谈谈中国临床试验研究中受试者权益保障问题

随着国家《药品临床试验管理规范》的颁布与实施,我国新药临床试验研究水平有相当的提高,但也存在着一些问题。其中包括保护受试者权益问题。国家GCP有关章节中明确指出要保护受试者权益。并有具体规定,但在执行中尚有出入,如试验方案未经伦理委员会批准,未向受试者告知试验内容。或告知的不够充分,甚至在未签署知情同意书时就开始试验等。为此,我国采取了相应的措施,如设立药品临床研究培训中心;定期对临床试验基地的研究者以及相关人员如与试验有关的医师,药师及申办者进行GCP培训,基地审核制度,以法律手段来保证受试者权益等。     

Religiosity and Attitudes Toward Diversity: A Potential Workplace Conflict?1

The study investigates relationships between 2 measures of religiosity (immanence orientation and religious fundamentalism) and 2 measures of attitudes toward diversity (universal‐diverse orientation and attitudes toward gay men and lesbians). As expected, different conceptualizations of religiosity relate differently to diversity attitudes. Immanence orientation related positively and religious fundamentalism related negatively with both measures of diversity attitudes. However, when both measures were included in the hierarchical regression equation, immanence orientation no longer related significantly with attitudes toward lesbians and gay men; and religious fundamentalism no longer related significantly with universal‐diverse orientation. Finally, gender and immanence interacted to predict universal‐diverse orientation. Implications are discussed.     

拜昂客体关系理论的精神病学思想

拜昂是20世纪后半期最具影响力的精神分析客体关系思想家和实践者之一.在精神病理学方面,他以一种温和的方式在咨询室中引起了一场分析师理解患者的革命.拜昂认为,精神病是先天倾向与后天环境相互作用的结果;人格既包括精神病部分也包括非精神病部分;在治疗中,分析师应该保持一种无记无欲的态度,以达到精神分析的目标.     

Turning it Upside Down: Areas of Preserved Cognitive Function in Schizophrenia

Patients with schizophrenia demonstrate marked impairments on most clinical neuropsychological tests. These findings suggest that patients suffer from a generalized form of cognitive impairment, with little evidence of spared performance documented in several large meta-analytic reviews of the clinical literature. In contrast, we review evidence for relative sparing of aspects of attention, procedural memory, and emotional processing observed in studies that have employed experimental approaches adapted from the cognitive and affective neuroscience literature. These islands of preserved performance suggest that the cognitive deficits in schizophrenia are not as general as they appear to be when assayed with clinical neuropsychological methods. The apparent contradiction in findings across methods may offer important clues about the nature of cognitive impairment in schizophrenia. The documentation of preserved cognitive function in schizophrenia may serve to sharpen hypotheses about the biological mechanisms that are implicated in the illness.     

5·12地震前后灾区大学生生命价值观比较

采用自编大学生生命价值观问卷（LVS）分别三次对灾区大学生进行测试：5·12地震前1年、震后2周、震后5个月。结果表明:（1）自编的大学生生命价值观问卷（LVS）包括存在价值、目标价值、规则价值三个维度,探索性与验证性因素分析证明问卷具有良好的信效度。（2）在LVS总分和其存在价值维度上,震后2周得分均高于震前1年和震后5个月。（3）在存在价值取向维度上,女生震后2周和震后5个月得分显著高于男生;理科生震后2周得分高于震后5个月和震前1年,理科生震后2周得分显著高于文科生。（4）在规则价值取向维度上,文科生震后2周得分显著低于震后5个月,文科生震后2周得分显著低于理科生     

母子关系、师幼关系与学前流动儿童的社会适应行为

母子关系和师幼关系是儿童早期发展中最重要的人际关系, 在儿童社会适应中扮演着重要的角色。研究以北京市271名学前流动儿童和552名学前城市儿童为被试, 采用问卷法考察母子关系、师幼关系与学前流动儿童的社会技能、外显和内隐问题行为之间的关系。结果发现:(1)流动儿童母子关系亲密程度低于城市儿童, 母子关系冲突性、师幼关系亲密性和冲突性与城市儿童没有差异;(2)母子关系对儿童社会适应行为的预测作用在流动儿童和城市儿童上没有显著差异, 母子关系亲密性正向预测社会技能, 负向预测内隐问题行为;母子关系冲突性负向预测社会技能, 正向预测外显和内隐问题行为;师幼关系亲密性正向预测城市和流动儿童的社会技能, 并且显著负向预测流动儿童的内隐问题行为;(3)师幼关系对母子关系的调节作用主要表现在流动儿童而非城市儿童上, 师幼关系低冲突能够减少母子关系冲突性对流动儿童社会技能的负向预测作用, 师幼关系高亲密能够减少母子关系冲突性对流动儿童内隐问题行为的正向预测作用。     

Memory: Between traumatic shock and psychic historicization

‘I want to erase him from my memory,’ she said, ‘his eyes, his eyes staring at me … they are there every night, his hand … every night it hits me again and again …’. On the basis of these few words taken from the therapeutic work with Keltoum, the author reflects theoretically and clinically on the place of memory in a traumatism: a fixed memory, a ‘frozen image’, the signs of a traumatic compulsion to repeat, are the expressions of a process of psychic disorganization but also of a ‘symbolized and symbolizing’ memory anticipating a possible historicization of this memory, of a path, therefore, towards a resolution of the trauma resulting from a process of psychic integration and reorganization.     

激情犯和累惯犯暴力态度的比较

实验采用《FMMU变态人格危险性评估问卷》和内隐联想测验分别测量激情犯和累惯犯的外显和内隐暴力态度, 探讨激情犯和累惯犯外显和内隐暴力态度特点及预测特征。研究发现, 激情犯和累惯犯的外显态度有显著差异, 但是内隐态度未见显著差异。由于部分罪犯在内隐态度中出现反转效应, 将激情犯和累惯犯分为激情犯未反转组和反转组, 累惯犯未反转组和反转组, 这四个组都产生了显著的IAT效应, 并且每个组的外显态度和内隐态度都发生了分离。从结果可见即使是同一犯罪类型, 犯罪时都实施了暴力行为, 但暴力态度却有差异。在测量外显暴力态度的基础上, 结合内隐暴力态度的测量, 将细化犯罪人分类, 为不同特点的犯罪人实施不同的改造和处遇方案提供新的思路。     

关于慢性乙肝病毒携带者治疗问题的商榷

从乙型肝炎病毒(HBV)在体内存在的不同阶段、乙型肝炎发病机制及目前抗病毒治疗现状对慢性乙肝病毒携带者治疗问题进行了初步探讨,提出了绝大多数慢性乙肝病毒携带者不需要治疗的理由,以指导患者和医务人员解决相关问题。
