Evidence supports the use of rhythmic external auditory signals to improve gait in PD patients (Arias & Cudeiro, 2008; Kenyon & Thaut, 2000; McIntosh, Rice & Thaut, 1994; McIntosh et al., 1997; Morris, Iansek, & Matyas, 1994; Thaut, McIntosh, & Rice, 1997; Suteerawattananon, Morris, Etnyre, Jankovic, & Protas , 2004; Willems, Nieuwboer, Chavert, & Desloovere, 2006). However, few prototypes are available for daily use, and to our knowledge, none utilize a smartphone application allowing individualized sounds and cadence. Therefore, we analyzed the effects on gait of Listenmee®, an intelligent glasses system with a portable auditory device, and present its smartphone application, the Listenmee app®, offering over 100 different sounds and an adjustable metronome to individualize the cueing rate as well as its smartwatch with accelerometer to detect magnitude and direction of the proper acceleration, track calorie count, sleep patterns, steps count and daily distances. The present study included patients with idiopathic PD presented gait disturbances including freezing. Auditory rhythmic cues were delivered through Listenmee®. Performance was analyzed in a motion and gait analysis laboratory. The results revealed significant improvements in gait performance over three major dependent variables: walking speed in 38.1%, cadence in 28.1% and stride length in 44.5%. Our findings suggest that auditory cueing through Listenmee® may significantly enhance gait performance. Further studies are needed to elucidate the potential role and maximize the benefits of these portable devices.  相似文献

Freezing of gait (FOG) in Parkinson’s disease (PD) is typically assumed to be a pure motor deficit, although it is important to consider how an abrupt loss of gait automaticity might be associated with an overloaded central resource capacity. If resource capacity limits are a factor underlying FOG, then obstacle crossing may be particularly sensitive to dual task effects in eliciting FOG. Participants performed a dual task (auditory digit monitoring) in order to increase cognitive load during obstacle crossing. Forty-two non-demented participants (14 PD patients with FOG, 13 PD who do not freeze, and 14 age-matched healthy control participants) were required to walk and step over a horizontal obstacle set at 15% of the participants’ height. Kinematic data were split into two phases of their approach: early (farthest away from the obstacle), and late (just prior to the obstacle). Interestingly, step length variability and step time variability increased when PD patients with FOG performed the dual task, but only in the late phase prior to the obstacle (i.e. when closest to the obstacle). Additionally, immediately after crossing, freezers landed the lead foot abnormally close to the obstacle regardless of dual task condition, and also contacted the obstacle more frequently (planning errors). Strength of the dual task effect was associated with low general cognitive status, declined executive function, and inappropriate spatial planning, but only in the PD-FOG group. This study is the first to demonstrate that cognitive load differentially impacts planning of the final steps needed to avoid an obstacle in PD patients with freezing, but not non-freezers or healthy controls, suggesting specific neural networks associated with FOG behaviours.  相似文献

Weighted walking is a functional activity common in daily life and can influence risks for musculoskeletal loading, injury and falling. Much information exists about weighted walking during military, occupational and recreational tasks, but less is known about strategies used to accommodate to weight carriage typical in daily life. The purposes of the study were to examine the effects of weight carriage on kinematics and peak ground reaction force (GRF) during walking, and explore relationships between these variables. Twenty subjects walked on a treadmill while carrying 0, 44.5 and 89 N weights in front of the body. Peak GRF, sagittal plane joint/segment angular kinematics, stride length and center of mass (COM) vertical displacement were measured. Changes in peak GRF and displacement variables between weight conditions represented accommodation. Effects of weight carriage were tested using analysis of variance. Relationships between peak GRF and kinematic accommodation variables were examined using correlation and regression. Subjects were classified into sub-groups based on peak GRF responses and the correlation analysis was repeated. Weight carriage increased peak GRF by an amount greater than the weight carried, decreased stride length, increased vertical COM displacement, and resulted in a more extended and upright posture, with less hip and trunk displacement during weight acceptance. A GRF increase was associated with decreases in hip extension (|r| = .53, p = .020) and thigh anterior rotation (|r| = .57, p = .009) displacements, and an increase in foot anterior rotation displacement (|r| = .58, p = .008). Sub-group analysis revealed that greater GRF increases were associated with changes at multiple sites, while lesser GRF increases were associated with changes in foot and trunk displacement. Weight carriage affected walking kinematics and revealed different accommodation strategies that could have implications for loading and stability.  相似文献

