IntroductionAlcohol interlock programs (AIP) are aimed at preventing drunk-driving recidivism. They first appeared in Europe in the nineties.ObjectivesThe purpose of this study was investigate whether AIPs are effective in reducing recidivism and determine what factors of such programs contribute to their positive impact.MethodWe performed a follow-up of AIP participants (n = 175) and control participants (n = 234) for five years. Data such as blood alcohol concentration (BAC) at time of arrest, previous and subsequent violations (alcohol-related or other types) were collected from the official driver's license files. For self-evaluation and evaluation of the program (since November 2009 only), we used questionnaires based on two theoretical models: the Transtheoretical Model of Change (Prochaska & DiClemente, 1984) and the Diamond of Change Model from the DRUID project, build to explain which are the elements of driver rehabilitation training courses that favor behavioral modification (Bukasa et al., 2009).ResultsThe data obtained allowed us to describe these populations of alcohol-drinking offenders, for both the interlock program group and the control group. Two salient features were male proneness to alcohol-related violations and a very high BAC (between 1.5 and 2 g/l) among the majority of drivers who drink under the influence of alcohol. In terms of recidivism reduction, the interlock program was not clearly linked to a beneficial effect. An analysis of behavioral change among the AIP participants showed that consciousness raising took effect more quickly than did environmental reevaluation or reinforcement management.ConclusionsThe application of Prochaska et al.’s model to the follow-up of AIP programs would be more fruitful in the future if psychologists are involved in the program. As we have seen, medical/psychological monitoring may not have been strong enough in the program studied here.  相似文献

Research on alcohol consumption suggests that friendship and peer relations are highly influential and have a decisive impact on drinking behaviours. Further, that women's friendships appear to be characterised by emotional disclosure and ‘knowing’ one another. In this paper I draw on debates around emotional intimacy, ideas of ‘closeness’ and ‘distance’ to explore how this might work when young women go out and drink alcohol. Drawing on in-depth interviews with 25 British undergraduate female students aged 18–25, this research suggests that one of the compelling appeals of alcohol is that it creates a ‘space of suspension’, whereby feelings of being both close and distant (intimate proximity) coexist, often for strategic reasons associated with security. Two themes emerged from the data: firstly that the young women had a preference for paradoxically maintaining ‘distance’ alongside ‘closeness’ with friends, and secondly, that contingencies of safety and vulnerability promoted the need for co-operation to manage risk in the night-time economy. These themes permeated relationships and became a significant dynamic in group behaviour creating a tension between functionality and friendship. This paper contributes to a growing body of work that suggests intimacy and women's friendships are complex: traditional conceptions about young women seeking intimate and close friendships need revaluating to take into consideration how ‘distance’ within groups is both desirable and negotiated, and may be facilitated through the drinking of alcohol.  相似文献

Awareness of health risks linked with excessive alcohol consumption appears to have little influence on how much some people drink. Compensatory health beliefs (CHBs), in which the consequences of unhealthy behaviour are considered to be neutralised by additional healthy behaviours, are one way of justifying poor health choices. Currently, the role of CHBs within the context of drinking behaviour is not well understood. This research examined associations between alcohol specific compensatory health beliefs (ACH-Beliefs) and behaviours (ACH-Behaviours), alcohol consumption and alcohol specific self-efficacy (ASE), via an online survey completed by 249 participants, aged 18 + years (63.1% female; M age = 41.62 years; SD = 14.80). Higher ACH-Beliefs were associated with increases in ACH-Behaviours. While both predicted alcohol consumption, a greater proportion of variance was explained by ACH-Behaviours. ASE was a significant mediator of those relationships, suggesting that those with higher ASE may be better equipped to regulate drinking behaviour. Recommendations for future research include measuring both CHBs and behaviours within an experimental design, and further investigation of related cognitions such as compensatory behaviour intentions. Alcohol misuse interventions may wish to consider the potential roles of CHBs and behaviours in facilitating maladaptive coping strategies, and how addressing these may reduce harms.  相似文献

