This qualitative study asked Australian Genetic Counselors and Clinical Geneticists working in cancer genetics to describe their practice when a woman attends a consultation about her family history and her risk of developing breast cancer. Twenty-nine out of 36 Clinical Geneticists/Genetic Counselors returned the questionnaire (82%). Participants identified the key goals of the consultation as (a) identifying the individual needs and concerns of the woman, (b) providing information on genes and chromosomes, (c) giving an individual risk assessment in the context of supportive interaction, and (d) discussing the pros and cons of genetic testing and putting a surveillance plan into place. Respondents emphasized the dual importance of counseling/support and information provision in this setting, suggesting that one could not be given without the other. Implications for clinical practice are discussed. 相似文献
A general least squares solution for successive intervals is presented, along with iterative procedures for obtaining stimulus scale values, discriminal dispersions, and category boundaries. Because provisions for weighting were incorporated into the derivation, the solution may be applied without loss of rigor to the typical experimental matrix of incomplete data, i.e., to a data matrix with missing entries, as well as to the rarely occurring matrix of complete data. The use of weights also permits adjustments for variations in the reliability of estimates obtained from the data. The computational steps involved in the solution are enumerated, the amount of labor required comparing favorably with other procedures. Recent studies have argued that Oppositional Defiant Disorder (ODD) problems continue into emerging adulthood; however, few studies have examined ODD problems in this population. Moreover, previous studies have found that corporal punishment mediated the relationship between maternal anxiety/depression and child ODD problems in young children and that parental psychopathology is likely to affect child ODD. This study examined how maternal as well as paternal maltreatment (i.e., psychological and physical) mediated the relationship between parental anxiety/depressive problems and emerging adult ODD problems (i.e., irritability and defiance). Furthermore, child and parent gender were examined as moderators (i.e., moderated mediation). Participants included 1,012 emerging adults who completed questionnaires about parental psychological and physical maltreatment, parental anxiety and depression, and affective and behavioral ODD symptoms. Results suggested that mediation occurred for the father–daughter dyad along the perceived paternal depressive problems → psychological and physical maltreatment → irritability paths and for the mother–son dyad along the perceived maternal depressive and anxiety problems → psychological maltreatment → defiance paths. Given that mediation occurred for only these gender dyads, moderated mediation was suggested. 相似文献
Supporters and opponents of affirmative action are often characterized as debating about a single, consensually understood type of affirmative action. However, supporters and opponents instead may have different types of policies in mind when thinking about affirmative action and may actually agree on specific manifestations of affirmative action policies more than is commonly believed. A survey conducted using a student sample and a sample from the broader Chicago-area community showed that affirmative action policies can be characterized into merit-violating versus merit-upholding manifestations. Supporters of affirmative action in general were more likely to think of affirmative action in its merit-upholding manifestations, whereas opponents were more likely to think of the merit-violating manifestations. However, both supporters and opponents showed more support for merit-upholding rather than merit-violating manifestations of affirmative action. The same pattern of results was upheld even when splitting the samples into those who endorsed negative racial attitudes versus those who did not, suggesting that even those who may be considered racist will endorse affirmative action policies that uphold merit values. The results are discussed in terms of the importance of clarifying the political discourse about what affirmative action is and what it is designed to do. 相似文献
Sexual minorities experience higher rates of several physical health problems compared to their heterosexual counterparts. The present study uses Meyer's Minority Stress Model (Psychological Bulletin, 129(5): 674-697, 2003) to examine physical health indicators among 250 adults who identified as sexual minorities. Study hypotheses include that sexual minority stress is predictive of two physical health indicators (i.e., engagement in a health-promoting lifestyle and number of physical health problems) and that planning (i.e., problem-focused) and social support coping will partially mediate the relationship between sexual minority stress and each physical health indicator. Results showed that as level of sexual minority stress increased, engagement in a health-promoting lifestyle decreased and the number of physical health problems increased. Planning and social support coping did not mediate these relationships; however, as levels of coping increased, engagement in a health-promoting lifestyle increased. These findings have implications for researchers and healthcare professionals in their efforts to promote the physical health of sexual minorities. 相似文献

Previous research has shown a strong connection between parental religiosity and parenting behaviors in the transmission of religion from parent to child. Our study expanded upon previous research by examining how perceived maternal and paternal warmth and overprotection mediated the relation between parental and personal religiosity in males and females. A sample of 486 emerging adults reported maternal, paternal, and personal religiosity as well as their parents’ parenting behaviors. Results indicated that perceived paternal warmth served as a mediator for males only but neither perceived maternal nor paternal overprotection mediated the relationship between parental religiosity and emerging adult religiosity. Gender moderated the paths from perceived paternal religiosity to paternal overprotection, such that the relationship was stronger for females, and perceived paternal warmth to emerging adult religiosity, such that the relationship was stronger for males. Overall, results demonstrated that mediation effects occurred for males only, suggesting moderated mediation.
Telehealth is increasingly used for outreach service in cancer genetic counseling; however what occurs during the consultation
and the roles practitioners adopt is largely unknown. Fifteen practitioners participated in semi-structured interviews that
explored their roles within telehealth, compared to face-to-face consultations, and the relationship between practitioners
during telehealth. As they were not physically present with the patient, most participants felt that telehealth altered the
genetic clinician’s role to one of a ‘visiting specialist’. Genetic counselors described undertaking multiple roles during
the telehealth process. Two models of interaction were observed. The medical model reduced the interaction to a dyadic consultation
by having the genetic counselor off-screen and included minimal clinician meetings and supervision. The triadic co-facilitation
model incorporated a high level of information exchange, counselor autonomy and included the counselor onscreen. The co-facilitation
model offers a useful framework for telehealth genetic counselling, offering complementary roles between practitioners and
