Three single nucleotide polymorphisms (SNPs) in alcohol-metabolizing genes – ADH1B (Arg47His), ADH1C (Ile350Val) and ALDH2 (Glu504Lys) have been extensively associated with flush reaction and alcoholism. Therefore, we investigated the association of these three SNPs with alcohol-induced reactions (AIRs), alcoholism risk, personality and anthropometric traits among Malaysian university students. AIRs, Self-Rating of the Effects of Alcohol (SRE) and Ten-Item Personality were surveyed, anthropometric measurements and DNA samples were taken. Among 264 valid drinkers (111 males, 153 females; 229 ethnic Chinese, 35 ethnic Indians), the minor allele frequencies for ADH1B, ADH1C, ALDH2 among Chinese/Indians were .45/.07, .33/.40, .32/.41, respectively; distribution of ADH1B alleles significantly different between ethnicities. Current/former experiences of flushing, hives, heart palpitations after alcohol consumption; and sleepiness, headache reactions, early and overall SRE were significantly different between ethnicities and genders, respectively. Overall SRE score was associated with ADH1C and ALDH2 alleles. ‘Openness to Experiences’ was associated with ALDH2 genotypes and alleles; Glu/Glu or Glu carriers showed significantly higher means. ADH1B Arg/Arg and Arg carriers showed significantly higher total body and subcutaneous fats but association was abolished after controlling for ethnicity. In conclusion, gender and ethnicity, but not alcohol-metabolizing gene variants, play a role in influencing the manifestation of AIRs. 相似文献

Men are less likely to seek medical help than women and are more likely to adopt unhealthy practices. This study investigated men’s constructions of alcohol and tobacco cessation interventions in relation to dominant masculine identities. Focus groups and interviews with 12 male university students were analysed using an eclectic approach informed by discursive psychology and Foucauldian discourse analysis. Findings suggested that interventions encouraging competition among friends were constructed as favourable, and autonomy and control were central to men’s accounts. While men presented their behaviour change as intentional, their accounts revealed a tendency to conceal this from others, suggesting a negative influence of peer pressure. However, participants who had raised money for charity whilst abstaining described this process as rewarding and acting as a “buffer” to legitimise their healthy behaviour when socializing with other men. Youth living with HIV (YLH) experience multiple disease‐related stresses along with the same structural and developmental challenges faced by their uninfected peers; alcohol use among YLH represents a risk behavior by virtue of potential effects on youth health and increased likelihood of engaging in unprotected sex while drinking alcohol. Research aimed at better understanding the interplay of individual‐ and neighborhood‐level influences on alcohol use for YLH is needed to inform interventions. This study examined whether socioeconomic disadvantage (SED) and social support influence, independently and through interaction, alcohol use in YLH. Data from the Adolescent Medicine Trials Network for HIV/AIDS Interventions (ATN) consisted of YLH across 538 neighborhoods in the United States who acquired HIV behaviorally. Neighborhood‐specific data were compiled from the 2010 U.S. Census Bureau and matched with individual‐level data from the ATN (N = 1,357) to examine effects that contribute to variation in frequency of alcohol use. Other drug use, being male, being non‐Black, and older age were associated with greater alcohol use. Higher social support was negatively associated with alcohol use frequency. A cross‐level interaction indicated that the association found between decreasing social support and increasing alcohol use frequency was weakened in areas with lower SED. This study explores three alcohol-related databases so as to provide a comprehensive understanding of drinking patterns and the prevalence of alcohol-impaired driving in Catalonia (Spain). The rate of alcohol-impaired drivers is compared with the percentage of drinkers in this population, with a particular focus on heavy episodic drinkers. Two strategies adopted by law enforcement agents when conducting alcohol breath tests are examined: (i) non-random and (ii) random approaches to roadblock location and driver selection. We find that heavy drinker profiles (in terms of age and gender) closely match those of alcohol-impaired drivers detected at strategically located, non-random sobriety checkpoints (especially in the case of female drivers), and that they also correlate with the age-gender distribution of drivers involved in road accidents with victims. Different drink driving patterns are detected when sobriety checkpoints are located randomly and drivers are tested at random. Older drivers are identified as a risk group as they abandon the safer driving habits in relation to alcohol shown when they were middle-aged. A combination of non-random and random controls would increase driver perception of their chances of being detected when drink driving. As such, the whole population, regardless of their drinking profile, would be alerted to the serious personal, social and legal implications of alcohol-impaired driving. 相似文献

