Women with a limited family history of breast cancer may be interested in cancer genetics information although their objective risk of breast cancer may not indicate routine referral to cancer genetics services. This study examined factors related to interest and use of cancer genetics services in a community sample of women with a limited family history of breast cancer (N = 187) who had no previous contact with cancer genetics services. Participants provided demographic information and ratings of perceived risk, cancer distress, attitudes, and intentions to initiate cancer genetics services. Participants were given information about a cancer genetics clinic that served women having concerns about their breast cancer risk. Women were contacted within 6 weeks and 8 months following their study appointment. Six weeks following their study appointment, 25% of women had initiated cancer genetics services. Eight months following their study appointment, 18% of women reported having completed a cancer genetics service appointment. Baseline intentions independently predicted both initiation at 6 weeks and appointment at 8 months. Cancer distress was positively associated with cancer genetics service initiation and appointment. Results suggest that some women with a limited family history of breast cancer are interested in seeking out cancer genetics information. Women with a limited family history of breast cancer may benefit from the availability of cancer genetics information provided through primary healthcare settings.  相似文献

Research examining the efficacy of community-based domestic violence and sexual assault services is yielding promising findings; however, little is known about the structure, components, and content of these services. Although service guidelines exist, a critical gap persists because review, comparison, and synthesis of these recommendations have not been undertaken. This research addresses this knowledge gap through a review of the domestic violence and sexual assault services literature. We gathered relevant literature by conducting systematic searches of databases, and by soliciting the service guidelines and training manuals from all the Domestic Violence and Sexual Assault Coalitions in the United States. These efforts yielded 43 articles, books, or manuals for review. The findings show strong consensus on core services for survivors, including crisis services, legal and medical advocacy, support groups, individual counseling, and shelter. This article presents the review results in tables comparing the service goals, intervention strategies, and recommendations for service delivery. We also found areas of contention (e.g., whether to continuously staff domestic violence shelters, and whether combined agencies can effectively offer both domestic violence and sexual assault services). Building upon the review findings, we discuss the implications for domestic violence and sexual assault services and intervention research.  相似文献

The impact of spouses' communication about economic uncertainty on biological stress responses was examined. The sample included 82 Latino and Caucasian couples and one of their adolescents. Wives with low economic uncertainty experienced the greatest cortisol increase to a stressful discussion with their spouse, while wives with high economic uncertainty experienced a blunted cortisol response. Husbands with high economic uncertainty exhibited a low, unresponsive cortisol pattern. Wives who experienced elevated cortisol used more corumination and had low community support. The amount parents talked about their financial worries to their adolescent was not associated with the adolescent's mental health or biosocial markers, but the parents' economic uncertainty and corumination with each other were negatively associated with their own mental health.  相似文献

Two ways to improve worked examples are investigated: presenting them in a segmented format (i.e., in pieces separated by blank lines) and instructing students to segment them actively (i.e., students must divide the examples in pieces). Segmented examples are expected to support learning because they show which information elements belong together. Learners might also be encouraged to think about which information elements belong together when they are instructed to actively segment, but on the negative side, this is an additional task that might impose extra cognitive load without facilitating learning. Results showed that students in the actively segmenting condition invested more effort in learning than students in the other conditions without performing better. Furthermore, studying segmented examples required less effort to achieve equal learning outcomes than studying nonsegmented ones. So, presenting learners with segmented examples improved the efficiency of the learning process, whereas instructing them to actively segment decreased this efficiency. Whether and how the co-occurrence of depression and diabetes in pregnancy may worsen infant development has not been reported. Pregnant women with diabetes and with (n?=?34) or without (n?=?34) major depressive disorder (MDD) were followed during pregnancy and 6-months postpartum. The MDD subset received randomly assigned treatment with cognitive behavior therapy (CBT) or supportive counseling (SC). Depression severity was measured with the Beck Depression Inventory (BDI); infant developmental outcomes were measured with the Bayley Scales of Infant Development (BSID) and its Behavior Rating Scale (BRS). Infants of women with MDD had lower BRS scores (p?=?.02). Reduction in depression scores was associated with better infant outcomes on the BSID and BRS (p values <.03). These preliminary findings suggest depression occurring in pregnant women with diabetes is associated with poorer infant development and improvement in prepartum depression is associated with improvement in measures of infant development.  相似文献

Abstract

Feelings of uncertainty are a central feature of the disaster experience. Surprisingly, though, there is very little systematic quantitative research about the impact of uncertainty on disaster survivors. Moreover, communal coping has increasingly received attention as a potential buffer of the negative effects of stressors but that literature is also limited in its application to disasters. This investigation applies research in the domain of uncertainty, together with the Theoretical Model of Communal Coping to better understand the experience of a community exposed to three wildfires in a one year period. A random-digit dialing procedure was used to gather data from 402 individuals. Participants completed measures of mental health, uncertainty, and communal coping in the context of their experience with the most personally stressful of the three wildfires. All analyses were examined separately for those who were evacuated and those who were not. Results support the negative impact of uncertainty across both evacuated and nonevacuated sub-samples and show a strong buffering role for communal coping among those who evacuated. The implications of these findings for the understanding of wildfire survivors' experiences are noted and future directions are proposed.  相似文献

Habituation – the most basic form of learning – is used to evaluate central nervous system (CNS) maturation and to detect abnormalities in fetal brain development. In the current study, habituation, stimulus specificity and dishabituation of auditory evoked responses were measured in fetuses and newborns using fetal magnetoencephalography (fMEG). An auditory habituation paradigm consisting of 100 trains of five 500 Hz tones, one 750 Hz tone (dishabituator) and two more 500 Hz tones, respectively, were presented to 41 fetuses (gestational age 30–39 weeks) and 22 newborns or babies (age 6–89 days). A response decrement between the first and fifth tones (habituation), an increment between the fifth tone and the dishabituator (stimulus specificity) and an increment between the fifth (last tone before the dishabituator) and seventh tones (first tone after the dishabituator) (dishabituation) were expected. Fetuses showed weak responses to the first tone. However, a significant response decrement between the second and fifth tones (habituation) and a significant increment between the fifth tone and the dishabituator (stimulus specificity) were found. No significant difference was found for dishabituation nor was a developmental trend found at the group level. From the neonatal data, significant values for stimulus specificity were found. Sensory fatigue or adaptation was ruled out as a reason for the response decrement due to the strong reactions to the dishabituator. Taken together, the current study used fMEG to directly show fetal habituation and provides evidence of fetal learning in the last trimester of pregnancy.  相似文献
