Motion of the body center of mass (CoM) can often indicate the overall effect of the strategy of forward progression used. In the present study, focus is placed on trunk movements in the sagittal, coronal, and transverse/rotation plane, as well as placement of the CoM, during gait in children with juvenile idiopathic arthritis (JIA). Seventeen children with JIA, all with polyarticular lower extremity involvement were examined before and approximately two weeks after treatment with intra-articular cortico-steroid injections. Movement was recorded with a 6-camera 3D motion analysis system in both the children with JIA and in 21 healthy controls. Trunk and center of mass movements were compared between JIA and controls, and effects of intra-articular cortico-steroid treatment were evaluated. Children with JIA were more posteriorly tilted in the trunk, contrary to the common clinical impression, and had their CoM placed more posterior and off-centred, which may have been a result of pain. With such knowledge, it might be possible to better understand the effects of their pain and involvement, and ultimately to plan a treatment strategy for improving their gait patterns. 相似文献

This study examined significant others' expressed emotion (EE) and a closely related construct, perceived criticism, as predictors of cognitive-behavioral therapy outcome in a sample of 40 patients with social phobia (social anxiety disorder). Patients enrolled in group therapy for social phobia completed pre- and post-treatment questionnaire measures of perceived criticism and anxious and depressive symptoms. Designated significant others were assessed for the components of high EE (criticism, hostility and emotional overinvolvement) using the Camberwell Family Interview. It was hypothesized that these high-EE components and patients' perceived criticism would be associated with poorer treatment outcome, and results ran counter to these expectations. Controlling for initial social phobia severity, lower levels of perceived criticism predicted treatment dropout. There was also a nonsignificant trend for participants with a significant other rated as high in emotional overinvolvement to show less change on a composite symptom measure. Findings from this study suggest that close relationships impact the outcome of cognitive-behavioral interventions for social phobia. 相似文献

Combining an effective psychological treatment with conventional anxiolytic medication is typically not more effective than unimodal therapy for treating anxiety disorders. However, recent advances in the neuroscience of fear reduction have led to novel approaches for combining psychological therapy and pharmacological agents. Exposure-based treatments in humans partly rely on extinction to reduce the fear response in anxiety disorders. Animal studies have shown that D-cycloserine (DCS), a partial agonist at the glycine recognition site of the glutamatergic N-methyl-D-aspartate receptor facilitates extinction learning. Similarly, recent human trials have shown that DCS enhances fear reduction during exposure therapy of some anxiety disorders. This article discusses the biological and psychological mechanisms of extinction learning and the therapeutic value of DCS as an augmentation strategy for exposure therapy. This study describes the use of a cognitive-behavioral computer-administered self-help program with minimal therapist contact for public speaking anxiety. Participants (N = 10) with social phobia, as measured by a structured clinical interview, completed the self-help program in an open clinical trial. The program was delivered via a CD-ROM during individual sessions at an outpatient clinic, including 4 sessions of psychoeducation/cognitive therapy and 4 sessions of exposure therapy using a virtual audience presented on a computer screen. A therapist was available in another room to answer questions and to debrief for up to 10 minutes after each session. Participants completed standardized self-report questionnaires assessing social anxiety at pretreatment, posttreatment, and 3-month follow-up. Participants also completed measures of computer usability. Results showed decreases on all self-report measures of social anxiety from pre- to posttreatment, which were maintained at follow-up (n = 8). Participants also reported that they were satisfied with treatment, that they felt improved after treatment, and that the computer program was user-friendly. This study provides preliminary evidence that a computer-administered cognitive-behavioral-based program that includes minimal therapist contact may reduce public speaking anxiety and suggests that further research with a controlled design is needed. 相似文献

In a particular success for translational research agendas, characterization of the neuronal circuits underlying fear extinction, and basic research in animal extinction paradigms, has led to intervention studies examining the use of D-cycloserine (DCS) to enhance therapeutic learning from exposure-based cognitive-behavioral therapy (CBT). In this article, we review these intervention studies, and discuss DCS augmentation of CBT relative to more traditional combination-treatment strategies in the treatment of anxiety disorders. We offer an accounting, based on evidence for internal context effects, of current limitations in the combination of antidepressant or benzodiazepine medications with CBT and discuss the advantages of isolated-dosing strategies with DCS relative to these limitations. This strategy is contrasted with the chronic-dosing applications of DCS for schizophrenia and Alzheimer's disease, and future directions for isolated-dosing strategies are discussed. 相似文献

A scale to measure attitudes towards seeking psychological help was developed (Beliefs About Psychological Services [BAPS]) and evaluated. This scale was based on items gathered from students, colleagues, and the authors when asked about common positive and negative attitudes toward psychologists and their services. Items were also based on the Fischer and Turner (1970) Attitudes Toward Seeking Professional Psychological Services (ATSPPH) measure. Three studies are reported describing the construction and evaluation of the psychometric properties of the BAPS. The BAPS is an 18-item measure consisting of three subscales: Intent, Stigma Tolerance, and Expertness. Support was found for the reliability and validity of the measure. 相似文献

Previous research suggests that movements are represented early in practice in visual-spatial coordinates/codes, which are effector independent, and later in practice in motor coordinates/codes (e.g., joint angles, activation patterns), which are effector dependent. In the present experiments, the task was to reproduce 1.3 s patterns of elbow flexions and extensions. An inter-manual transfer paradigm was used in Experiment 1 and an inter-manual practice paradigm was used in Experiment 2. The present results clearly indicated a strong advantage of effector transfer when the motor coordinates available during acquisition were reinstated (Experiment 1) and demonstrate that inter-manual practice with the same motor coordinates results in enhanced retention performance relative to transfer and practice where the same visual-spatial coordinates are used. These results demonstrate that the more effective movement code (motor or visual-spatial) is dependent on the movement sequence characteristics (e.g., difficulty, number of elements, and mode of control [preplanned or on-line]). These results are also interesting because they indicate, contrary to previous findings with more complex movement sequences, that an effective motor code can be developed relatively early in practice for rapid movement sequences. 相似文献

The radial arm maze is one of the most commonly used tests for assessing working memory in laboratory animals. However, to date, there exists no quantitative method of estimating working memory capacity from performance on this task. Here, we present a mathematical model of performance on the radial arm maze from which we can derive estimates of capacity. We derive explicit results for the two most commonly used measures of performance as functions of number of arms in the maze and memory capacity, assuming a uniform random search. We simulate random non-uniform search strategies. Comparing our model to previous experiments, we show that our model predicts a working memory capacity in the range of 3-9 at the level of performance observed in these experiments. This estimate is within the typical estimate of human working memory capacity. Performance of rats on large mazes (e.g. 48 arms) has been used as evidence that the working memory capacity of rats may be significantly larger than that of humans. We report that memory capacity in the range 3-9 is sufficient to explain the performance of rats in very large radial mazes. Furthermore, when we simulate non-uniform random search strategies observed in the experiments, the resulting estimates do not differ significantly from those assuming a uniform random search. We conclude that a list-based model of working memory with modest capacity is more powerful than previously expected. 相似文献
