This paper attempts to address the general questionwhether information technologies, as applied in thearea of medicine and health care, have or are likelyto change fundamental concepts regarding disease andhealth. After a short excursion into the domain ofmedical informatics I provide a brief overview of someof the current theories of what a disease is from amore philosophical perspective, i.e. the ``valuefree' and ``value laden' view of disease. Next, Iconsider at some length, whether health careinformatics is currently modifying fundamentalconcepts of disease. To this question I will answerlargely in the negative, and I will provide the sketchof some arguments from current research programs inmedical informatics why I think this is the case. Thisargumentation is supported by a detailed account ofhow the disease profile for beriberi heart disease,used in one of the major medical informaticsdiagnostic programs, QMR (and its ancestorINTERNIST-1), was developed, and why at least thisprogram essentially follows received views oftraditional medicine.The one main exception to theconformity of this program to ``received' views of adisease occurs when the program's designers need tofine-tune a disease definition. This fine-tuning is tocomport with the expert's perspective on the disease,including his or her epistemic values, as well as theprogram's other resources for diagnosing components ofa disease. 相似文献

An attitude against public presentations has been part of the inward looking stance of organized psychoanalysis and has contributed to the often-heard comment that psychoanalysis is a dying profession. Because of the very private nature of clinical psychoanalytic work, this ambivalence to public appearances continues to exist in all psychoanalysts. We have to realize that it is crucial for psychoanalysts to educate the public about psychoanalytic ideas while being aware of possible unintended negative consequences, such as interference in transference issues with patients, ethical and privacy violations, distortion in the press coverage, unfairly biased antipsychoanalytic coverage, and concern about disapproval from prominent and influential members of psychoanalytic organizations about the nature of the press coverage (or even of press coverage at all) leading to criticism of an individual psychoanalyst and interference with his or her progression. Our public information efforts have to include using misfired efforts as teaching tools and insure that presentation of clinical material is limited and sufficiently disguised. 相似文献

Standard theories suggest that humans should seek information only when it can help them make better decisions. However, recent work suggests that people choose to seek information even when it cannot influence the outcome of a choice. Across three experiments, we examined how this preference for non-instrumental information was related to the risk, regret, and rejoice associated with different choices. Experiment 1 examined how risk preference informed the appetite for non-instrumental information and tested how risk and information preference in a gamble-task related to the desire for knowledge across a range of hypothetical real-world scenarios. In Experiment 2, we tested how risk, operationalized as variance, related to non-instrumental information seeking when allowing participants to mentally simulate the potential outcomes of gambles. In Experiment 3, we provided explicit feedback about forgone options, intending to make the potential for regret or rejoice more salient. Taken together, our results show a consistent appetite for information that was robust to changes across all experimental manipulations. We found some evidence of a positive correlation between the desire for knowledge and the level of anticipated regret (Experiment 1), but overall, our data appear more consistent with the idea that non-instrumental information seeking is driven by a general aversion to uncertainty than by an attempt to regulate specific future emotions. 相似文献

College students' food situations may have been disrupted by the COVID-19 pandemic, which could have affected their health and well-being. However, little is known about how the pandemic impacted students' food experiences and how students coped with food-related disruptions, particularly from the student's own perspective. This study reviews data from 571 undergraduates who attended public colleges during Fall 2021 and Spring 2022 and wrote a narrative about the pandemic's impact on their food experiences. Most students indicated they experienced a wide range of food difficulties and adapted new food practices. Despite these challenges, students emphasized proactive coping strategies and positive pandemic outcomes rather than difficulties and negative outcomes. These findings have implications for students' long-term health and well-being. 相似文献

Some nonparametric dimensionality assessment procedures, such as DIMTEST and DETECT, use nonparametric estimates of item pair conditional covariances given an appropriately chosen subtest score as their basic building blocks. Such conditional covariances given some subtest score can be regarded as an approximation to the conditional covariances given an appropriately chosen unidimensional latent composite, where the composite is oriented in the multidimensional test space direction in which the subtest score measures best. In this paper, the structure and properties of such item pair conditional covariances given a unidimensional latent composite are thoroughly investigated, assuming a semiparametric IRT modeling framework called a generalized compensatory model. It is shown that such conditional covariances are highly informative about the multidimensionality structure of a test. The theory developed here is very useful in establishing properties of dimensionality assessment procedures, current and yet to be developed, that are based upon estimating such conditional covariances.In particular, the new theory is used to justify the DIMTEST procedure. Because of the importance of conditional covariance estimation, a new bias reducing approach is presented. A byproduct of likely independent importance beyond the study of conditional covariances is a rigorous score information based definition of an item's and a score's direction of best measurement in the multidimensional test space.This paper is based on a chapter of the first author's doctoral dissertation, written at the University of Illinois and supervised by the second author. The information criterion AIC was introduced to extend the method of maximum likelihood to the multimodel situation. It was obtained by relating the successful experience of the order determination of an autoregressive model to the determination of the number of factors in the maximum likelihood factor analysis. The use of the AIC criterion in the factor analysis is particularly interesting when it is viewed as the choice of a Bayesian model. This observation shows that the area of application of AIC can be much wider than the conventional i.i.d. type models on which the original derivation of the criterion was based. Information available for perception has been shown to be specific, to have validity extending over time, and to be meaningful. Thus, cognition cannot be distinguished as that which exclusively provides adaptive coherence of action and reliance on constraints. By reference to nominated attributes, a genus, being a population of objects of one specified kind, may be partitioned into species, being subpopulations of different kinds. A prototype is an object representative of its species within the genus. Using this framework, the paper describes how objects can be relatively differentiated with respect to attributes, and how attributes can be relatively differentiating with respect to objects. Methods and rationale for such differential ordering of objects and attributes are presented by example, formal development, and application.For a genus comprisingn species of object there is a subsetP ofn distinct prototypes. With respect tom nominated attributes, each object in has anm-element characterization. Together these determine ann ×m objects × attributes matrix, the rows of which are the characterizations of the prototypical objects. Over then species in , an associated relative frequency vector gives the distribution of objects (and of their characterizations). The matrix and vector associate the objects in with points in a metric space (P, ); and it is with respect to various sums of distances in this attribute space that one can differentially order objects and attributes.The definition of the distance function is generalized across kinds of difference, types of characterization, scale-types of measurement, Minkowskiindex 1, and any form of distribution of objects over species. Explanatory and taxonomic applications in psychology and other fields are discussed, with focus on classification, identification, recognition, and search. The Braille code and the identification of its characters provide illustration.An anticipation and some preliminary indications were given in Sutcliffe and Bristow (1966), Sutcliffe (1972, 1973), and Nowakowska (1975). Subsequently the ideas have been extended and their applications realized in a computer program which, in its development, has so far gone through many minor and three major revisions to its present form in Sutcliffe (1985).
