This article argues that psychiatric diagnoses are not valid or useful. The use of psychiatric diagnosis increases stigma, does not aid treatment decisions, is associated with worsening long-term prognosis for mental health problems, and imposes Western beliefs about mental distress on other cultures. This article reviews the evidence base focusing in particular on empirical findings in relation to the topics of: aetiology, validity, reliability, treatment and outcome, prognosis, colonialism, and cultural and public policy impact. This evidence points toward diagnostic based frameworks for understanding and intervening in mental health difficulties being unable to either improve our scientific knowledge or improve outcomes in clinical practice and suggests that we need to move away from reliance on diagnostic based approaches for organising research and service delivery. Alternative evidence-based models for organising effective mental health care are available. Therefore formal psychiatric diagnostic systems such as the mental health section of the International Classification of Diseases Tenth Edition (ICD-10) and Diagnostic Statistical Manual Fifth Edition (DSM 5) should be abolished.  相似文献

The objective of this research study was to assess pharmacological, somatic and/or psychological treatments in adults with a diagnosis of major depressive disorder who have not responded to at least one course of antidepressant medication. We conducted a systematic review to identify systematic scientific reviews and meta-analyses on treatment-resistant depression (TRD) published until February 2012. Of the sixty studies selected, sixteen met the inclusion criteria and were therefore included in the review. We considered eight main themes, including the definition of TRD, long-term results, and different treatment strategies, including so-called somatic therapies. Based on the review, the definition of TRD should be standardized in order to achieve a shared conceptualization of this disorder. This would allow a better understanding among clinicians and researchers in the field, promoting a homogeneous research methodology and thus leading to more reliable and comparable results. This essential conceptual clarification would also have a positive impact on patients with TRD, their families, and social and health systems.  相似文献

The h-index originates from the assumption that the number of citations received by a scientist is a better indicator of the relevance of his or her work than the number of papers he or she publishes or the journals where they are published. It takes into account the number of papers published and the citations to those papers in a balanced way, and thus is useful to make comparisons between scientists. The present paper addresses the most frequent questions about the h-index. Specifically, it explains its origin, its advantages compared to other indices, the factors that can influence it (e.g. age, field of knowledge, topic of research and language of publication), its variants, and the injustices it may lead to. In short, this paper provides a clear exposition of the hoped-for role of the h-index in the evaluation of scientists: that it serves as a useful complement to other indicators that are more subjective, and that it contributes to the progress of science by aiding decision-making on allocation of research resources in a more effective way, and on rewarding researchers who contribute to scientific progress in a more fair way.  相似文献

Many commentators propose that Hume thinks that we are not or should not be motivated to perform naturally virtuous actions from moral sentiments. I take issue with this interpretation in this paper, arguing that Hume fully incorporates the moral sentiments into his understanding of how human beings act when it comes to the natural virtues and that he does not see the moral sentiments as a problematic kind of motivation that threatens or weakens the virtuous status of the action.  相似文献

This paper reports on a case study undertaken within a small, rural primary school in the UK to explore the extent to which it was possible to run a moral and spiritual programme as an intrinsic and inclusive part of the curriculum based on the children’s acquisition of virtues. The pupils were encouraged to build on their own experience and reflect on virtues such as ‘kindness’, ‘reliability’ or ‘responsibility’. Some positive associations were indicated between the moral and spiritual programme at the school and outcomes such as the pupil’s ability to resolve conflict and self-regulating behaviour as well as examples of empathy and altruism. The curriculum showed some effective ways of encouraging young people to be reflective and responsible for their own actions. This study was developed by observation of an assembly and targeted lessons together with semi-structured interviews. There was also a review of documentation related to the virtues programme to give a triangulated approach.  相似文献

创造性思维(CT)是人类高级认知的重要组成部分,而工作记忆容量(WMC)是个体认知功能的重要指标,对于WMC能否预测CT存在不同的观点。本文先从正向预测、负向预测和不能预测的角度分类总结了相关研究证据,然后用游乐园模型(APT)、双通道模型(DPCM)和注意控制理论(CAT)对不同结果进行解释,分析了以往研究可能存在的问题,包括测量WMC和测量CT的任务类型不匹配、测量CT的领域特殊性问题、忽略其他认知能力差异以及未考虑智力、人格等因素对二者关系的影响等。最后,结合已有问题与当前的研究重点对未来研究进行了相应的展望。  相似文献

The paper calls for a re-evaluation of physical education’s cognitive value claims, as this issue is fundamental to many of the conceptual difficulties the subject faces. Current epistemological challenges are reviewed before analysing the structural connections between intelligent practice and intelligent virtues, and the possibilities for physical education to better articulate its’ intrinsic and instrumental values claims. The paper evaluates arguments made on this basis and reviews revised curriculum planning and pedagogical practices, which could support an enhanced focus on learners’ wider aspirations and achievements; factors crucial for a virtuous life. While applauding enquiry into the possibilities for considering physical education as a moral endeavor, the paper raises concerns about conceptual clarity in intellectual and empirical accounts of virtue. The paper concludes by advocating further research on the interrelationship between intellectual and empirical accounts of skill, evaluation of learners’ decision-making and the aims of physical education and school sport programmes.  相似文献

ABSTRACT

Discussion of the Cambridge Platonists, by Constantinos Patrides and others, is often vitiated by the mistaken contrasts drawn between those philosophers and late antique Platonists such as Plotinus. I draw attention especially to Patrides’s errors, and argue in particular that Plotinus and his immediate followers were as concerned about this world and our immediate duties to our neighbours as the Cambridge Platonists. Even the doctrine of deification is one shared by all Platonists, though it is also here that genuine differences between pre-Christian and Christian exegesis can be found. All, it can be said, hope and expect to join ‘the dance of immortal love’, but Christian Platonists had a deeper sense of God’s ‘humility’ in His Word’s material and temporal manifestation. Not Olympian Zeus but the Crucified Christ was their preferred image of divine involvement, and their better guide to heaven.  相似文献

Abstract: Contextualism in epistemology has been proposed both as a way to avoid skepticism and as an explanation for the variability found in our use of "knows." When we turn to contextualism to perform these two functions, we should ensure that the version we endorse is well suited for these tasks. I compare two versions of epistemic contextualism: attributor contextualism (from Keith DeRose) and methodological contextualism (from Michael Williams). I argue that methodological contextualism is superior both in its response to skepticism and in its mechanism for changing contexts. However, methodological contextualism still faces two challenges: explaining why we are solidly committed to some contexts, and explaining why knowledge within a context is valuable. I propose virtue contextualism as a useful extension of methodological contextualism, focusing on the way that our virtues depend on our social roles. My proposed virtue contextualism retains the benefits of methodological contextualism while explaining both our commitment to particular contexts and the value of knowledge held within those contexts.  相似文献
