This paper addresses the question of what is involved in psychodynamic counselling with borderline clients. The concept of borderline functioning or structure is explained and the technical difficulties of working in this area noted, especially in the establishment of a working alliance. The types of transference manifestations with borderline clients are then described, looking in particular at the emergence of a psychotic transference. Two case examples are given of working with borderline clients, where the transference was dominated by primitive elements, provoking powerful countertransference reactions in the counsellor. How both the client and the counsellor can be contained in the therapeutic work is then addressed. Finally, some implications of this discussion are drawn out for the training of psychodynamic counsellors, in order to equip them best for working with borderline clients. Three features of what a training should offer are identified: it should provide students with the opportunity for the exploration of the psychotic parts of their own personality; it should allow for and encourage the internalization of psychoanalysis itself as a sustaining internal object; and it should provide students with the experience of working with clients over sufficient time and at sufficient depth so they can learn about the timing and effectiveness of interventions in work with borderline clients.  相似文献

This article explores the scaffolding of learning experiences in a postgraduate program in New Zealand that offers training in narrative counseling. The authors draw on positioning theory to identify student shifts in learning, and in agency, that help build an increasingly skilled and peer-generated context for learning. We describe a selection of exercises and one key assignment, introduced in the course in a particular order, that we believe enable students to step into positions of agency which ultimately allow a competent community of learner-practitioners to emerge. We also describe a dance of positioning for ourselves as teachers in this program. We suggest that, at any given time, our own positioning is tied up with possibilities for student positioning. Acknowledging relationships of power in classrooms, we explore ways to align poststructuralist counseling practices and the teaching of these practices.  相似文献

Previous studies on working memory training have indicated that transfer to non-trained tasks of other cognitive domains may be possible. The aim of this study is to compare working memory training and transfer effects between younger and older adults (n = 60). A novel approach to adaptive n-back training (12 sessions) was implemented by varying the working memory load and the presentation speed. All participants completed a neuropsychological battery of tests before and after the training. On average, younger training participants achieved difficulty level 12 after training, while older training participants only reached difficulty level 5. In younger participants, transfer to Verbal Fluency and Digit Symbol Substitution test was found. In older participants, we observed a transfer to Digit Span Forward, CERAD Delayed Recall, and Digit Symbol Substitution test. Results suggest that working memory training may be a beneficial intervention for maintaining and improving cognitive functioning in old age.  相似文献

In this study, we contrast results from two differential item functioning (DIF) approaches (manifest and latent class) by the number of items and sources of items identified as DIF using data from an international reading assessment. The latter approach yielded three latent classes, presenting evidence of heterogeneity in examinee response patterns. It also yielded more DIF items with larger effect sizes and more consistent item response patterns by substantive aspects (e.g., reading comprehension processes and cognitive complexity of items). Based on our findings, we suggest empirically evaluating the homogeneity assumption in international assessments because international populations cannot be assumed to have homogeneous item response patterns. Otherwise, differences in response patterns within these populations may be under-detected when conducting manifest DIF analyses. Detecting differences in item responses across international examinee populations has implications on the generalizability and meaningfulness of DIF findings as they apply to heterogeneous examinee subgroups.  相似文献

A dynamic model of affect suggests that positive and negative affect (PA and NA) are normally relatively independent of one another, whereas the heightened apprehensiveness and narrowed cognitive attention in persons with anxiety may contribute to a more unidimensional affect structure. This possibility was examined in a sample of 230 patients seeking treatment for anxiety and depressive disorders in the Netherlands. Two methods, a multiple-group confirmatory factor analysis (CFA) and Fisher's z test of correlations, were used to test these predicted relationships within a sample of persons diagnosed with either a depressive or an anxiety disorder. Both methods supported these predictions, with the depressed group exhibiting relatively independent PA and NA while the anxious group's affects were more strongly inversely correlated.  相似文献

Background/Objective: Intimate partner relationship problems and intimate partner abuse and neglect — referred to in this paper as “relational problems and maltreatment” — have substantial and well-documented impact on both physical and mental health. However, classification guidelines, such as those found in the International Classification of Diseases (ICD-10), are vague and unlikely to support consistent application. Revised guidelines proposed for ICD-11 are much more operationalized. We used standardized clinical vignette conditions with an international panel of clinicians to test if ICD-11 changes resulted in improved classification accuracy. Method: English-speaking mental health professionals (N = 738) from 65 nations applied ICD-10 or ICD-11 (proposed) guidelines with experimentally manipulated case presentations of presence or absence of (a) individual mental health diagnoses and (b) relational problems or maltreatment. Results: ICD-11, compared with ICD-10, guidelines resulted in significantly better classification accuracy, although only in the presence of co-morbid mental health problems. Clinician factors (e. g., gender, language, world region) largely did not impact classification performance. Conclusions: Despite being considerably more explicated, raters’ performance with ICD-11 guidelines reveals training issues that should be addressed prior to the release of ICD-11 in 2018 (e. g., overriding the guidelines with pre-existing archetypes for relationship problems and physical and psychological abuse).  相似文献

The article explores the fundamental difference between two aspects of justice: international and global. It is then argued that for the sake of global justice, the difference can be overcome by taking a closer look at the basic human right of self-preservation in relation to moral agency, human well-being and social/distributive justice at both global and national levels. The authors address the prophylactic value of behavioral techniques in relation to children's maladaptive fears. The major settings of interest from the viewpoint of preventing children's maladaptive fears are the home, school and health-care settings. Encouraging findings have been reported in the use of film-modeling with children about to undergo dental treatment, tonsillectomies, injections, etc. Several theoretical explanations (stimulus pre-exposure, reciprocal inhibition, reinforcement contingencies, self-efficacy) of the success of such interventions can be advanced to affect the outcome of maladaptive fear prevention programs.  相似文献

The American Association for the Advancement of Tension Control was founded in 1973 as an interdisciplinary effort encompassing the fields of dentistry, education, medicine, physical therapy, psychology, and speech pathology. The broad purpose was to facilitate the acquisition, dissemination, and application of sound knowledge for the benefit of society throughout the world. Annual meetings were held through 1978 with dues-paying members numbering almost 1000. Name changes were first to the International Stress and Tension-Control Association and then to the International Stress Management Association (ISMA) whereupon international meetings were held in 1979, 1981, 1983, 1988, and 1992.
