The objective of this study is to compare elderly individuals with late (60 years old) versus early (<60 years old) onset spinal cord injury (SCI) across quality of life (QOL) domains for which cross-sectional design was used. The outcome measures selected were secondary medical complications (e.g., pneumonia, autonomic dysreflexia, number of days hospitalized), Functional Independence Measure (FIM), Satisfaction With Life Scale (SWLS), and the Craig Handicap Assessment and Reporting Technique (CHART). Analyses between groups showed that individuals with SCI onset 60 years of age or older were significantly older, had a greater proportion of incomplete lesions, were more likely to have SCI resulting from medical complication, and were less likely to be working. After controlling for differences in demographic and lesion characteristics, the majority of QOL domains were similar between groups. However, overall self-reported handicap (CHART-total score) was significantly greater among elderly with late onset SCI, particularly in the areas of physical independence and social integration. Differences in QOL between elderly with late versus early onset SCI were most prominent in the area of physical independence and social integration. The importance of appropriate statistical control, theoretical implications, and future directions are discussed.  相似文献

This article presents and interprets the case study of an elderly woman's story of suffering. It results from interviews conducted with African-American and Caucasian men and women, aged 70 and above, concerning their personal definitions, attributions, and theories about suffering. Elders' experiences of suffering were examined under the rubrics of gerontology, narrative, and religious studies. Key themes offered in this case study are: (a) How the life course unfolds because of or despite suffering, (b) what moral status an elder confers on past or present suffering, (c) how a personal sense of the sacred accommodates suffering, (d) how this particular respondent alters her identity based on assimilating the new information of suffering, and (e) how the story of suffering stands out against or blends in with the backdrop of the life story. The uniqueness of this case study also mirrors commonalities of elders' narratives of suffering, particularly those of women. This elder's story demonstrates that smaller individual suffering is rooted in the frame of the larger public world. It also illustrates that the language women use to attribute, define, and theorize suffering is embedded in relationships.  相似文献

Recent brain imaging studies in Huntington's disease (HD) and normal aging suggest a relationship between central dopaminergic neurotransmission and cognitive performance. Results demonstrate substantial losses in dopamine (DA) function in both HD and aging. Moreover, HD patients and older adults show deficits across multiple cognitive domains, including episodic memory, speed of processing, and executive functioning. Although few studies are available at present, there is converging evidence that multiple measures of pre- and postsynaptic DA biochemistry are (a) highly interrelated, and (b) strongly associated with the cognitive deficits that accompany HD and aging. There is also emerging evidence that DA neurotransmission influences cognitive performance independent of HD or age. In general, the research reviewed in this article indicates that the nigrostriatal DA system is an important component of a frontostriatal circuitry that is critically involved in cognitive functioning.  相似文献

This article reviews the significance of postmodernism as applied to understanding social gerontology. We begin by looking at the disciplinary dominance of biomedical models to understanding bodily change through later life. We then move our analysis to the lack of theoretical models within social gerontology which have not attempted to analyze or develop social explanations of the body. The article introduces the concept of postmodernism and reviews the work of major theorists who have impacted postmodern gerontology. We conclude by arguing for an interface between social theory and social gerontology by drawing on recent examples of the mask of aging thesis, biotechnologies, and new technologies that impact aging identity and understanding the self.  相似文献

Medical and health languages have emerged as master narratives used to police the identities that older offenders adopt in contemporary special hospitals such as Broadmoor, Rampton, and Ashworth, in the United Kingdom. Both contain continually changing technologies that function to mediate relations between older offenders and special hospitals. Medical and institutional discourses have been presented as reducing limitations associated with psychiatric disorders. This represents an increase in professional control that can be exerted on inmates' lifestyles in special hospitals which extends to the surveillance and governance of older persons in such secure settings. It is evident that the use of the indeterminate sentence, compulsory treatment prevents older offenders from rehabilitation. The process of treatment is to transform, discipline and normalize their behavior. In order to achieve normalization, coercive forms of treatment are invoked from informal social rules to the use and overuse of psychothropic drugs. Mentally disordered older offenders are placed under the gaze of perpetual surveillance but find ways of negotiating, resisting, and subverting that gaze. Furthermore, this article illuminates how the aging body and the use of time become other sources of punishment, which are pivotal to the structural organization of secure settings in the United Kingdom.  相似文献

