This study tested the hypothesis that breathlessness in asthma relates linearly to airway obstruction when situational, attentional and emotional influences are held constant via random presentation of different intensities of externally applied airflow obstruction. Adolescents with stable asthma and normal controls (n=25+25) with lung functions of approximately 3.5 l forced expiratory volume in 1 s (FEV1) breathed through a device which obstructed airflow with five stimulus intensities, analogous to a mean reduction in FEV1 of 8–66%. A session consisted of 10 blocks, each with presentation of five stimulus intensities plus the baseline resistance of the apparatus. Breathlessness was continuously reported by moving a lever along a 10-point scale. The mean breathlessness was computed per stimulus intensity. Lung function and anxiety were measured before and after the test.
Participants with asthma, not controls, manifested a paradoxical response: they reported significantly more breathlessness, but undifferentially. One patient against 12 controls reported consistently more breathlessness from baseline to severe obstruction. The hypothesis was only supported for controls. Breathlessness did not correlate with severity of asthma, lung function, duration of asthma, number of exacerbations over the last six months, age, sex or anxiety.
It was concluded that the meaning of airflow obstruction in patients with asthma has changed and underlies their paradoxical responses, even when situational, attentional and emotional factors are controlled. 相似文献
One measure of the impact of genetic counseling and testing (GCT) is the extent to which it fosters behavioral change that is consistent with mutation status. We describe and illustrate how two different signal detection methods, receiver operating characteristic (ROC) analysis and recursive partitioning, can be used in this context to evaluate the impact of GCT. We analyzed real screening behavior data obtained in the 12 months following GCT for Hereditary Nonpolyposis Colon Cancer (HNPCC) using these two different signal detection approaches. Each approach demonstrated that GCT had an impact on behavioral outcomes, and was effective in fostering behavioral outcomes appropriate to mutation status. The ROC approach demonstrated that GCT was effective because mutation positive and mutation negative individuals could be distinguished on the basis of the number of recommended screening behaviors. The recursive partitioning approach demonstrated that GCT was effective because there were generally high rates of adherence to screening guidelines among subjects. The recursive partitioning technique also identified four subgroups of subjects, each with distinct characteristics, for which tailored interventions could be developed to increase rates of adherence to screening guidelines. Quantitative analyses of stimulus control and reinforcer control in conditional discriminations and delayed matching-to-sample procedures often encounter a problem; it is not clear how to analyze data when subjects have not made errors. The present article examines two common methods for overcoming this problem. Monte Carlo simulations of performance demonstrated that both methods introduced systematic deviations into the results, and that there were genuine risks of drawing misleading conclusions concerning behavioral models of signal detection and animal short-term memory. 相似文献
Lip reading is the ability to partially understand speech by looking at the speaker's lips. It improves the intelligibility of speech in noise when audio-visual perception is compared with audio-only perception. A recent set of experiments showed that seeing the speaker's lips also enhances sensitivity to acoustic information, decreasing the auditory detection threshold of speech embedded in noise [J. Acoust. Soc. Am. 109 (2001) 2272; J. Acoust. Soc. Am. 108 (2000) 1197]. However, detection is different from comprehension, and it remains to be seen whether improved sensitivity also results in an intelligibility gain in audio-visual speech perception. In this work, we use an original paradigm to show that seeing the speaker's lips enables the listener to hear better and hence to understand better. The audio-visual stimuli used here could not be differentiated by lip reading per se since they contained exactly the same lip gesture matched with different compatible speech sounds. Nevertheless, the noise-masked stimuli were more intelligible in the audio-visual condition than in the audio-only condition due to the contribution of visual information to the extraction of acoustic cues. Replacing the lip gesture by a non-speech visual input with exactly the same time course, providing the same temporal cues for extraction, removed the intelligibility benefit. This early contribution to audio-visual speech identification is discussed in relationships with recent neurophysiological data on audio-visual perception. 相似文献
Do nonhuman primates attribute goals to others? Traditional studies with chimpanzees provide equivocal evidence for “mind reading” in nonhuman primates. Here we adopt looking time, a methodology commonly used with human infants to test infant chimpanzees. In this experiment, four infant chimpanzees saw computer-generated stimuli that mimicked a goal-directed behavior. The baby chimps performed as well as human infants, namely, they were sensitive to the trajectories of the objects, thus suggesting that chimpanzees may be endowed with a disposition to understand goal-directed behaviors. In line with the ironic processing theory of Wegner (Psychol. Rev. 101 (1994) 34), it is often argued that the suppression of anxiety-related thoughts results in a paradoxical increase of anxiety and thought intrusions, both after and during the thought suppression. In a sample of undergraduate students (14 men, 18 women), we investigated the effects of suppressing anxious thoughts about an imminent painful electrocutaneous stimulus. During thought suppression, self-reported anxiety and frequency of anxious thoughts did not increase, and duration of anxious thoughts decreased. After thought suppression, participants experienced an increase in self-reported anxiety and the frequency of anxious thoughts. There was no effect upon thought duration. The results support the idea that suppression of anxiety-related thoughts may result in a paradoxical increase in anxiety, and may cause and/or maintain anxiety problems. 相似文献
Anger is a significant human emotion with far‐reaching implications for individuals and relationships. We propose a transactional model of anger that highlights its relational relevance and potentially positive function, in addition to problematic malformations. By evolutionary design, physical, self‐concept, or attachment threats all similarly trigger diffuse physiological arousal, psychologically experienced as anger‐emotion. Anger is first a signaling and motivational system. Anger is then formed to affirming, productive use or malformed to destructive ends. A functional, prosocial approach to anger organizes it for protective and corrective personal and relational adaptation. In our model, threat perception interacts with a person's view of self in relation to other to produce helpful or harmful anger. Inflated or collapsed views of self in relation to other produce distinct manifestations of destructive anger that are harmful to self, other, and relationship. Conversely, a balanced view of self in relation to other promotes constructive anger and catalyzes self, other, and relationship healing. Clinical use of the model to shape healing personal and relational contact with anger is explored. 相似文献
