Intensive Treatment of Specific Phobias in Children and Adolescents   总被引:1,自引:1,他引:0  
One-session treatment (OST), a variant of cognitive-behavioral therapy, combines graduated in vivo exposure, participant modeling, reinforcement, psychoeducation, cognitive challenges, and skills training in an intensive treatment model. Treatment is maximized to one 3-hour session. In this paper, we review the application of OST for specific phobia in youth and highlight practical matters related to OST and its use in a clinical setting. We also briefly review results of treatment outcome studies and suggest future directions for clinical research and practice. We conclude that OST is an efficient and efficacious treatment.  相似文献

Facilities which provide services to the elderly offer a wide variety of activities to engage their clients. In many cases, however, clients do not participate in these activities, or they do not participate in ways which are likely to provide them with benefits. This study sought to increase meaningful participation in an exercise class by four elderly clients attending an adult day care program; in these four cases, the quality of their participation was seriously inadequate. Participants were praised when they participated correctly in an exercise, verbally prompted if they did not participate, and physically assisted to participate if they did not participate following verbal prompting. To show experimental control over exercise behaviors, a single subject multiple baseline design was used. The results suggest that clients attending facilities such as an adult day care program may be easily recruited to participate correctly in exercise sessions through the use of prompts and social reinforcers. The purpose of the current research was to investigate adolescent offenders' perspectives about responses to interpersonal aggressive encounters. Specifically, participants' perspectives were assessed regarding the role of a bystander when either a friend or an acquaintance of the bystander was the victim of an aggressive act. Two aggressive acts were presented. First, the bystander witnessed an acquaintance stealing from the victim. Second, the bystander witnessed an acquaintance hitting the victim. Participants were asked to indicate (a) if the bystander would do anything (bystander expected behavior), (b) what the bystander would do (expected behavioral action), (c) if the action would be the right thing to do (evaluation of bystander expected behavioral action), and (d) what the bystander should do in response to the violation (prescribed bystander behavioral action). Results indicate that the adolescent offenders' perspectives varied as a function of offender status, type of aggressive act, as well as relationship of the victim to the bystander. Aggr. Two types of sexual trauma, sexual abuse in childhood and rape in adulthood, were investigated in terms of possible effects on personality. Four groups of participants were studied: women who had experienced sexual abuse in childhood, women who had experienced rape as adults, women who had experienced both of these sexual traumas, and a control group of women who had experienced no sexual trauma. Personality functioning was assessed using the Dimensional Assessment of Personality Pathology. Groups who had experienced childhood sexual abuse displayed the highest degree of personality disturbance; however, the additive effects of repeated sexual trauma were limited. These findings may reflect the outcome of specific adversity in childhood on the psychobiological constructs underlying personality.  相似文献

We used two reaction time tasks to examine age differences in the ability to use an endogenous cue to shift attention covertly and to ignore distractors. In Experiment 1, 8‐year‐olds, 10‐year‐olds and adults (n = 24 per age) were asked to push a button as soon as they detected a target that was presented in a cued, miscued or non‐cued peripheral location at 100, 400 or 800 ms after the appearance of a central cue. In Experiment 2, 10‐year‐olds and adults (n = 24 per age) were asked to indicate which of two shapes appeared in the periphery 400 ms after a central cue, with those shapes surrounded by compatible or incompatible distractors. Unlike previous studies, the data were corrected for a reaction time bias that can inflate the apparent effect of cueing. Children were slower and more variable than adults overall. However, there were no age differences in the effects of the cues in either experiment: at all ages, the speed of responding was increased similarly by correct cueing and slowed similarly by incorrect cueing. Thus, under these conditions, the ability to use endogenous cues to orient covertly to the periphery is already adult‐like by 8–10 years of age, although there may be subsequent changes in the consistency of responding. In Experiment 2, 10‐year‐olds were slowed more than adults by incompatible distractors. Thus, the ability to ignore distracting information is not adult‐like even by 10 years of age. The findings suggest different rates of development for the ability to shift attention following an endogenous cue and for the ability to filter out irrelevant information.  相似文献

This study tested the reliability and validity of a diagnostic thermal vascular test (TVT) for patients with Raynaud's Phenomenon (RP). The TVT examined digital blood pressure responses to combined cooling and occlusion and was developed as part of the Raynaud's Treatment Study, a multicenter clinical trial comparing the efficacy of biofeedback and pharmacological treatment. A computerized system permitted efficient, accurate, and uniform testing at different geographical sites. A comparison of 199 patients with RP and 52 healthy controls is reported. The TVT showed a sensitivity of 79% and a specificity of 88%. Test-retest reliability was acceptable (r = .80). Addition of a psychological challenge failed to improve the discrimination between patients with RP and controls. The TVT separated patients with RP and controls as well as or better than existing tests and did so with enhanced ease of operation.  相似文献

Change in task persistence was assessed in two annual assessments using teachers', testers', and observers' ratings. Participants included 79 monozygotic and 116 same-sex dizygotic twin pairs who were in Kindergarten or 1st grade (4.3 to 7.9 years old) at the initial assessment. Task persistence was widely distributed and higher among older children and girls. Overall, there was modest growth in persistence over time, and moderate stability of individual differences. Most of the stability was accounted for by genetic influences, whereas most of the change was accounted for by nonshared environment, including an association with observed differential maternal warm supportive behavior.  相似文献
