要注意中药对肝的损伤作用

含有生物碱类、皂苷类、萜类、毒蛋白类、重金属类成分的中药易致急性肝损伤.中药对肝脏损伤的原因及机制较为复杂,可能与药物对肝脏的直接毒性或特异性体质有关.此外,中药炮制方法、给药途径、剂型、剂量、疗程及配伍不当也会引起药物性肝损伤.中药所致的药物性肝损伤的诊断主要依据用药史、发病的时间、过程和临床表现并排除其他因素.治疗可选用还原型谷胱甘肽、S-腺苷蛋氨酸、多烯磷脂酰胆碱、熊去氧胆酸、水飞蓟素等药物,重症可选择人工肝脏支持治疗.     

急性泪囊炎不同排脓途径的疗效观察

由于急性泪囊炎的一期处理过程中经泪小管引流脓液与经皮肤切开排脓对治疗效果的影响不同,将我院2007年以来接诊的急性泪囊炎患者随机分为观察组与对照组:患者抗感染治疗后经泪小管引流脓液,成人行经鼻内镜泪囊开窗引流术,先天性泪囊炎患儿行泪道探通;对照组患者急性期经皮肤切开排脓、放置引流条和消炎处理后再行泪囊鼻腔吻合术.观察组患者治疗成功率98.46％,外观满意率98.25％;对照组患者治疗成功率89.11％,外观满意率54.35％.观察组患者较对照组患者疗程缩短约7天～10天.急性泪囊炎经全身抗感染治疗后行泪小管引流脓液能更有效地控制炎症,不给患者带来新的创伤,值得临床推广应用.     

Common future and personal responsibilities: A comparison between Italian and Burundian students

The political, social, and cultural history of a nation modulates the representations of rights and duties. The aim of this research is to compare students from two countries (Italy and Burundi) in terms of how they define their rights and duties. In the two countries, there are differences both in the legal protection of fundamental rights and in regard to material conditions, which in turn ensure the effectiveness of rights. Focus groups structured around nine questions were conducted in Burundi and in Italy. The discussions with Italian and Burundian students showed some clear differences. Although both groups speak of rights as something to be safeguarded and something that everyone is born with, Italian students do not recognize the complementarity of rights and duties and consider the latter simply as a limit and an obstacle to individual enhancement. On the contrary, Burundian adolescents seem more aware of their personal responsibilities and their role in protecting human rights.     

Communal Versus Individual Learning of a Math-Estimation Task: African American Children and the Culture of Learning Contexts

The authors compared the performance of 78 African American 5th-grade students who studied a math-estimation task in one of two learning contexts. Learning contexts differed in the degree to which they afforded the expression of communalism. ANCOVA confirmed that posttest performance was best for students who studied in the high communal-learning context. The findings support A. W. Boykin's (1994) contention that the cultural context of learning can be a critical mediator of children's performance.     

“Hidden Treasures” from Israeli Women's Writing Groups: Exploring an Integrative,Feminist Therapy

This article presents the work of mental health practitioners and laywomen who have developed and applied a new model of working with women in groups, a model founded on feminist principles that uses women's writing as the vehicle for group work. The model maintains a delicate balance between the needs of the group, the needs of the women in their social environment, and the women's ability to be the broker of their time, place, and mode of expression. This article presents the theoretical roots of the model and the qualitative evaluation of the writing groups that have been held over seven years, and it discusses how and why this model works for women of all ages and from all walks of life.

Abstract

Some salient features of a Sullivanian perspective are given as a background for a discussion of the case of Anna. Lived experience is mentioned as of equal importance as fantasy. Technically, analytic inquiry in the form of questions is stressed. In his review of the case presentation, the author observes that too much emphasis is given to content, compared to form. He notes that the therapist does not report key transference and non-transference interactions with the patient and further suggests that the patient still has significant narcissistic problems. He suggests that it is time to confront the patient with her characterological ways of relating to the analyst.

This paper addresses issues of infantile gender identity as they are demonstrated through group processes amongst the carers of disturbed adolescents. It uses this and other clinical material to explore gender narcissism – both male and female. It examines how such narcissism is linked to sado-masochism, and how it can impede a healthy development towards bisexual and triadic functioning.     

Disaster Impact Across Cultural Groups: Comparison of Whites, African Americans, and Latinos

The current study extends knowledge regarding the differential impact of natural disasters among White, African American, and Latino survivors of Hurricane Ike through its use of a large, regional sample recruited via representative sampling procedures to examine the associations between cultural identification and disaster impact, including loss, damage, and negative mental health outcomes. Consistent with previous research, results indicated disparities between cultural groups with regard to disaster exposure. Additionally, type of disaster impact was differentially associated with PTSD and depression status dependent on cultural group. Specifically, the extent of personal disaster exposure, property damage, and loss of services made significant contributions to PTSD status among White survivors. African-Americans were more likely than White and Latino Ike survivors to endorse post-disaster PTSD and depression and endorsement of depression was predicted by severity of property damage. With respect to Latino respondents, only the extent of personal disaster exposure significantly contributed to both PTSD and depression status. Implications of the current findings are discussed with regard to future disaster preparedness and response efforts and the implementation and evaluation of community-based disaster resources.
