower extremity muscle demands (both paretic and non-paretic), Borg ratings of perceived exertion (RPE), and blood pressure.DesignA prospective, repeated measures design was used.MethodsTen inpatients post unilateral stroke participated. Following three familiarization sessions, participants engaged in PBWSTT at four predetermined speeds (0.5, 1.0, 1.5 and 2.0 mph) while bilateral electromyographic and stride characteristic data were recorded. RPE was evaluated immediately following each trial.ResultsStride length, cadence, and paretic single limb support increased with faster walking speeds (p ⩽ 0.001), while non-paretic single limb support remained nearly constant. Faster walking resulted in greater peak and mean muscle activation in the paretic medial hamstrings, vastus lateralis and medial gastrocnemius, and non-paretic medial gastrocnemius (p ⩽ 0.001). RPE also was greatest at the fastest compared to two slowest speeds (p < 0.05).ConclusionsDuring the acute phase of stroke recovery, PBWSTT at the fastest speed (2.0 mph) promoted practice of a more optimal gait pattern with greater intensity of effort as evidenced by the longer stride length, increased between-limb symmetry, greater muscle activation, and higher RPE compared to training at the slowest speeds. 相似文献

Based on our clinical experience with anorexia, we propose to distinguish two paradoxical aspects of this disorder. On the one hand, anorexia may serve as a shield and support to open up a space for subjectivity, promoting a separation from the mother. Eros is responsible for this defense of subjectivity and of the assumption of one's own wish. But the anorexic individual may, on the other hand, be subjected to the death drive and to the voice of a sadistic and ferocious superego that demands immediate obedience. The death drive, present in different combinations with Eros, is an obstacle to the work of the psychoanalyst. 相似文献

The aim of this cross-sectional study was to investigate attitudes of New Zealanders towards death and dying. We administered an online version of Collett–Lester Fear of Death Scale and Concerns About Dying Instrument subscales to a representative sample of the New Zealand population. In total, 1001 people responded to the survey. Women reported more anxiousness concerning their own death (p?=?.001) and dying at a young age (p?>?.001). Women also showed more agreement towards spiritual aspects of dying (p?>?.001). Single female respondents worry more about dying (p =?.02) especially when young (p?=?.003). Single participants (p >?.001) aged 18 and 29 (p?>?.001) reported higher anxiety for Items relating to the death of others especially concerning never being able to communicate with the person again. Our findings show that marital status and gender strongly predict higher levels of death anxiety among New Zealanders. This is probably due to the cultural identity of those sampled. 相似文献

The study sought to examine the role of frontal lobe functioning in focal prospective memory (PM) performance and its relation to PM deficit in older adults. PM and working memory (WM) differences were studied in younger aged (n = 21), older aged (n = 20), and frontal injury (n = 14) groups. An event-based focal PM task was employed and three measures of WM were administered. The younger aged group differed from the other two groups in showing significantly higher scores on PM and on one of the WM measures, but there were no differences at a statistically significant level between the older aged group and the frontal injury groups on any of the memory measures. There were, however, some differences in correlations with a WM measure between groups. It is concluded that there are similarities and differences in the deficits in PM between older adults and patients with frontal lobe injury on focal as well as nonfocal PM tasks. 相似文献

The present study investigated the influence of direct on-site experience of violent death on the symptoms involved in posttraumatic stress disorder (PTSD) of bereaved Japanese family members (n = 367) in forensic autopsy cases, using a questionnaire survey. Factor analyses of three core symptoms of PTSD confirmed the factor structure. In general linear modeling, the major effect of direct on-site experience of violent death on PTSD score was significant, while manner of death alone did not have such an effect. These findings suggest that direct on-site experience of violent death is essential in the clinical assessment of PTSD-related symptoms of bereaved family members. 相似文献

This paper interprets Hegel’s engagement with tragedy and especially tragic action as an interpretive model for understanding ethical life in complex societies in which independent value spheres collide. Tragic recognition, in contrast to the kind of recognition introduced in the master and slave dialectic, is not based on desire, but arises from the suffering deriving from clashing value spheres. As a coming to terms with one’s finitude, tragic recognition presents an important corrective to the account of mutual recognition that has been the reference point of contemporary interpretations of Hegel’s social and political philosophy. The paper concludes by pointing to some of the limits of tragedy as a universal interpretive framework for modern societies. 相似文献

Recent reports of the discovery of a "God module" in the human brain derive from the fact that epileptic seizures in the left temporal lobe are associated with ecstatic feelings sometimes described as an experience of the presence of God. The brain area involved has been described as either (a) the seat of an innate human faculty for experiencing the divine or (b) the seat of religious delusions.
In fact, religious experience is extremely various and involves many parts of the brain, including some that are prehuman in their evolutionary history and some that are characteristically human. In the continuing integration of such experiences, spiritual formation takes place. Thus the entire human brain might be described as a "God module."
Such a process is only possible because of the brain's complexity. The human brain is the most complex entity for its size that we know of. As used here, complexity is a specialized term denoting the presence of a web of interlinked and significant connections—the more intricate the web, the more complex the entity. Complex systems develop only in a milieu that provides both lawfulness and freedom, and they tend to be self-organizing, becoming more complex and more effective as a result of both inward and outward experience. The evidence suggests that both personal growth and spiritual growth are processes of complexification of character, and of the brain itself.
