There are minimal data on the impact of genetic counselors in subspecialty clinics, including the pediatric arrhythmia clinic. This study aimed to describe the clinical encounters of a genetic counselor integrated into a pediatric arrhythmia clinic. In the 20 months between July 2015 and February 2017, a total of 1914 scheduled patients were screened for indications relevant for assessment by a genetic counselor. Of these, the genetic counselor completed 276 patient encounters, seeing 14.4% of all patients in clinic. The most expected and common indications for genetic counselor involvement were related to suspicion for primary heritable arrhythmia conditions, though patients seen in this clinic display a wide range of cardiac problems and many additional indications for genetic evaluation were identified. Roughly 75% (211/276) of encounters were for personal history of confirmed/suspected heritable disease, including cardiac channelopathies, cardiomyopathies, ventricular arrhythmias, and congenital heart defects, and 25% (65/276) were for family history of disease, including long QT syndrome and sudden unexplained death. Overall, this study shows that about 1 in 7 patients seen in a pediatric arrhythmia clinic have indications that likely benefit from genetic counselor involvement and care. Similar service delivery models embedding genetic counselors in pediatric arrhythmia clinics should be encouraged, and this model could be emulated to increase patient access to genetic counseling services.  相似文献

Two experiments examined age differences in the effect of a sad mood induction (MI) on attention to emotional images. Younger and older adults viewed sets of four images while their eye gaze was tracked throughout an 8-s presentation. Images were viewed before and after a sad MI to assess the effect of a sad mood on attention to positive and negative scenes. Younger and older adults exhibited positively biased attention after the sad MI, significantly increasing their attention to positive images, with no evidence of an age difference in either experiment. A test of participants’ recognition memory for the images indicated that the sad MI reduced memory accuracy for sad images for younger and older adults. The results suggest that heightened attention to positive images following a sad MI reflects an affect regulation strategy related to mood repair. The implications for theories of the positivity effect are discussed.  相似文献

Suicidal behavior is developmentally mediated, but the degree to which interventions for suicidal behaviors have been developmentally tailored has varied widely. Published controlled studies of psychosocial treatment interventions for reducing adolescent suicidal behavior are reviewed, with a particular emphasis on the developmental nuances of these interventions. In addition, developmental considerations important in the treatment of suicidal adolescents are discussed. There are insufficient data available from controlled trials to recommend one intervention over another for the treatment of suicidal youth, but interventions that are sensitive to the multiple developmental contexts have potential for greater effectiveness in reducing adolescent suicidal behavior.  相似文献

Increasing state legislation and media interest give the appearance of public support for parental responsibility laws; however, some national polls suggest otherwise. Based on disparate global and specific attitudes in other areas of the criminal justice literature, it was hypothesized that relatively weak global support for parental responsibility would be diminished even more if a specific juvenile was described. There is growing recognition of the need by funding agencies, universities, and research units for interdisciplinary research to tackle complex societal problems that cannot be adequately addressed by single disciplines alone. Interdisciplinary collaboration capitalizes on a diversity of perspectives and practices that each discipline offers in hopes of providing innovative solutions to multifaceted problems. However, for interdisciplinary work to be effective, members of the collaboration must recognize that cultural differences exist between and within disciplines. This paper conceptualizes disciplines as cultural groups and advocates for culturally competent practices to facilitate interdisciplinary research and practice. Specifically, each participant in interdisciplinary collaborations must value diversity, develop the capacity for self-assessment, work towards understanding one's own disciplinary culture, and be sensitive to the dynamics inherent when cultures interact. Additionally, members of any interdisciplinary endeavor must be cognizant of power dynamics at play and avoid such things as tokenism, informal hierarchies, and disciplinary policing. Through awareness of one's own disciplinary culture and sensitivity to others, interdisciplinary research and practice may provide creative solutions to important problems.  相似文献
