A nonblinded randomized trial was conducted at two Canadian provincial outpatient addiction clinics that tested the effectiveness of a systemic congruence couple therapy (CCT) versus individual-based treatment-as-usual (TAU) on nine clinical outcomes: (1) primary outcomes—alcohol use and gambling, psychiatric symptoms, and couple adjustment; (2) secondary outcomes—emotion regulation, substance use, depression, post-traumatic stress symptoms, and life stress. Data of primary clients and partners (N = 46) were analyzed longitudinally across baseline, posttreatment (5 months), and follow-up (8 months). Alcohol use disorder (95%) and gambling disorder (5%) were in the severe range at baseline, and co-addiction was 27%. Psychiatric comorbidity was 100%, and 18% of couples were jointly addicted. Between-group comparison favored CCT in primary outcomes with medium-to-large effect sizes (Cohen's h = 0.74–1.44). Secondary outcomes were also significantly stronger for CCT (Cohen's h = 0.27–1.53). Within-group, for all primary outcomes, a significant proportion of symptomatic CCT clients and partners improved, converging with ANOVA results of large effect sizes (0.14–0.29). All secondary outcomes improved significantly in CCT with large effect sizes (0.14–0.50). TAU showed significant within-group improvement in alcohol use, other substance use, and life stress with large effect sizes (0.16–0.40). Primary clients and partners made largely equivalent improvement within CCT and within TAU. Results were triangulated with clients' satisfaction ratings and counselors' reports. Overall, significant within-group effects were detected for CCT both clinically and statistically and between-group difference favored CCT. Future trials are required to validate these promising findings.  相似文献

The proportion dominance effect (PDE) refers to a higher motivation to help when the victims are part of a small (you can help 56 out of 60) rather than a large (you can help 56 out of 560) reference group. In two studies using different experimental paradigms, we investigated possible mediators of the PDE. Study 1 (N = 168) was conducted in three separate steps in order to test each link of the mediator model independently. Students read six vignettes where it was possible to help a fixed number of victims but where the size of the reference group was either small or large. When the reference group was small, helping motivation and perceived utility were higher, whereas sympathy toward the victims and perceived rights were not. A within‐subject mediation analysis showed that perceived utility mediated the PDE. Study 2 (N = 36) presented four versions of a single helping situation in a joint evaluation mode where the size of the reference group became gradually smaller in each version. All participants compared and responded to each version. Helping motivation increased as the reference group became smaller, and this effect was mediated by perceived utility rather than by distress, sympathy, or perceived responsibilities. Our results suggest that unlike, for example, the identifiability and singularity effects, which have been suggested to be mediated by emotional reactions, the PDE is mediated by perceived utility. This study aimed to examine the importance of value congruence in work engagement and to extend the previous literature by investigating the potential mediating effects of autonomous and controlled motivation in this relationship. The Perceived Value Congruence, Autonomous and Controlled Motivation and Work Engagement scales were administered to 767 teachers in Mainland China. Correlation analysis revealed that value congruence, autonomous and controlled motivation and work engagement were significantly correlated with each other. Furthermore, structural equation modeling (SEM) analysis showed that value congruence exerted its indirect effect on teacher’s work engagement through the mediating effect of autonomous and controlled motivation. These findings provide a new perspective to explain the complex association between value congruence and work engagement. The possible explanations and the research limitations are discussed.  相似文献

Background: It is vital that the treatment offered at transgender health services can be evaluated to ensure a high quality of care. However, the tools currently used to evaluate treatment at transgender health services are limited by mainly focusing on mental health or because they have been developed for binary transgender people only. This study therefore aimed to develop and validate a tool that addresses these limitations. The Gender Congruence and Life Satisfaction Scale (GCLS) was developed through reviewing the literature, conducting interviews with transgender people, and holding discussions with experts working in transgender healthcare. An initial pool of items was developed and feedback on these was obtained. The tool was then validated.

Method: For the validation of the tool, a total of 789 participants (451 transgender [171 transgender females, 147 transgender males, 133 people identifying as non-binary], and 338 cisgender [254 females, 84 males]) were recruited from the United Kingdom to test the factor structure and validity of the GCLS.

Results: Exploratory factor analysis retained 38 items which formed seven subscales (psychological functioning; genitalia; social gender role recognition; physical and emotional intimacy; chest; other secondary sex characteristics; and life satisfaction). These seven subscales were found to have good internal consistency and convergent validity. The GCLS was also found to be capable of discriminating between groups (e.g., people who have and have not undergone gender affirming medical interventions). Transgender and cisgender subscale norms are provided for the GCLS.

Conclusion: The GCLS is a suitable tool to use with the transgender population to measure health-related outcomes for both clinical and research purposes.  相似文献
