Depression is the most common mental disorder for young people, and it is associated with educational underachievement, self‐harm, and suicidality. Current psychological therapies for adolescent depression are usually focused only on individual‐level change and often neglect family or contextual influences. The efficacy of interventions may be enhanced with a broader therapeutic focus on family factors such as communication, conflict, support, and cohesion. This article describes a structured multi‐family group approach to the treatment of adolescent depression: Behaviour Exchange Systems Therapy for adolescent depression (BEST MOOD). BEST MOOD is a manualized intervention that is designed to address both individual and family factors in the treatment of adolescent depression. BEST MOOD adopts a family systems approach that also incorporates psychoeducation and elements of attachment theories. The program consists of eight multifamily group therapy sessions delivered over 2 hours per week, where parents attend the first four sessions and young people and siblings join from week 5. The program design is specifically aimed to engage youth who are initially resistant to treatment and to optimize youth and family mental health outcomes. This article presents an overview of the theoretical model, session content, and evaluations to date, and provides a case study to illustrate the approach.  相似文献

A systematic review of published and unpublished English language articles identified 14 studies containing 18 comparisons between functional family therapy (FFT) and another condition in the treatment of adolescent disruptive behavior and substance use disorders. In 11 of these comparisons, assignment to conditions was random, while nonrandom assignment occurred in seven studies. For both random and nonrandom comparisons, separate meta‐analyses were conducted for subgroups of studies depending on the type of comparison group used. Data from studies of untreated control groups (CTL), treatment as usual (TAU), and well‐defined alternative treatments (ALTs) were analyzed separately. Effect sizes from these six meta‐analyses were as follows: random assignment FFT versus CTL (k = 3, = 0.48, < .01); random assignment FFT versus TAU (k = 3, = .20, ns); random assignment FFT versus ALT (k = 5, = .35, < .05); nonrandom assignment FFT versus CTL (k = 2, = .90, ns); nonrandom assignment FFT versus TAU (k = 2, = .08, ns); and nonrandom assignment FFT versus ALT (k = 3, = .75, < .001). These results provide support for the effectiveness of FFT compared with untreated controls and well‐defined ALTs, such as cognitive behavior therapy, other models of family therapy, and individual and group therapy for adolescents.  相似文献

This is the second of two companion papers that provide an overview of mentalization‐based concepts and techniques when working with the seeming “mindlessness” of intra‐family violence. The focus of this paper is on general mentalization‐oriented approaches and specific interventions that aim to (1) disrupt the non‐mentalizing cycles that can generate intra‐family violence and (2) encourage the emergence of patterns of family interactions that provide the foundation for non‐violent alternatives. Various playful exercises and activities are described, including the taking of “mental state snapshots” and “selfies” in sessions and staging inverted role‐plays, as well as using theatrical masks and creating body–mind maps and scans. These can make “chronic” relationship issues come alive in session and permit “here and now” experiences that generate a safe context for mentalizing to take place. At the core of the work is the continuous focus on integrating experience and reflection. Without acute awareness of the thoughts and feelings occurring in the sessions, mere reflection is not likely to enable change. By increasing mentalizing in the family system, family members’ trusting attitudes grow, both within and outside the family.  相似文献

This paper reviews a decade of research (2006–2016) on a family assessment instrument called the Systemic Clinical Outcome and Routine Evaluation (SCORE). The SCORE was developed in Europe to monitor progress and outcome in systemic therapy and has been adopted by the European Family Therapy Association as the main instrument for assessing the outcome in systemic family and couple therapy. There are currently six main versions of this instrument: SCORE‐40, SCORE‐15, SCORE‐28, SCORE‐29, Child SCORE‐15, and Relational SCORE‐15. It has also been translated into a number of European languages. Fifteen empirical studies of the SCORE “family of measures” have been conducted. Most have aimed to establish psychometric properties of these instruments in English and other languages. Others have used the SCORE to document the level of family adjustment in clinical samples or evaluate outcome in treatment trials. There is now sufficient evidence for the reliability and validity of the SCORE to justify the use of brief versions of this instrument to monitor progress and outcome in the routine practice of systemic therapy.  相似文献

以348名中小学教师为被试, 运用《教师工作家庭冲突问卷》、《教师自我决定动机问卷》与《教师职业倦怠问卷》, 探讨了自我决定动机在中小学教师工作家庭冲突与职业倦怠关系中的中介作用。研究发现: (1) 中小学教师自我报告的工作干扰家庭水平高于家庭干扰工作水平;(2)家庭干扰工作、工作干扰家庭与自我决定动机、职业倦怠之间均存在显著的相关关系;(3)自我决定动机在中小学教师工作家庭冲突对职业倦怠的影响过程中所起的中介效应显著, 具体为在家庭干扰工作与职业倦怠的关系中起部分中介作用, 在工作干扰家庭与职业倦怠的关系中起完全中介作用。  相似文献

Children with disruptive behaviors are at risk for adverse outcomes. Family involvement is a significant predictor of positive child behavior outcomes; however, little research has investigated parent psychological variables that influence family involvement for children with disruptive behaviors. This study investigated the role of parental motivational beliefs (i.e., role construction and efficacy) as a potential mechanism by which parenting stress impacts family involvement for families of children with disruptive behaviors. Results indicated that parent role construction mediated the relation between parenting stress and all aspects of family involvement examined (i.e., home-based involvement, school-based involvement, and home-school communication). Parent efficacy mediated the relation between parenting stress and home-based involvement only. Parents of children with disruptive behaviors reporting stress may experience negative beliefs about their role and efficacy to support their child's education, which may thereby negatively influence their actual involvement. Therefore, parent motivational beliefs may serve as an important point for intervention to support involvement of families of children with disruptive behavior.  相似文献

The aim of the study is to explore the process of microtransitions in families with adolescent children. Original methodological procedures were designed in order to have families as the objects of study and to analyze data with particular attention to the family process of change. A family interview focused on the adolescent and family change was conducted with 12 families having an adolescent child. As indicators of change, we used coordination and oscillation. Our results highlight different patterns of family interactions, illustrating various ways through which families deal with change. Conclusive remarks focus on the theoretical relevance of the study, the method and the implications for family practice and policy.  相似文献

The aim in this study was to underline the importance of family environment as a significant associate of suicide probability among university students. For this aim 226 Turkish university students completed Suicide Probability Scale, Family Environment Questionnaire, Beck Depression Inventory, and Positive and Negative Affect Scale. As a result of the formulated regression analysis, after controlling for the 51% of the total variance accounted for by the control variables (i.e., gender, age, living with family versus away from family, and Grade Point Averages) and mood related variables (i.e., depressive symptoms, and positive and negative affect), family cohesion still emerged as a significant associate of the suicide probability. These findings underline the importance of family relations as a risk factor of suicide among university students.This work has been supported by the Turkish Academy of Sciences, in the framework of the Young Scientist Award Program. This article suggests the use of cognitive-behavioral techniques, namely cognitive restructuring combined with family-of-origin sessions, for reconstructing intergenerational family schemas. A case example portrays the use of the techniques as an effective intervention for addressing ingrained schemas, such as in the case of spousal abuse.Portions of this article were drawn from a lecture provided to the University of London’s Institute of Psychiatry at the Maudsley Hospital, UK in August of 2005.
