Premature termination of therapy by patients is a common phenomenon that can be deleterious to treatment outcome for patients and also negatively affect therapists, treatment centers, and research programs. Therefore, a method of identifying patients at risk for premature termination could have widespread benefits. This study investigated whether patients’ MMPI-2 profiles, including clinical scale elevations and Negative Treatment Indicator (TRT) scores, could predict premature termination in an outpatient sample, controlling for personality disorder diagnosis and symptom severity at intake. Results indicated that while TRT scores were not incrementally predictive of premature termination, the total number of clinical scale elevations was significantly incrementally predictive of dropout. Clinical implications are discussed.  相似文献

After reviewing the target articles of this issue, Hayes and Ellis respectively see less and more possibilities for integration. This concluding article attempts to better elucidate possibilities for integration between second and third wave CBT’s, as exemplified by Rational-Emotive & Cognitive-Behavior Therapy (REBT) and Acceptance and Commitment Therapy (ACT). We suggest that if ACT is going to err, it will be in the direction of being too afraid of talk. If REBT is going to err, it will be in the direction of not being afraid enough. In numerous languages determiner forms depend not only on semantic information but also on several other kinds of information, such as the grammatical gender of the controlling noun or the phonological properties of the context. In the present research we contrasted two possible accounts of determiner retrieval: one in which every type of required information is bundled into a unitized representation for determiner retrieval and one in which each type of information can individually activate determiner forms. These alternative hypotheses were investigated in three experiments in which native speakers of French named pictures with simple [determiner + noun] or complex [determiner + adjective + noun] noun phrases. In the experiments, the properties of the contextual cues that drive the retrieval of the determiner were manipulated - for example, we manipulated the number of determiner forms that are compatible with a given grammatical gender and the number of grammatical genders that a given determiner form can be used with. Neither hypothesis can fully account for the results of the three experiments. However, a hybrid hypothesis that combines the principal features of the two hypotheses provides a good account of the data.  相似文献

Vulvar vestibulitis syndrome (VVS), a vulvar pain disorder, continues to puzzle medical and mental health professionals due to its unknown etiology and lack of effective treatment. This study used transcendental phenomenology methodology to explore the experiences of couples in which the woman has a diagnosis of VVS. Sixteen in-depth semi-structured interviews were conducted with 13 heterosexual couples and 3 women. Four essences emerged: (1) In search of ... the medical journey required extensive searching for knowledgeable and respectful practitioners to provide treatment. (2) The process of developing a personal understanding of this disorder led many couples to question their role in causing and maintaining VVS. (3) Developing strategies for coping with painful intercourse led to three strategies: becoming non-sexual, using alternatives to vaginal sex, and altering or enduring painful intercourse. (4) Feelings of isolation were experienced as adapting to this chronic pain syndrome was often a lonely process. Clinical suggestions included: treating the couple, not just the woman with VVS; encouraging couples to broaden definitions about the importance and primacy of vaginal intercourse and suggest alternative sexual activities less likely to cause vulvar pain; developing shared meaning as a couple, and assisting couples in locating physicians and resources. Suggestions are relevant for couples with VVS and those with chronic health problems affecting sexual relationships.  相似文献

After a brief exposition of Elementary Pragmatic Model about changes in dyadic interactions, the validity and clinical usefulness of the model was evaluated with a newly created test, called SISCI-Sentences. This test is composed of 90 Sentences with strong psychological impact. Administration of this test to non-clinical and clinical participants produced statistically significant differences between the choices of participants in the two groups. Implications of these results for the use of these Sentences in the course of psychotherapy are discussed.

This investigation examined the visuomotor tracking abilities of persons with apraxia of speech (AOS) or conduction aphasia (CA). In addition, tracking performance was correlated with perceptual judgments of speech accuracy. Five individuals with AOS and four with CA served as participants, as well as an equal number of healthy controls matched by age and gender. Participants tracked predictable (sinusoidal) and unpredictable signals using jaw and lip movements transduced with strain gauges. Tracking performance in participants with AOS was poorest for predictable signals, with decreased kinematic measures of cross-correlation and gain ratio and increased target-tracker difference. In contrast, tracking of the unpredictable signal by participants with AOS was performed as well as for other groups (e.g. participants with CA, healthy controls). Performance of the subjects with AOS on the predictable tracking task was found to strongly correlate with perceptual judgments of speech. These findings suggest that motor control capabilities are impaired in AOS, but not in CA. Results suggest that AOS has its basis in motor programming deficits, not impaired motor execution.  相似文献

Vocal babbling involves production of rhythmic sequences of a mouth close–open alternation giving the perceptual impression of a sequence of consonant–vowel syllables. Petitto and co-workers have argued vocal babbling rhythm is the same as manual syllabic babbling rhythm, in that it has a frequency of 1 cycle per second. They also assert that adult speech and sign language display the same frequency. However, available evidence suggests that the vocal babbling frequency approximates 3 cycles per second. Both adult spoken language and sign language show higher frequencies than babbling in their respective modalities. No information is currently available on the basic rhythmic parameter of intercyclical variability in either modality. A study of reduplicative babbling by 4 infants and 4 adults producing reduplicated syllables confirms the 3 per second vocal babbling rate, as well as a faster rate in adults, and provides new information on intercyclical variability.  相似文献

Adult humans were studied for improvements in their ability to segregate natural whole speech in background noise, in 6 test sessions spaced with a very wide range of inter-session interval (ISI) ranging from minutes to weeks apart so as to examine the effect of this parameter on initial (early) and late components of perceptual learning. Improvements were found even with spacings of 3 weeks between the punctate task sessions. All subjects showed similar total learning amounts but there were sex- and ISI-dependent differences in learning patterns, which we indexed by dividing the overall exponentially-decreasing learning pattern into an early phase between the first two sessions and a later phase between the second and sixth sessions. Males tested at all ISIs and females tested at short (2, 5 and 15 min) and long (1–21 days) ISIs showed small amounts of early-phase learning and large amounts of late-phase learning. However, females tested at intermediate (30 min and 1 h) ISIs showed only early learning, i.e., faster learning given that the total learning was the same. This sex- and ISI-specific deviant pattern could be changed to the standard pattern by interposing an overnight interruption that included sleep amongst test sessions. Thus, improvement in this complex auditory streaming and identification task can occur even with very brief and widely-spaced exposure, generally through a standard pattern of slower overall learning, but also through a sex- and ISI-specific deviant pattern of very rapid early learning which can be modulated by interposed delay unlike the standard pattern.  相似文献

Toddlers’ and preschoolers’ knowledge of the phonological forms of words was tested in Spanish-learning, Catalan-learning, and bilingual children. These populations are of particular interest because of differences in the Spanish and Catalan vowel systems: Catalan has two vowels in a phonetic region where Spanish has only one. The proximity of the Spanish vowel to the Catalan ones might pose special learning problems. Children were shown picture pairs; the target picture’s name was spoken correctly, or a vowel in the target word was altered. Altered vowels either contrasted with the usual vowel in Spanish and Catalan, or only in Catalan. Children’s looking to the target picture was used as a measure of word recognition. Monolinguals’ word recognition was hindered by within-language, but not non-native, vowel changes. Surprisingly, bilingual toddlers did not show sensitivity to changes in vowels contrastive only in Catalan. Among preschoolers, Catalan-dominant bilinguals but not Spanish-dominant bilinguals revealed mispronunciation sensitivity for the Catalan-only contrast. These studies reveal monolingual children’s robust knowledge of native-language vowel categories in words, and show that bilingual children whose two languages contain phonetically overlapping vowel categories may not treat those categories as separate in language comprehension.  相似文献
