This paper describes a unique treatment program for complex pediatric illness. The Hasbro Children's Partial Hospital Program uses a family systems orientation, integrated care, and a partial hospital setting to treat children with a wide range of pediatric illnesses that have failed outpatient and inpatient treatments. We have treated more than 2000 children with at least 80 different ICD‐9 diagnoses. The multidisciplinary treatment team functions as a meta‐family for children and their families who present with illness and family beliefs that impede successful outcomes with standard care. The three features: family systems orientation, integrated care, and partial hospital setting, hopefully interact to create an environment that helps families expand and modify their explanatory models regarding participating in effective medical care. The goal of treatment is for both children and their parents to feel empowered to take control of the illness. Parents completing standardized measures at intake describe their children and families as experiencing significant emotional distress, low levels of general family functioning, and poor quality of life. Although the children are described as having distinct behavioral differences, the families are described as responding to the experience of a seriously ill child in similar ways. A treatment program that addresses the noncategorical aspects of how families respond to illness while addressing the specific diseases of the children can allow children and their families to respond favorably to treatment.  相似文献

Human connectome studies suggest that the brain has a modular small world network structure with rich-club effect. Such structure emerges spontaneously in simple model neural networks, (e.g. coupled maps), through adaptive rewiring according to the dynamic functional connectivity. The utility of adaptive rewiring has so far exclusively been demonstrated for unweighted networks; it is anything but guaranteed to work as well for weighted networks. We investigate adaptive rewiring in weighted networks, comparing various right-skewed, symmetrical, and left-skewed fixed weight distributions. We examine how network clustering, path length, modularity, and rich club coefficients develop for weakly, intermediate and strongly coupled networks. At low coupling strength, the weight distribution, as well as episodes of functional synchrony, have a significant effect on network evolution. With increased coupling strengths, all weighted networks robustly develop architectures similar to the unweighted ones. Adaptive rewiring appears relatively ineffective in networks with (biologically implausibly) extreme right-skewed weight distributions but performed most economically in biologically plausible log-normal distributions.  相似文献

ABSTRACT

The play of the ancient Greek tragedian Sophocles is the most famous story of Oedipus. However, similar stories have been told before and thereafter throughout history as legends and folk tales in different parts of the world. There is a specific folk tale type classified according to its narrative components and named the Oedipus tale. There are somewhat conflicting opinions about the universality of the Oedipus tales and Oedipus complex. This review deals with the nature of the Oedipus legend and its folk tale analogs. There are 25 examples of Oedipus folk tales that are known to be from Finland – a high prevalence compared to other regions of the world. From Scandinavia, for instance, they are virtually absent. The most of Finnish Oedipus stories have a special feature that is unique in the world.: a peculiar prophecy of a sheep that will be born at the same time and eaten by a wolf. An example of these Finnish folk tales is told. Possible interpretations of this tale are offered and discussed. The specific feature of the Finnish Oedipus tales stimulates a discussion on the different opinions regarding the timing of the Oedipus complex, especially considering its oral precursors.  相似文献

ABSTRACT

The current study builds on the non-linear Dynamic Systems (NDS) perspective to test the assumption that change in sickness absenteeism is non-linear, and that such change is due to workload, team adaptability and task cohesion. Participants were 37 firefighter teams (n = 250 individuals) from a main European capital city. The research hypotheses were tested using SPSS and the “cusp” package, in the statistical software R. The results suggest that change in sickness absenteeism behaviours over time is non-linear, with the cusp catastrophe model predicting such behaviours better than the linear and logistic models. In our model, task cohesion functions as an asymmetry factor (i.e., the independent variable that determines the strength and discrepancy between the two stable states of the dependent variable) leading to a linear change in sickness absenteeism. Interestingly, both workload and team adaptability function as bifurcation (i.e., the independent variable that determines the change between the two stable states of the order parameter) and asymmetry factors leading to non-linear and linear change in sickness absenteeism over time. This study contributes to the growing evidence that incorporating the NDS perspective enables a better understanding of action teams, namely those working in extreme environments.  相似文献

Abstract

Psychoanalytic inquiry into operas based on the life of Oedipus may provide further knowledge on the Oedipus complex. Therefore, we chose to analyze Enescu’s ?dipe and Stravinsky’s Oedipus Rex. Two distinct methodologies were used in our study. The first explored the concept of free association through musical themes in the operas. The second involved the comparative study of the Oedipus myth in order to provide a deeper understanding of Oedipus’s character. We observed that Oedipus displayed symptoms of his complex through the traits of aggressiveness and arrogance. Moreover, we noticed that Oedipus was compelled by the necessity of finding out who his real parents were and by unconsciously accomplishing the prophecy. Oedipus assumed the responsibility to free the Thebans from plague. Yet, it was too late, for the feared part of the prophecy was already accomplished. He provided a wrong answer to the Sphinx and then received the most severe punishment, one that would have ostracism as its outcome. It was, however, not too late for Oedipus to finally discover who his real parents were. Nevertheless, afraid of losing his place as King of Thebes, he investigated the plague’s causes. This resulted in his aggression as he resisted discovering “where babies come from.”  相似文献

In the experimental neural pacemaker of a rat, a novel firing pattern has been discovered. This pattern was generated between the period 2 firing pattern and the period 3 firing pattern during the periodic adding bifurcation and inverse periodic adding bifurcation. The pattern was observed and analyzed in the present investigation. The composition of this novel firing pattern could be regarded as a transition between a string of period 2 burst and a string of period 3 burst without single period 2 or period 3 burst, which was different from those chaotic and stochastic neural firing patterns in previous reports. It was identified to be stochastic by the inter-event intervals (IEIs) analysis, although it exhibited chaos-like characteristics with the results of the inter-spike intervals (ISIs) analysis. The numerical simulation suggested that the new pattern observed in the real biological system could be simulated in the stochastic Chay model but not in the deterministic model. With the signal to noise ratio (SNR) analysis and bifurcation analysis, this novel firing pattern was considered to be generated by stochastic resonance under the influence of noise near the periodic adding (inverse) bifurcation point. The probability analysis of transformed binary chain further confirmed that the origin of stochastic and chaos (deterministic)-like characteristics of this novel firing pattern.  相似文献