A variety of neurodevelopmental impairments related to fetal alcohol spectrum disorder (FASD) diagnoses have been consistently documented. However, it is not clear whether such variables are predictive of a diagnosis. The purpose of the present study is to use logistic regressions to identify predictors of FASD in neuropsychological assessment. Charts of 180 children and adolescents with prenatal alcohol exposure (PAE) who underwent psychological and diagnostic assessment for FASD were retrospectively reviewed. A total of 107 received an FASD diagnosis (the PAE-FASD group) and 73 did not (the PAE group). Following preliminary analyses, direct logistic regressions were performed to assess the contribution of different neuropsychological testing measures on the likelihood of a child or adolescent receiving an FASD diagnosis. The results indicate that the classification accuracy of the PAE-FASD and PAE groups is clinically significant across models of intelligence, academic achievement, memory, and executive functioning. Classification rates across the various models range from 67.1% to 75.5%, with models incorporating 10 intelligence subtests or 3 academic subtests emerging as superior to those using broad indices of intelligence and/or individual subtests of memory or executive functioning. A “test battery” model incorporating verbal intelligence, verbal/auditory working memory (digit span), basic reading and spelling skills, math calculations, delayed story recall, and spatial planning and problem-solving yielded a classification rate of 74.7%. These results suggest that neuropsychological testing is a critical component of FASD assessment and help guide decisions to maximize the efficiency and efficacy of the diagnostic process and treatment recommendations.  相似文献

Intra-individual variability (IIV) is defined as systematic within-person variation in performance either across test sessions (e.g., test/retest performance on the same task) or in one session (e.g., variations in performance on multiple trials of a single task). Higher levels of IIV have been noted as a characteristic of neurodevelopmental disorders such as attention deficit/hyperactivity disorder? (ADHD), but IIV is yet to be investigated in fetal alcohol spectrum disorder (FASD). FASD is a term used to describe a range of conditions resulting from prenatal exposure to alcohol. As part of a comprehensive neuropsychological battery, four study groups (1. fetal alcohol syndrome/partial fetal alcohol syndrome; 2. static encephalopathy/alcohol exposed; 3. neurobehavioral disorder/alcohol exposed as diagnosed using the University of Washington FASD 4-Digit Code; 4. typically-developing (TD) age-matched children with no prenatal alcohol exposure) were administered measures of motor response and inhibitory control, attention, and adaptive behavior. The results indicate increased levels of IIV in those with FASD compared to the TD controls. It was found that IIV uniquely contributes to predicting adaptive behavior above and beyond attention, while attention partially mediates the relationship between IIV and adaptive behavior. This is the first study to the authors’ knowledge to show the presence of increased IIV in children with FASD. It additionally provides evidence that IIV measures some inherent variability in performance independent of poor attention in children with FASD.  相似文献

Evidence suggests that the antibiotic d-cycloserine (DCS) enhances the treatment effects of exposure and response prevention (ERP) for Obsessive-Compulsive Disorder (OCD). Further, evidence suggests that the effects of DCS diminish partway through treatment, but it is unclear to what extent. In an effort to evaluate these issues, the current study re-analyzes data from a 10-session randomized controlled trial of ERP + DCS versus ERP + placebo in a sample of 22 adults with OCD. We analyzed repeated-measures mixed models with random slopes and intercepts across different intervals: sessions 1-10, 1-5, and 6-10. The results indicate that the course of ERP was 2.3 times faster over the full 10 sessions for the DCS compared to the placebo group, and nearly six times quicker in the first half of ERP. Further interpretation of the results suggests that DCS does not amplify the effects of ERP, but instead initiates treatment effects sooner in treatment. In addition, DCS does not necessarily lose its effect over repeated use, but instead may exhaust its maximum utility after effectively jump-starting ERP. Ultimately, DCS may provide a means for curtailing treatment costs, decreasing treatment dropout and refusal rates, and enhancing access to care.  相似文献