Sociometric Status and Social Drinking: Observations of Modelling and Persuasion in Young Adult Peer Groups 总被引:1,自引:0,他引:1
Because young adult drinking occurs primarily in peer groups, this should be taken into account when studying influences on drinking behaviour. This paper aimed to assess influences on drinking by observing existing peer groups in a naturalistic setting. We first analysed the basic levels at which two types of influence take place. The first, modelling (imitating others' drinking), was found to significantly influence individual drinking, whereas for the second one, persuasion (drinking resulting from others offering drinks), no predictions were found. Subsequently, we examined whether peer group members' sociometric status in the group affected the amount of influence and persuasion exerted and received. No indications were found that sociometric status had an impact on influence in alcohol consumption within a drinking situation. Features and weaknesses of the study are discussed. 相似文献

This paper takes a phenomenological hermeneutic orientation to explicate and explore the notion of the grey zone of health
and illness and seeks to develop the concept through an examination of the case of alcohol consumption. The grey zone is an
interpretive area referring to the irremediable zone of ambiguity that haunts even the most apparently resolute discourse.
This idea points to an ontological indeterminacy, in the face of which decisions have to be made with regard to the health
of a person (e.g., an alcoholic), a system (e.g., the health system), or a society. The fundamental character of this notion
will be developed in relation to the discourse on health and the limitations of different disciplinary practices. The case
of alcohol consumption will be used to tease out the grey zone embedded in the different kinds of knowledge made available
through the disciplinary traditions of medical science, with its emphasis on somatic well-being, and anthropology, with its
focus on communal well-being. This tension or grey zone embedded in different knowledge outcomes will be shown to have a discursive
parallel with the dialogue between the Athenian, the Spartan, and the Cretan in Plato’s Laws. Making use of the dialogical approach as described by Gadamer, the Athenian’s particular resolution of the tension will be
explored as a case study to demonstrate the necessarily particular analysis involved in a grey zone resolution.

The goal of typology research is to identify subtypes of alcohol dependent (AD) patients sharing fundamental characteristics
and try to match each subtype, with the most precise treatment strategy. This review provides a comprehensive history of the
literature on alcohol dependent subtypes starting from the earliest attempt made by Jellinek. The binary models identified
most closely with Cloninger and Babor as well as the successively more complex classifications are discussed. Typology classification
potentially useful in guiding the treatment of AD patients, especially in the case of the serotonergic medications. Contrasting
data suggests that other factors could influence the response to a medication and/or that more complex typologies should be
identified. In summary, typology models may assist in the ascertainment criteria for clinical trials performed in behavioral
and pharmacotherapeutic interventions. Greater emphasis, however, must be made to more clearly delineate this field of research,
while moving toward more standardized typologies. 相似文献

In old and even middle age, there are associations between physical health and both intelligence and education. This may occur because intelligence and/or education exert effects on lifestyle choices that, in turn, affect later health. Substance use is one aspect of lifestyle choice in young adulthood that could play such a role. The effects of intelligence and/or education on substance use could be direct and environmental, or indirect due to the presence of confounding genetic and shared family influences. We used the Minnesota Twin Family Study to distinguish these effects in males and females at age 24. In contrast to prevailing expectations, there were moderately negative direct nonshared environmental effects of both IQ and education on both smoking and drinking in both males and females. That is, controlling for family background effects in the form of both genetic and shared environmental influences, both higher IQ and greater education were associated with greater alcohol and nicotine use. These effects were accounted for by alcohol and nicotine use at age 17. Our results suggest that genetic and family-culture variables confound the associations between intelligence and education and substance use in young adults, rendering them indirect. Further research is needed to understand the roles of IQ and education in alcohol and nicotine use and their relative impacts on physical health throughout the lifespan. 相似文献

This study investigates the association between school context and adolescent alcohol misuse. Data are from the first two waves of the National Longitudinal Study of Adolescent Health (N = 10,574 adolescents nested within 128 schools). Multilevel multinomial logistic regression is used to assess the association between school-level characteristics and the risk for non-, moderate, and heavy drinking. The risk for adolescent alcohol misuse varies significantly across schools after adjusting for adolescent-level predictors. Several school-level characteristics predict alcohol misuse. Notably, the risk for heavy drinking is elevated in schools located in communities that are socioeconomically advantaged, have high proportions of Non-Hispanic White residents, and are located in suburban (versus urban) areas. High aggregate levels of intoxication in schools increases the risk for heavy drinking among individual adolescents. Results suggest that the influence of social contexts on health is not uniform and that adolescent drinking is more likely in communities that are conceptualized as advantaged. 相似文献