A major characteristic of hemiplegic gait observed in individuals post-stroke is spatial and temporal asymmetry, which may increase energy expenditure and the risk of falls. The purpose of this study was to examine the effects of swing resistance/assistance applied to the affected leg on gait symmetry in individuals post-stroke. We recruited 10 subjects with chronic stroke who demonstrated a shorter step length with their affected leg in comparison to the non-affected leg during walking. They participated in two test sessions for swing resistance and swing assistance, respectively. During the adaptation period, subjects counteracted the step length deviation caused by the applied swing resistance force, resulting in an aftereffect consisting of improved step length symmetry during the post-adaptation period. In contrast, subjects did not counteract step length deviation caused by swing assistance during adaptation period and produced no aftereffect during the post-adaptation period. Locomotor training with swing resistance applied to the affected leg may improve step length symmetry through error-based learning. Swing assistance reduces errors in step length during stepping; however, it is unclear whether this approach would improve step length symmetry. Results from this study may be used to develop training paradigms for improving gait symmetry of stroke survivors.  相似文献

The ability to safely perform cognitive-motor dual-tasks is critical for independence of older adults. We compared age-associated differences in global and segmental control during dual-task walking in sub-optimal sensory conditions. Thirteen young (YA) and 13 healthy older (OA) adults walked a straight pathway with cognitive dual-task of walking-while-talking (WT) or no-WT under four sensory conditions. On randomly selected trials, visual and vestibular inputs were manipulated using blurring goggles (BV) and Galvanic Vestibular Stimulation (GVS), respectively. Gait speed decreased more in YA than OA during WT. Gait speed increased with GVS with normal vision but not BV. Step length considerably decreased with WT. Trunk roll significantly decreased only in OA with GVS in WT. Head roll significantly decreased with GVS regardless of age. Results indicate GVS-induced adaptations were dependent on available visual information. YA reduced their gait speed more than OA to achieve a similar pace to safely perform WT. GVS resulted in both age-groups to reduce head movement. However, with the addition of WT during GVS, OA also stiffened their trunk. Therefore, with increased attentional demands healthy OA employed different compensatory strategies than YA to maintain postural control.  相似文献

Psychoanalysts’ written reports on initial consultations are a window into the complexities of a crucial aspect of psychoanalytic work. However, systematic research in this area has largely focused on patients’ demographic factors or standardized measures. The present study looked at reports of all the consultations taking place at the London Clinic of Psychoanalysis over one calendar year (N = 100). The aim was to explore psychoanalysts’ different explicit styles of working and reporting as well as further understanding implicit processes used in thinking and writing about each particular consultation experience. A thematic analysis revealed a set of themes that related to a style of working and thinking about the consultation process as a dyadic experience, where the interaction, affective reactions and contact made between the two are the focus when thinking of making a recommendation for psychoanalysis. The majority of the reports had an open, exploratory quality. The writing of reports appears to give the analyst an opportunity to process the consultation experience and arrive at a more triangular position. Writing reports is a more valuable part of the consultation process than has formally been recognized and acknowledged. The limitations of this study as well as the relevance of these findings for future research are discussed.  相似文献

