A novel classification framework for clinical decision making that uses an Extremely Randomized Tree (ERT) based feature selection and a Diverse Intensified Strawberry Optimized Neural network (DISON) is proposed. DISON is a Feed Forward Artificial Neural Network where the optimization of weights and bias is done using a two phase training strategy. Two algorithms namely Strawberry Plant Optimization (SPO) algorithm and Gradient-descent Back-propagation algorithm are used sequentially to identify the optimum weights and bias. The novel two phase training method and the stochastic duplicate-elimination strategy of SPO helps in addressing the issue of local optima associated with conventional neural networks. The relevant attributes are selected based on the feature importance values computed using an ERT classifier.Vertebral Column, Pima Indian diabetes (PID), Cleveland Heart disease (CHD) and Statlog Heart disease (SHD) datasets from the University of California Irvine machine learning repository are used for experimentation. The framework has achieved an accuracy of 87.17% for Vertebral Column, 90.92% for PID, 93.67% for CHD and 94.5% for SHD. The classifier performance has been compared with existing works and is found to be competitive in terms of accuracy, sensitivity and specificity. Wilcoxon test confirms the statistical superiority of the proposed method. 相似文献

Early mother‐child interaction is one of the factors suggested to have an impact on neurocognitive development of extremely low gestational age (ELGA) children. Our aim was to examine associations of mother‐child interaction with neurocognitive outcome, neurological impairments and neonatal brain injuries in ELGA children. A prospective study of 48 ELGA children, born before 28 gestational weeks (26.3 ± 1.2 weeks, birth weight 876 g ± 194 g), and 16 term controls. Brain MRI was performed at term‐equivalent age. At two years of corrected age, the mother‐child interaction was assessed in a structured play situation using the Erickson Scales and Mutually Responsive Orientation Scales. Neurocognitive outcome was assessed with Griffiths Mental Developmental Scales (GMDS) and Bayley Scales of Infant and Toddler Development ‐ Third Edition (BSID‐III) and with Hempel neurological examination. Among ELGA children, higher quality of dyadic relationship and maternal sensitivity, responsiveness, and supportiveness were associated with positive neurocognitive outcome measured both with GMDS and BSID‐III (adjusted p < 0.05). This association remained after adjusting for mother's educational level. Neurological impairments at two years, white matter or gray matter abnormalities in MRI at term‐equivalent age, and grade III‐IV intraventricular hemorrhage during the neonatal period were not associated with mother‐child interaction. This study emphasizes the importance of the quality of mother‐child interaction after extremely preterm birth for neurocognitive development. Neonatal brain injury and neurological impairments were not associated with worse parent‐child interaction after two years. 相似文献

The present study analyzed the links between prematurity, attention, and global cognitive performance in infancy and early childhood. At 7 months, focused attention (FA) was examined with an object examination task in 93 preterm infants (39 of them born extremely/very preterm, 54 born moderately/late preterm, and 38 infants born full-term). Global cognition was assessed at 7 and 24 months with the Bayley-II cognitive scale. Groups did not differ with respect to global cognitive performance but FA of infants born extremely/very preterm was significantly lower than in infants born moderately/late preterm. FA correlated significantly with both prematurity and cognitive performance at 7 months of age but not with global cognition in childhood. Findings point to a subtle adverse effect of prematurity on early attention and reveal evidence for the mediating role of FA on the effect of prematurity on cognition. 相似文献

The effects of ad libitum access to a pacifier on the behavioral state and motor activity of preterm infants have been compared with those observed in full-term neonates. Regardless of maturity, nonnutritive sucking (NNS) decreases the amount of time spent in active states and increases that spent in quiescent states, lengthens the longest state bout, and decreases the frequency of state transitions. NNS also reduces overall motor activity as well as that during Active Sleep. Provided such ad libitum sucking opportunity, preterm infants thus appear to derive no less benefit from NNS than do term neonates. This result contrasts with an earlier finding that preterms are less soothed by NNA than are term infants. 相似文献

Its consequences range from risk of infant mortality to lifelong disease and disability. Stress is a contributor to preterm labor and delivery. This article summarizes research on stress as a risk factor for PTB. Preterm born children have more behaviour problems than term born children. Perinatal risks, current child functioning, sociodemographic characteristics, parental psychological distress and parental perceptions of child vulnerability (PCV) have been shown to be risk factors for behaviour problems. However, the role of maternal and paternal PCV is unclear, as these have not been investigated as a risk factor for behaviour problems, with all other risk factors taken into account. Aim of this study is to investigate whether maternal and paternal PCV are independent risk factors for behaviour problems in very preterm (VP) and term born children. The present study is a single centre prospective cohort study. Preterm children (n = 104), born at <30 weeks' gestation and/or birth weight <1000 g, and term children (n = 95) were assessed at age 5. Results showed that risk factors for parent‐rated behaviour problems were low/middle parental education, VP birth, parental stress and, in VP children, maternal PCV. Risk factors for teacher‐rated behaviour problems were low/middle parental education, foreign parental country of birth, intrauterine growth restriction and objective child vulnerabilities. It can be concluded that maternal PCV is a risk factor for parent‐rated behaviour problems in VP children. When VP children are presented with behavioural problems, clinicians ought to be aware of the possibility that parents might still perceive their child as vulnerable. The neonatal history of the child, the way parents experienced that period, their perceptions of the child and possible consequences of these perceptions could be subjects for conversation during visits at follow‐up clinics. Play observations with a total of 400 toddlers and preschoolers were videotaped and rated for Intensity and Quality of play with their parents. Parents were asked about perceived stress and personality characteristics (Big 5). Child's motor, cognitive skills, temperament, and internalizing behaviors were assessed. Study 1 investigated the robustness of play across child age and gender, and examined differences between fathers and mothers. Study 2 explored the vulnerability of play with fathers of children born preterm (PT‐fathers) and fathers who had experienced adverse childhoods (AC‐fathers). Study 3 investigated child internalizing behaviors. Intensity of play was maintained almost independently of child age and gender. It was similar for AC‐ and PT‐fathers, and similar to maternal Intensity. In contrast, paternal Quality of play was higher with boys and independent of fathers’ personality and perceived parenting stress whereas maternal Quality of play was higher with girls and linked to mothers’ perceived parenting competence, acceptability of the child, and neuroticism. AC‐fathers scored significantly low on Quality, as did PT‐fathers, but the Quality of their play became better with growing child age, birth weight, and cognitive (but not motor and temperament) scores. Finally, child internalizing behaviors were negatively related to paternal Quality of play. 相似文献

The validity of a Personality Inventory for Children-Revised edition (PIC-R) typology was examined in a sample of 323 children aged 6–16 years. These children had been referred to a children's mental health centre for neuropsychological assessment. In study 1, K-means cluster analysis (k = 12) was applied to the PIC clinical scales in an attempt to replicate the 12 clusters identified by Gdowski, Lachar, and Kline (1985). Partial cluster replication was achieved. Examination of the obtained clusters revealed significant overlap, suggesting that fewer clusters would represent an optimal solution. In study 2, a two-stage cluster analysis yielded a seven-cluster solution consistent with several key forms of psychopathology previously reported in the literature using specific neuropsychological populations. Identified subtypes included profiles characterized as: normal, cognitive deficit, cognitive deficit with internalized psychopathology, cognitive deficit with social impairment, cognitive deficit with hyperactivity, cognitive deficit with both internalized and externalized psychopathology, and combined internalized and externalized psychopathology without a cognitive deficit component. 相似文献
