临终患者自主权的两难困境与出路

面对人口老龄化程度越来越高的社会,尊重临终患者的自主权既是对患者知情同意的尊重,也是对生命价值的尊重。临终患者自主权的实现途径中存在诸多两难问题。研究临终患者自主权的两难困境问题,有助于权衡不同价值观念之间的冲突,并且从中找到与之相应的解决方案。破解临终患者自主权两难困境的出路在于充分尊重临终患者的自主权,构建互信的医患关系和共同决策机制。并探索预设医疗指示制度和伦理审查制度在我国医疗环境下实施的可行性。     

The Relationship of Impulsiveness to Psychoticism and Extraversion in Elderly Psychiatric Patients

I conducted a correlational study to test the hypothesis that resource allocation outcomes become more favorable as uncertainty assessment accuracy (i.e., resolution and calibration) increases. For each of 32 college basketball games, participants predicted which team would win, estimated the subjective probability that their prediction was correct, and were given the opportunity to place a wager. The dependent variable was profit earned over the 32 bets. Proportion correct and mean certainty were controlled in the analyses. Results indicated that individuals with higher resolution scores earned significantly more profit than those with lower scores. Higher profits were also associated with better calibration.     

RESPONSE ALLOCATION IN CONCURRENT CHAINS WHEN TERMINAL‐LINK DELAYS FOLLOW AN ASCENDING AND DESCENDING SERIES

Eight pigeons were trained in a concurrent‐chains procedure in which the terminal‐link immediacy ratio followed an ascending or descending series. Across sessions, one terminal‐link delay changed from 2 s to 32 s to 2 s or from 32 s to 2 s to 32 s, while the other was always 8 s. For all pigeons, response allocation tracked changes in delay and was biased towards the 8‐s alternative on the descending series, indicating a hysteresis effect, and was more sensitive to changes in the terminal‐link delay ratio for relatively long (> 8 s) than short (< 8 s) delays. Both the hysteresis and effect of delay duration were predicted by an extended version of Grace and McLean's (2006) decision model. The extended decision model provided an overall better account of the results than a simple linear‐operator model (Grace, 2002), and holds promise for an integrated account of choice in concurrent chains for both acquisition and steady‐state conditions.

在人口老龄化的背景下,为尽快改善终末期患者的死亡质量,探求符合东方文化诉求的安宁疗护发展道路,从历史角度对中国台湾地区安宁疗护的发展历程进行梳理。依托相关规定和制度,中国台湾地区安宁疗护取得快速发展,在不同发展阶段依次完成了解决初期困难,完善服务内容,提高服务质量的发展目标。同时,安宁疗护发展目标的实现需要基金会、安宁疗护社团与各界热心人士的精诚合作和不懈努力。中国台湾地区安宁疗护未来的发展仍然面临挑战,质量把控、服务转型、体系融合将是其发展的方向。

     

Essential anxiety: COVID-19 in analytic practice

This paper explores the impact of the COVID-19 pandemic on my relationship with analysands and my inner world. I reflect on the role of the archetypal Self during times of existential anxiety that may lead to an experience of ‘essential anxiety’. This term refers to a meeting by a fearful ego with an inward recognition of the Self, when faced with threat. The efforts to curb the spread of the pandemic changed our ways of life, while the virus itself threatened our existence in debilitating or outright destructive ways. But what also came into view, in sessions of analysis and supervision, was the creative instinct, and a celebration of life. The soul-to-soul relationship, and the connection with images of the archetypal Self, made the experience of existential anxiety at times an essential experience that facilitated psychological growth. I discuss some advantages of on-line Jungian analysis where, despite distance and partial view, the body still serves as container to hold important psychological material, conferring a sense of wholeness for analyst and analysand. The COVID-19 crisis is terrible and terrifying but it also provides an opportunity for self-regulation and individuation.     

