Two randomly assigned groups, N = 7 each, of neurologically normal patients were composed. One group, RM, had regional cerebral blood flow measurements during a recognition memory probe test; the other group, SC, had rCBF measurements during a semantic classification task, using the same stimulus and response features as the RM task. Thus, the groups differed only in the cognitive instruction they were executing. The RM group showed a significantly lower change from resting baseline to activation, in mean left hemisphere initial slope index, than did the SC group. The RM group, but not the SC group, showed a significant inverse correlation between occipital flow and accuracy of memory performance as indicated by d′. Correlations between age and hemispheric initial slope index, and between homologous left and right hemisphere sites, were also described. The results are considered to support an anatomical basis for the distinction between episodic and semantic memory and to suggest that occipital flow may diminish with accurate memory performance because of an upstream demand of blood in the medial temporal lobes. 相似文献

Single letters were presented tachistoscopically in the left or right hemifield to right-handed observers. Superior recognition of letters on the right was found for letters chosen from subsets containing four items, but no differences were found when any letter, or any symmetrical letter, could appear. In a second experiment, the right hemifield advantage was found under conditions requiring the subject to determine the subset only when the subset chosen was appropriate. Right CVAs, mild left CVAs, and non-brain-damaged adults received the Token Test under three conditions: quiet, white noise, and speech babble. Left CVAs performed most poorly of all groups in all conditions experiencing difficulty with subtests III, IV, and V. Right CVAs were inferior to controls on each of these three subtests in the speech babble condition only. Thus, right CVA difficulty with both memory and syntactic tasks was demonstrated in speech babble. These results are explained by the hypothesis that both hemispheres are necessary to process auditory information under complex listening conditions. Total brain capacity of right CVA patients may be overloaded if either memory load or linguistic complexity is increased in the presence of certain types of listening competition. 相似文献

Children ranging in age from 3 to 12 tapped on a morse key with their index finger as fast as possible. After a control test, they concurrently tapped and recited a nursery rhyme, recited animal names, and memorized shapes. Previous results suggested that more interference in a motor task results from attempting at the same time to perform another task controlled by the same, than by a different, hemisphere. Right-handed tapping and talking both used left hemisphere space; left-handed tapping and talking were controlled by different hemispheres. Relative to silent tapping concurrent talking caused a greater drop in right-hand tapping rate than in left-hand tapping rate in both the rhyme and animal conditions. Interference by shape memorizing was equal across hands. The differential effect of speaking on right-hand preference indicates the left lateralization of speech output control. The size of this effect did not vary with increasing grade level. The findings support the view that speech output control is fully lateralized at least by age 3 years. 相似文献

Clinical and autopsy studies were made on a right-handed man who had central deafness and subcortical motor aphasia, and the literature on central deafness and on subcortical motor aphasia was analyzed. Central deafness is due to bilateral destruction of the primary auditory cortex. It is sometimes difficult to distinguish from word deafness and from auditory agnosia, which are due to pathology in other parts of the temporal lobes. There is almost always some preserved hearing in central deafness, possibly from some auditory pathway other than the classical pathway. In this patient the subcortical motor aphasia was due to bilateral destruction of the motor cortex for the mouth and throat. In some other cases subcortical motor aphasia was due to the same pathology that usually causes Broca's aphasia; in these cases the unexpected preservation of writing was perhaps related to some difference in how language functions were organized in the brain. 相似文献

The communication disorder in dialysis dementia is an integral feature of the syndrome. This report describes the communication disorder in a 67-year-old man over a period of 4 months from his admission to the hospital to his death. In the early stage of the syndrome, the most prominent feature was a speech problem, the characteristics of which may vary among cases. In the middle stage, it was difficult to distinguish among aphemia, aphasia, or a combination of dysarthria and confusional state. The severity of the problems in these stages was temporally related to the dialysis treatments. The final stage was characterized by constant mutism and was unrelated to the dialysis treatments. Death occurred 10 months after the communication problems emerged. Communication studies of dialysis dementia patients may offer a systematic method to monitor the course of the syndrome, its response to therapeutic trials, and a clinical model for the study of communication disorders in general. 相似文献

The ability to retain lists of verbal and nonverbalizable items across recurrent recognition tasks was tested in three groups: (1) stroke patients with a left-brain lesion and aphasia, (2) stroke patients with a right-brain lesion and left hemiplegia, and (3) nonneurologically impaired outpatients. As determined with signal detection measures, aphasics were deficient in discriminating words that were to be remembered from those that were not; their recognition of nonverbal visual (geometric art) or auditory (bird calls) patterns, however, was unimpaired. Left hemiplegics showed the opposite pattern. After a long-term interval (<10 min), correct recognition of words was diminished in all groups whereas recognition of visual patterns increased. Both groups of stroke patients adopted material-specific decision criteria which in part accounted for the dissociation of verbal and nonverbal recognition memory by laterality of lesion. Item analysis indicated that aphasics' verbal memory difficulties were affected by acoustic-semantic confusion of list words. 相似文献

Comprehension of language and visuo-spatial abilities were studied in 12 surgical patients with left cerebral lesions and right-hemisphere speech representation verified by sodium amobarbital tests. When left-hemisphere symptoms preceded the acquisition of speech, impaired understanding of syntactically complex sentences was associated with an early, but not late, onset of convulsive seizures. Other behavioral measures did not show a similar effect of age at seizure onset. Sparing or loss of comprehension could not be related to any aspects of the neurological histories in cases with right-hemisphere speech acquired before the onset of left-hemisphere symptoms. Factors which may have influenced the development of right-hemisphere language comprehension are discussed. 相似文献