PurposeThis study described the proportion of children who stutter who exhibit Attention Deficit Hyperactivity Disorder (ADHD) symptoms, manifesting in inattentive and hyperactive/impulsive behaviours. Children who stutter with these challenging behaviours may not respond as quickly and successfully to stuttering treatment. A preliminary exploration of differences in treatment responsiveness for children with and without ADHD symptoms was undertaken.MethodParticipants were 185 preschool children who stutter who had completed stuttering therapy within 3 months prior to study commencement. Differences between groups of children who stutter with and without elevated ADHD symptoms were investigated, in terms of pre-treatment stuttering features (stuttering severity and typography), demographic variables (age at onset, time between onset and commencement of therapy, family history and sex) and treatment data (post-treatment stuttering severity and number of sessions to achieve discharge criteria).ResultsOne-half (50%) of participants exhibited elevated ADHD symptoms. These children required 25% more clinical intervention time to achieve successful fluency outcomes than children without elevated ADHD symptoms. Findings suggest that more ADHD symptoms, increased pre-treatment stuttering severity, and male sex were associated with poorer responsiveness to stuttering treatment.ConclusionThe large proportion of children exhibiting elevated ADHD symptoms, and the increase in clinical contact time required in this subgroup to achieve successful fluency outcomes, is suggestive of the need for clinicians to tailor stuttering intervention to address these concomitant behaviour challenges. Findings support the use of careful caseload management strategies to account for individual differences between children, and strengthen prognostic information available to parents and clinicians.  相似文献

Motor difficulties may be an early Autism Spectrum Disorder (ASD) risk indicator and may predict subsequent expressive language skills. Further understanding of motor functioning in the first year of life in children with ASD is needed. We examined motor skills in 6-month-olds (n = 140) at high and low familial risk for ASD using the Peabody Developmental Motor Scales (Grasping, Visual-Motor Integration, and Stationary subscales). In Study 1, motor skill at 6 months predicted ASD status at 24–36 months; ASD was associated with poorer infant motor skills. In Study 2, motor skill at 6 months predicted expressive language at 30 and 36 months. Findings provide evidence that vulnerability in motor function early in development is present in ASD. Findings highlight the importance of developmental monitoring in high-risk infants and possible cascading effects of early disruption in motor development.  相似文献

The aim of the present study was to examine the relationship between the Five-Factor Model of personality and alcohol expectancies (AEs) with different alcohol outcomes. The sample was composed of 738 participants (63.7% females). Path and regression analyses were performed to test the mediation and moderation effects. The results indicated that Neuroticism was related to alcohol consumption through Positive Alcohol Expectancies, and that Negative Alcohol Expectancies, but also Positive Alcohol Expectancies, partially mediated the relationship of Neuroticism to alcohol-related problems. In addition, Positive Alcohol Expectancies partially mediated the associations of extraversion and low conscientiousness with weekend Standard Drink Units (SDUs), and they completely mediated the associations of these personality variables with alcohol-related problems. Additional direct paths were found from low agreeableness to weekly SDUs and alcohol-related problems; and from low openness to weekend SDUs. Moderation effects of alcohol expectancies on personality and both alcohol use and alcohol-related problems were also found. The present research contributes new evidence on the influence of the five factors of personality on alcohol outcomes, and the mediation/moderation role of alcohol expectancies. These findings can be useful to develop prevention/intervention programmes.  相似文献

Objective: Pre-drinking, the practice of consuming alcohol prior to attending a subsequent event, increases the risk of alcohol-related harm, and is common in undergraduate student populations. The current study tested an integrated behaviour change model to identify the motivational, social-cognitive, and implicit predictors of pre-drinking.

Design: University students (N?=?289) completed an online questionnaire comprising measures of motivational and social-cognitive constructs related to reducing pre-drinking alcohol consumption and past behaviour, and an implicit association test for drinking identity. Participants reported their pre-drinking alcohol consumption at follow-up, 4 weeks from baseline.

Main Outcome Measures: Self-reported pre-drinking alcohol consumption.

Results: A variance-based structural equation model revealed that few model hypotheses were supported. Although the effects of past behaviour, perceived behavioural control, and implicit drinking identity, on follow-up pre-drinking alcohol consumption were statistically significant, the effect of intention was not.

Conclusions: Current findings indicate pre-drinking alcohol consumption is associated with past behaviour, perceived behavioural control and implicit drinking identity, and no intentions to reduce pre-drinking alcohol consumption. The finding raise questions over the validity of applying the integrated model in this context. Interventions should consider these factors and attempt to facilitate the formation of intentions that lead to subsequent behaviour.  相似文献