The objective of this study was to use structural equation modeling (SEM) to examine the relationships between factors of cognitive complaints in 427 adults with HIV receiving care from an AIDS service organization (ASO) in Alabama. A face-to-face interview assessed cognitive complaints, mood, self-perceived health status, and substance use. Direct and indirect paths were specified between years with HIV (chronicity) age, education, stress, self-perceived health status, substance use, and cognitive complaints. The final model fit the data well (GFI = .95; AGFI = .92), revealing that age, education, substance use, self-perceived health status, and stress contributed directly or indirectly to cognitive complaints. Chronicity was only related to age, suggesting that duration of HIV may be of limited value in predicting cognitive complaints. These results demonstrate the complexity between the factors that influence cognitive complaints and offer possible solutions that health professionals can use to mitigate cognitive complaints in this population.  相似文献

ABSTRACT

We present here the first direct measurement of the radiation-damage-induced energy stored in δ-phase plutonium. The primary mode of radioactive decay of 239Pu occurs with a time constant of τ=1.1?×?1012?s. Each decay imparts about 85?keV of recoil energy to the uranium byproduct, 5.2?Mev to the alpha particle, and a spectrum of mostly low energy gamma rays with the most probable at 51?keV [1]. Most of the decay energy is converted immediately to heat, releasing about 1.9?mW/g. However, some thermally-recoverable energy remains trapped. Reported here are measurements of that stored energy using differential scanning calorimetry (DSC) applied to 239Pu-2.0 at.%Ga δ-phase alloy. Retained energy of ~2 J/g saturates at about 5 months and is unchanged after 30 years. The magnitude of the stored energy agrees with a short-bond defect model that that we present here. This model treats radiation damage as a Pu impurity with shortened bond lengths. It explains the change in known properties with age and predicts that density increases with age, contrary to current thinking. The short-bond impurities proposed are expected to act like other impurities, affecting strength, phase transitions, grain boundaries and other metallurgical properties.  相似文献

Experimental measurements of stress drops and band strains in type-B Portevin–Le Chatelier (PLC) bands were carried out for 5xxx series Al–Mg sheets with Mg content between 1.8 and 4.5?wt%. While the stress drops increase with global strain, the band strain values in all the samples follow a common linear relationship with global strain. The results indicate that the type-B PLC band strain is independent of solute content at given strain rate.  相似文献

Background

To assess the hypothesis that in a sample of very healthy elderly men selected to minimize risk for Alzheimer’s disease (AD) and cerebrovascular disease, myelin breakdown in late-myelinating regions mediates age-related slowing in cognitive processing speed (CPS).

Materials and methods

The prefrontal lobe white matter and the genu of the corpus callosum myelinate later in brain development (late-myelinating white matter; LMWM) and are more vulnerable to breakdown due to the effects of normal aging. An in vivo MRI biomarker of myelin integrity (transverse relaxation rates; R2) of LMWM was obtained for 38 very healthy elderly adult men (mean age = 66.3 years; SD = 6.0; range = 55–76). To evaluate regional specificity, we also assessed a contrasting early-myelinating region (splenium of the corpus callosum; SWM), which primarily contains axons involved in visual processing. CPS was assessed using the Trail Making Test.

Results

LMWM R2 and CPS measures were significantly correlated (r = .515, p = .0009), but no significant association between R2 and CPS was detected in the splenium (p = .409). LMWM R2, but not SWM R2, was a significant mediator of the relationship between age and CPS (p = .037).

Conclusions

In this very healthy elderly sample, age-related slowing in CPS is mediated by myelin breakdown in highly vulnerable late-myelinating regions but not in the splenium.  相似文献
