AbstractEthically challenging situations routinely arise in the course of illness and healthcare. However, very few studies have surveyed patients and family members about their experiences with ethically challenging situations. To address this gap in the literature, we surveyed patients and family members at three hospitals. We conducted a content analysis of their responses to open-ended questions about their most memorable experience with an ethical concern for them or their family member. Participants (N?=?196) described 219 unique ethical experiences that spanned many of the prevailing themes of bioethics, including the patient-physician relationship, end-of-life care, decision-making capacity, healthcare costs, and genetic testing. Participants focused on relational issues in the course of experiencing illness and receiving medical care and concerns regarding the patient-physician encounters. Many concerns arose outside of a healthcare setting. These data indicate areas for improvement for healthcare providers but some concerns may be better addressed outside of the traditional healthcare setting. 相似文献

A growing body of evidence from functional neuroimaging and computational modeling studies indicates that the anterior cingulate cortex (ACC) detects the presence of response conflict and conveys this information to other brain regions, enabling subsequent adjustments in cognitive control. The present study examined previous empirical findings of increased ACC for low-frequency stimuli across three distinct speeded response tasks (two-alternative forced choice, go/no-go, and oddball). Simulations conducted in a neural network model incorporating sequential priming mechanisms (developed in Cho et al., 2002) confirmed that a computational measure of response conflict was higher on low-frequency trials across all three tasks. In addition, the model captured detailed aspects of behavioral reaction time and accuracy data, predicted the dynamics of ACC activity related to trial sequence effects, and provided evidence for the functional role of conflict information in performance monitoring and optimization. The results indicate that the conflict-monitoring hypothesis, augmented by mechanisms for encoding stimulus history, can explain key phenomena associated with performance in sequential speeded response tasks. 相似文献

Rabbits with the electrolytic lesions of bilateral entorhinal cortex (EC) were trained with the hippocampal-dependent trace conditioning of the nictitating membrane response. The multiple-unit activity of the hippocampal CA1 region was recorded during conditioning. The conditioned stimulus was a tone (1 kHz, 85 dB, 200-ms duration), the unconditioned stimulus was a corneal air puff (3 psi, 150-ms duration), and the interstimulus interval was 750 ms. The EC-lesioned animals showed only 30% conditioned response (CR) by the ninth session while the sham-operated animals showed above 80% CR. The lesioned animals did not show learning-related changes in the hippocampal activity. When the training was switched to the 300-ms interstimulus interval trace conditioning, both groups learned above 80% CR. The EC-lesioned animals, however, showed less learning-related activity in the hippocampus than the sham-operated animals. These results suggest that the development of the learning-related activity in the hippocampus depends on the intact EC, and that the EC may provide a possible pathway conveying learning information from the cerebellum or cerebral cortex to the hippocampus during the trace conditioning. 相似文献

This paper aims to deal with the change of sexual discourse through the rise of a feminist movement in Korea from a constructivist point of view. First, the paper discusses the Confucianism of the Chosun dynasty as an historical background of the issue of sexuality (since Confucianism still has a far-reaching grip and effect on many aspects of everyday life in Korea). Second, it deals with chastity ideology and the double standard of sexuality between men and women as ongoing Confucian sexual discourses. Third, it focuses on three themes: (1) the change from sex for procreation to sex for pleasure, (2) the change from genitally-oriented sexuality to intimacy- or relationship-oriented sexuality, and (3) the change from a woman as a sexual object to a sexual subject as part of the changes of sexual discourses. Fourth, it tries to show clashes and/or alliances between the Confucian and feminist discourses on sexuality, which make up the process of the social construction of sexuality. This shows that sexuality is a socio-historical construction in Korea as elsewhere. 相似文献

We used a national database (Educational Longitudinal Study) to investigate the effects of parent’s gender, child’s gender,
and parental involvement in school on the academic achievement of adolescents in single-parent families. A three way 2 × 2 × 2
(parent’s gender × child’s gender × parental involvement) MANCOVA was conducted with four student academic achievement indicators
as dependent variables and SES as a covariate. The results indicated that parent gender and child gender interact with parent
involvement to affect adolescents’ academic achievement differentially. Specifically, daughters who lived with highly involved
single-fathers performed better academically than the other groups did. These findings suggest that researchers who study
single-parents’ involvement in their adolescents’ academic achievement need to pay more attention to gender-specific effects. 相似文献

Ecological view of human development calls for an investigation of multiple contexts surrounding children. In South Korea, traditional Confucianism and recent technological advancements serve as influential social and cultural contexts that affect parent–child relations and child development. High levels of parental control and strong emphasis on academic achievement have long been distinctive features of Korean parenting practices, which are attributable to Confucian values. Additionally, Korean children’s mobile phone dependency (MPD) has become a growing concern, as South Korea developed as an IT powerhouse. Combined, these contexts resulted in high parental control of children’s mobile phone use, so that such electronic devices would not hinder learning and achievement. Effects of high parental control on children’s MPD and their school life, however, have yet to be discovered. We hypothesized, that, unlike parents’ intentions, psychological control is more likely to increase MPD and disrupt school life. To examine this research model, we made use of the first and third wave data from Korea Children and Youth Panel Survey. Participants were 2378 children (52.2 % boys) of the same age of 10 in the first wave. After multiple imputation for missing values, hierarchical logistic regression followed to examine the mediational model. The results verified the hypothesized model, showing significant adverse influence of psychological control on MPD, self-regulated learning and school adjustment. MPD fully mediated the effect of psychological control on self-regulations, while partially mediating the effects on school adjustment. Implications for Korean parents with regard to supporting children’s psychological autonomy was discussed. 相似文献

COVID‐19 is changing everyday life. COVID‐19 is also changing the look of the church. The church is a community of people who gather for worship, fellowship, and sharing. However, due to the coronavirus, the church is no longer able to gather and worship together. Moreover, because of the coronavirus, social distancing with as little as possible face‐to‐face contact has been recommended worldwide. If this situation is prolonged, the church community interactions will have difficulty in surviving. Multidimensional item response theory (MIRT) is widely used in assessment and evaluation of educational and psychological tests. It models the individual response patterns by specifying a functional relationship between individuals' multiple latent traits and their responses to test items. One major challenge in parameter estimation in MIRT is that the likelihood involves intractable multidimensional integrals due to the latent variable structure. Various methods have been proposed that involve either direct numerical approximations to the integrals or Monte Carlo simulations. However, these methods are known to be computationally demanding in high dimensions and rely on sampling data points from a posterior distribution. We propose a new Gaussian variational expectation--maximization (GVEM) algorithm which adopts variational inference to approximate the intractable marginal likelihood by a computationally feasible lower bound. In addition, the proposed algorithm can be applied to assess the dimensionality of the latent traits in an exploratory analysis. Simulation studies are conducted to demonstrate the computational efficiency and estimation precision of the new GVEM algorithm compared to the popular alternative Metropolis–Hastings Robbins–Monro algorithm.
