Interdisciplinary guidelines allowed for new quality standards in pain appraisal. Simultaneously, the scientific notion of fibromyalgia changed from a subjectively overvalued malaise to a central disturbance of coping with stress, which is neurobiologically determined. Therefore, in this article a psychosomatics-based guide is given for the socio-medical appraisal of fibromyalgia with reference to the new International Classification for Functioning and Disability (ICF) of the World Health Organization (WHO). Hereditary breast cancer (HBC) accounts for 5–10% of breast cancer cases and it significantly increases the lifetime risk of cancer. Our objective was to evaluate the sociodemographic variables, family history of cancer, breast cancer (BC) screening practices and the risk profile of cancer affected or asymptomatic at-risk women that undergo genetic counseling for hereditary breast cancer in public Brazilian cancer genetics services. Estimated lifetime risk of BC was calculated for asymptomatic women using the Gail and Claus models. The majority of women showed a moderate lifetime risk of developing BC, with an average risk of 19.7% and 19.9% by the Gail and Claus models, respectively. The average prior probability of carrying a BRCA1/2 gene mutation was 16.7% and overall only 32% fulfilled criteria for a hereditary breast cancer syndrome as assessed by family history. We conclude that a significant number of individuals at high-risk for HBC syndromes may not have access to the benefits of cancer genetic counseling in these centers. Contributing factors may include insufficient training of healthcare professionals, disinformation of cancer patients; difficult access to genetic testing and/or resistance in seeking such services. The identification and understanding of these barriers is essential to develop specific strategies to effectively achieve cancer risk reduction in this and other countries were clinical cancer genetics is not yet fully established.  相似文献

Following our hypothesis that juvenile emotional and/or cognitive experience should affect learning performance at preweaning age as well as adulthood, the present study in female Wistar rats aimed to examine the impact of (i) avoidance training at preweaning age, (ii) exposure to repeated maternal separation, (iii) the combination of both, and (iv) the blockade of dopaminergic neurotransmission on adult two-way active avoidance learning in rats. We found that preweaning, i.e. three week old, rats were less capable of avoidance learning compared to adults. Our main findings revealed that preweaning avoidance training alone improved avoidance learning in adulthood. Furthermore, maternal separation alone also improved avoidance learning in preweaning and in adult rats, but this effect of maternal separation did not add up to the beneficial effect of preweaning avoidance training on adult learning. In addition, the pharmacological blockade of dopamine receptors during preweaning avoidance training via systemic application of haloperidol impaired preweaning avoidance performance in a dose-dependent manner. Testing the haloperidol-treated preweaning presumed "non-learners" as adults revealed that they still showed improved learning as adults. Taken together, our results strongly support the hypothesis that emotional as well as cognitive experience at preweaning age leaves an enduring "memory trace," which can facilitate learning in adulthood. Our pharmaco-behavioral studies suggest that unlike the adult brain, preweaning learning and memory formation is less dependent on dopaminergic mechanisms, which raises the intriguing question of possible alternative pathways.  相似文献

A direct relationship between perception and action implies bi-directionality, and predicts not only effects of perception on action but also effects of action on perception. Modern theories of social cognition have intensively examined the relation from perception to action and propose that mirroring the observed actions of others underlies action understanding. Here, we suggest that this view is incomplete, as it neglects the perspective of the actor. We will review empirical evidence showing the effects of self-generated action on perceptual judgments. We propose that producing action might prime perception in a way that observers are selectively sensitive to related or similar actions of conspecifics. Therefore, perceptual resonance, not motor resonance, might be decisive for grounding sympathy and empathy and, thus, successful social interactions.  相似文献

PALME     

Background

Single mothers and their children are exposed to increased psychosocial burdens and risks. Nevertheless, in Germany offers of help especially designed for the needs of this group are lacking.

Method

An emotion-oriented parental training program (PALME, http://www.palme-elterntraining.de) based on the attachment theory was developed particularly for single mothers with preschool children. The training is guided by trained kindergarten nurses. The effectiveness of the parental training program on the measure of maternal psychological impairment, emotional competence and child problem behavior was investigated within a randomized, controlled study with 61 single mothers suffering from medium grade psychosocial impairment.

Results

Analyses of variance revealed significant group×time interaction effects in favor of the intervention group compared to controls. After the training the mothers showed improvement in their psychological impairment, depression and emotional competence. In addition, evidence suggests that behavioral problems of the children were reduced.

Conclusion

In view of the benefits of this parental training and its low costs it should be recommended as a regular offer of support for single mothers within community settings.  相似文献

We compare Karl Popper’s ideas concerning the falsifiability of a theory with similar notions from the part of statistical learning theory known as VC-theory. Popper’s notion of the dimension of a theory is contrasted with the apparently very similar VC-dimension. Having located some divergences, we discuss how best to view Popper’s work from the perspective of statistical learning theory, either as a precursor or as aiming to capture a different learning activity.  相似文献

A core tenet of the original conflict-monitoring model is that regulation is triggered automatically when conflict is present and that the same regulation mechanism explains both trial-to-trial adaptation effects as well as effects of block-wise conflict manipulations. We present here results from two experiments using the Stroop task that show (a) that adaptation effects in the absence of response repetitions may occur only at the beginning of testing and that (b) robust block-wise effects can be found even in the absence of trial-by-trial effects. Furthermore, we show that the failure to eliminate target-to-distractor repetitions can produce artificial trial-to-trial adaptation effects. Based on the evidence of a weak link between conflict and conflict adaptation, we argue that a wider range of possible reasons for conflict adaptation effects needs to be taken into consideration.  相似文献

Within the context of the psychological refractory period (PRP) paradigm, we developed a general theoretical framework for deciding when it is more efficient to process two tasks in serial and when it is more efficient to process them in parallel. This analysis suggests that a serial mode is more efficient than a parallel mode under a wide variety of conditions and thereby suggests that ubiquitous evidence of serial processing in PRP tasks could result from performance optimization rather than from a structural bottleneck. The analysis further suggests that the experimenter-selected distribution of stimulus onset asynchronies (SOAs) influences the relative efficiency of the serial and parallel modes, with a preponderance of short SOAs favoring a parallel mode. Experiments varying the distribution of SOAs were conducted, and the results suggest that there is a shift from a more serial mode to a more parallel mode as the likelihood of short SOAs increases.  相似文献

Several studies have demonstrated that acquired expertise influences aesthetic judgments. In this paradigm we used functional magnetic resonance imaging (fMRI) to study aesthetic judgments of visually presented architectural stimuli and control-stimuli (faces) for a group of architects and a group of non-architects. This design allowed us to test whether level of expertise modulates neural activity in brain areas associated with either perceptual processing, memory, or reward processing. We show that experts and non-experts recruit bilateral medial orbitofrontal cortex (OFC) and subcallosal cingulate gyrus differentially during aesthetic judgment, even in the absence of behavioural aesthetic rating differences between experts and non-experts. By contrast, activity in nucleus accumbens (NAcc) exhibits a differential response profile compared to OFC and subcallosal cingulate gyrus, suggesting a dissociable role between these regions in the reward processing of expertise. Finally, categorical responses (irrespective of aesthetic ratings) resulted in expertise effects in memory-related areas such as hippocampus and precuneus. These results highlight the fact that expertise not only modulates cognitive processing, but also modulates the response in reward related brain areas.  相似文献
