This review describes methodological improvements of the 133Xe-inhalation technique for measurement of regional cerebral blood flow (rCBF) comprising correction routines both for scattered radiation from air passages and for remaining activity from previous measurements, as well as a new index of curve fit exactness. Measurements in normals are described showing new characteristics of the resting flow pattern and localized flow changes during activation with a variety of mental tasks. Our clinical applications in psychiatry are reviewed demonstrating the ability of the rCBF technique for differential diagnosis of dementia, for the evaluation of treatment by drugs, ECT, or psychosurgery, as well as for following, cases of toxic influence. The paper finally discusses the potentials as well as the limitations and artifacts of the technique. 相似文献

A robust literature has developed documenting neurocognitive late effects in survivors of leukemia and central nervous system (CNS) tumors, the most frequent cancer diagnoses of childhood. Patterns of late effects include deficits in attention and concentration, working memory, processing speed, and executive function, as well as other domains. As childhood cancer survivors are living longer, ameliorating deficits both in broad and specific neurocognitive domains has been increasingly recognized as an endeavor of paramount importance. Interventions to improve cognitive functioning were first applied to the field of pediatric oncology in the 1990s, based on strategies used effectively with adults who had sustained a traumatic brain injury (TBI). Compilation and modification of these techniques has led to the development of structured cognitive training programs, with the effectiveness and feasibility of such interventions currently an active area of research. Consequently, the purpose of this critical review is to: (1) review cognitive training programs intended to remediate or prevent neurocognitive deficits in pediatric cancer patients and survivors, (2) critically analyze training program strengths and weaknesses to inform practice, and (3) provide recommendations for future directions of clinical care and research. 相似文献

Aboriginal children in Canada are at high risk of fetal alcohol spectrum disorder (FASD) but there is little research on the cognitive impact of prenatal alcohol exposure (PAE) in this population. This paper reviews the literature on parent report of executive functioning in children with FASD that used the Behavior Rating Inventory of Executive Function (BRIEF). New data on the BRIEF is then reported in a sample of 52 Aboriginal Canadian children with FASD for whom a primary caregiver completed the BRIEF. The children also completed a battery of neuropsychological tests. The results reveal mean scores in the impaired range for all three BRIEF index scores and seven of the eight scales, with the greatest difficulties found on the Working Memory, Inhibit and Shift scales. The majority of the children were reported as impaired on the index scores and scales, with Working Memory being most commonly impaired scale. On the performance-based tests, Trails B and Letter Fluency are most often reported as impaired, though the prevalence of impairment is greater for parent ratings than test performance. No gender difference is noted for the parent report, but the boys had slightly slower intellectual functioning and were more perseverative than the girls on testing. The presence of psychiatric comorbidity is unrelated to either BRIEF or test scores. These findings are generally consistent with prior studies indicating that parents observe considerable executive dysfunction in children with FASD, and that children with FASD may have more difficulty with executive functions in everyday life than is detected by laboratory-based tests alone. 相似文献

This study examined the potential of using the regular administration of a common neuropsychological test, the CVLT-II, to assess learning potential in schizophrenia. Based on List A trial 1 performance and the learning slope, a schizophrenia sample was divided into three learning potential groups (non-learners, learners and high-achievers) that differed in the use of learning strategies. High-achievers utilized more semantic clustering than learners and non-learners, and non-learners were less consistent in words recalled than the other two groups. This standard administration approach is a promising, time-saving alternative to the modified tests of learning potential used so far. 相似文献

Parkinson’s disease with dementia (PDD) and dementia with Lewy bodies (DLB) are neurodegenerative conditions sharing a disorder
of α-synuclein metabolism. Temporal differences in the emergence of symptoms and clinical features warrant the continued clinical
distinction between DLB and PDD. While DLB and PDD groups’ neuropsychological profiles often differ from those in Alzheimer’s
disease (AD), the diagnostic sensitivity, specificity, and predictive values of these profiles remain largely unknown. PDD
and DLB neuropsychological profiles share sufficient similarity to resist accurate and reliable differentiation. Although
heterogeneous cognitive changes (predominantly in memory and executive function) may manifest earlier and more frequently
than previously appreciated in Parkinson’s disease (PD), and executive deficits may be harbingers of dementia, the enthusiasm
to uncritically extend the concept of mild cognitive impairment (MCI) to PD should be tempered. Instead, future research might
strive to identify the precise neuropsychological characteristics of the prodromal stages of PD, PDD, and DLB which, in conjunction
with other potential biomarkers, facilitate early and accurate diagnosis, and the definition of neuroprotective, neurorestorative,

In this paper, we review the most recent and often conflicting findings on conventional measures of executive control in autism
spectrum disorders. We discuss the obstacles to accurate measurement of executive control, such as: its prolonged developmental
trajectory; lack of consensus on its definition and whether it is a unitary construct; the inherent complexity of executive
control; and the difficulty measuring executive-control functions in laboratory or clinical settings. We review the potential
of an ecological-validity framework to address some of these problems, and describe new tests claiming verisimilitude, or
close resemblance to “real life” demands. We also review the concept of veridicality, which allows for the measurement of
the ecological validity of any task, and discuss the few studies addressing ecological validity in individuals with autism.
Our review suggests that a multi-source approach emphasizing veridicality may provide the most comprehensive assessment of

The current study examined the contribution of brain areas affected by Parkinson's disease (PD) to sequence learning, with a specific focus on response-related processes, spatial attentional control, and executive functioning. Patients with mild PD, patients with moderate PD, and healthy age-matched participants performed three tasks-a sequence learning task with a spatial pattern that was incidental to response selection, a spatial cuing task, and neuropsychological tests of executive function. Whereas moderate PD patients failed to show significant sequence learning, mild PD patients performed comparably with controls. Neither group of PD patients was impaired in the control of spatial attention. Sequence learning was correlated with neuropsychological measures of executive function but not with the ability to control spatial attention. These results suggest that the contribution of the brain areas affected by PD to sequence learning extends beyond motor learning to include the learning of perceptual patterns and involves executive function, including cognitive flexibility and set shifting. 相似文献

Lesional epilepsy is thought to be a direct consequence of focal brain lesions of dysgenetic, neoplastic, vascular, or traumatic
origin. It has been estimated that at least half of all epilepsies are the result of such lesions. The current discussion
includes an overview of the cognitive and behavioral presentations in adults with epilepsy secondary to focal pathology. The
neuropsychological presentation in this population is influenced by many factors, including the location and nature of the
underlying lesion, seizure characteristics, the effects of treatment, and patient variables. Few studies attempt to disentangle
the specific contributions of these variables to cognitive performance. However, where available studies examining the separable
effects of seizure-related variables on cognitive functioning in individuals with lesional epilepsy are also reviewed. This
overview includes a discussion of focal malformations of cortical and vascular development and select foreign tissue and acquired