The effect of job stress on self-reported safety behaviour in container terminal operations: The moderating role of emotional intelligence

This study empirically examined the moderating effects of emotional intelligence on job stress and self-reported safety behaviour in the context of container terminal operations using survey data collected from 430 respondents who worked in the container terminals in Taiwan. A hierarchical regression analysis was used for data analysis. The research findings indicate that job stress negatively affects safety behaviour in terms of safety compliance, as well as emotional intelligence positively affects safety behaviour in terms of safety participation and safety compliance. Results also indicated that emotional intelligence plays a moderating role on the relationships between job stress and safety compliance. The theoretical and practical implications from the research findings are discussed.     

Grayson Perry and the recalibration of masculine emotional reactions to vulnerability and healing

The so-called ‘crisis in masculinity’ in the post-industrial/post-feminist world is not a new idea but in recent years it does seem to have forced its way back onto the agenda due to a lack of traditional employment, such as coal mining, thus robbing generations of men of their cultural identities. This collective ‘crisis’ has emerged through media attention on a succession of well-known men in the arts and sports worlds such as Bruce Springsteen, Stormzy, Professor Green and Rio Ferdinand, who share in common a strong sense of masculine identification. In addition, all have openly discussed the healing and relief experienced by embracing the vulnerable, emotional aspects of themselves, or what has been historically characterized as the anima archetype in Jungian psychology. This essay will recount some of their stories of struggle as they have bravely spoken up about their mental health issues, perhaps diminishing, by example, the erroneous notion of depression as a ‘woman's problem’. As noted, in recent times we have witnessed a proliferation of famous men speaking out about their emotional difficulties, often in the context of loss and depression, thus providing potential role-modelling for a more expanded view of contemporary masculinity. This paper attempts to consider the tendency amongst a particular type of modern man who is both traditionally macho and simultaneously emotionally articulate with the capacity to open up about the experiences of their inner worlds and the environmental pressures they face in a fast-paced and ever-changing world. Beginning with some brief stories about the men listed above, the primary focus will be on the work of the UK cross-dressing artist and Turner Prize winner (2003) Grayson Perry (b. 1960), by studying his unusual and highly creative clarion call to challenge modern men to recalibrate their emotional behaviour and to speak openly about loss and emotional difficulty. As readers will see, he has developed a unique method that includes filmed, televised interactional interviews with various individuals and groups and he then produces art pieces that embody what has transpired through his engagement with their suffering and real-life dilemmas. This highly creative process offers a double or even triple containment for those struggling with all-too-human pain via the holding interview with him directly; the artwork that he creates as a result; and then through the recording on film that is shared with television audiences who are deeply affected by what they witness and who perhaps find a form of healing via identification.     

中学生价值观问卷的编制及其结构验证

基于理论分析和开放式调查的结果,通过系统过程编制了中学生价值观问卷,对来自北京、河北和广西等地971名中学生的价值观测量数据进行了分析。结果发现,中学生价值观由目标价值和手段价值两个基本维度构成,前者包括生理与安全、享乐、物质、自我实现、认知、尊重、归属、情感、抽象感受、社会和公理等11个因素;后者则包括自身能力、性格特点、与人交往的态度和行事方式等4个因素。《中学生价值观问卷》具有良好的信度和效度,且其结构在不同性别、年级样本中具有一致性,可作为中学生价值观的评定工具。     

新冠疫情对安宁疗护工作的影响及应对策略

通过对新冠疫情期间安宁疗护实践经验的总结和国内外相关文献的研究,分析和梳理新冠疫情对临终患者和家属身心健康的影响,及新冠疫情期间安宁疗护团队工作模式发生的转变和团队成员面临的身心压力,并对此背景下安宁疗护实施过程中涉及的伦理问题进行思考,总结安宁疗护团队在疫情期间采取的临床应对策略及在生理支持、心理支持、人文关怀和哀伤辅导等方面发挥的作用,强调了在我国普及安宁疗护的重要性和必要性,为今后安宁疗护的发展提供参考。