Asymmetric gait is a hallmark of many neurological and musculoskeletal conditions. This behavior is often the result of a decrease in the stability of interlimb coordination, and synchronization to external signals such as auditory cuing or another walking individual may be helpful for altering abnormal movement patterns. The purpose of this study was to investigate the interaction between interlimb coordination and unintentional, interpersonal synchronization of gait in healthy individuals in response to unilateral ankle loading. Fifty participants completed four trials while walking on a motorized treadmill: (1) by themselves, (2) with a partner on an adjacent treadmill, (3) by themselves with additional weight applied unilaterally to their right ankle, and (4) with both a partner and unilateral weight. As expected, the addition of unilateral weight increased asymmetry according to several spatiotemporal measures of gait, but the presence of a partner on an adjacent treadmill significantly reduced this effect. Further, the amount of unintentional, interpersonal synchronization among pairings was relatively unaffected by the addition of ankle weight to one of the partners. All pairings realized a beneficial effect on asymmetrical gait but this effect was greater for pairings that consistently synchronized unintentionally. These results suggest that side by side walking might be an effective approach for influencing bilateral coordination of gait and may hold insight for understanding gait asymmetry and interlimb movement variability.  相似文献

Certain styles of children’s shoes reduce 1st metatarsophalangeal joint (MTPJ) and midfoot motion during propulsion of walking. However, no studies have investigated if the splinting effect of shoes on children’s 1st MTPJ and midfoot motion occurs during running. This study investigated the effect of sports shoes on multi-segment foot kinematics of children during propulsion of walking and running. Twenty children walked and ran at a self-selected velocity while barefoot and shod in a random order. Reflective markers were used to quantify sagittal plane motion of the 1st MTPJ and three-dimensional motion of the midfoot and ankle. Gait velocity increased during shod walking and running and was considered a covariate in the statistical analysis. Shoes reduced 1st MTPJ motion during propulsion of walking from 36.0° to 10.7° and during running from 31.5° to 12.6°. Midfoot sagittal plane motion during propulsion reduced from 22.5° to 6.2° during walking and from 27.4° to 9.6° during running. Sagittal plane ankle motion during propulsion increased during shod running from 26.7° to 34.1°. During propulsion of walking and running, children’s sports shoes have a splinting effect on 1st MTPJ and midfoot motion which is partially compensated by an increase in ankle plantarflexion during running.  相似文献

The pendular mechanism does not act as a primary mechanism in uphill walking due to the monotonic behavior of the mechanical energies of the center of mass. Nevertheless, recent evidence shows that there is an important minimization of energy expenditure by the pendular mechanism during walking on uphill gradients. In this study, we analyzed the optimum speed (OPT) of loaded human walking and the pendulum-like determining variables (Recovery R, Instantaneous pendular re-conversion Rint, and Congruity percentage %Cong). Ten young men walked on a treadmill at five different speeds and at three different treadmill incline gradients (0, +7 and +15%), with and without a load carried in their backpacks. We used indirect calorimetry and 3D motion analysis, and all of the data were analyzed by computational algorithms. Rint increased at higher speeds and decreased with increasing gradient. R and %Cong decreased with increasing gradient and increased with speed, independent of load. Thus, energy conversion by the pendular mechanism during walking on a 15% gradient is supported, and although this mechanism can explain the maintenance of OPT at low walking speeds, the pendular mechanism does not fully explain the energy minimization at higher speeds.  相似文献

Undergoing an admission process (an initiation) can induce exaggerated feelings about a group, but there is little research about the role of rewards. This study replicated Aronson and Mills’ (1959) experiment. Seventy participants underwent either a severe initiation or a mild initiation. After the initiation, about half the sample received an extrinsic reward for merely completing the task. The remaining half did not receive an extrinsic reward. This was to vary the amount of dissonance. Initiation severity and reward condition had significant, noncrossing interactions. A reward led to higher group identity than no reward, supporting Levine and Moreland's (1994) group socialization model. A severe initiation did not lead to more group identity than a mild initiation; therefore, Aronson and Mills’ findings were not replicated. Interestingly, a mild initiation followed by a reward led to more group identity than a severe initiation followed by a reward. Another unexpected finding was that the extrinsic reward made no difference to group identity if the initiation was severe. Effects on ratings of the discussion were nonsignificant. Future research needs to establish how new group members ponder the severity of the admission process during the cost–benefit calculation preceding their identification with a group.  相似文献
