This paper considers the implications for training and practice of counsellors’ responses to the notion of challenging clients’ prejudices. It explores tensions in counselling discourse between social responsibility, responsibility to the client and responsibility for one's self as counsellor. Three focus groups of counsellors were asked whether a counsellor should challenge clients’ prejudices. Responses were categorised as challenge, not-challenge or exit responses. Discursive themes identified in the responses are: managing congruence and being non-judgemental; self-care and self-respect; and social responsibility. Strategies for managing the discursive conflicts identified in responding to counsellor-identified client prejudices are summarised. 相似文献

Deciding who should receive maximal technological treatment options and who should not represents an ethical, moral, psychological
and medico-legal challenge for health care providers. Especially in patients with chronic heart failure, the ethical and medico-legal
issues associated with providing maximal possible care or withholding the same are coming to the forefront. Procedures, such
as cardiac transplantation, have strict criteria for adequate candidacy. These criteria for subsequent listing are based on
clinical outcome data but also reflect the reality of organ shortage. Lack of compliance and non-adherence to lifestyle changes
represent relative contraindications to heart transplant candidacy. Mechanical circulatory support therapy using ventricular
assist devices is becoming a more prominent therapeutic option for patients with end-stage heart failure who are not candidates
for transplantation, which also requires strict criteria to enable beneficial outcome for the patient. Physicians need to
critically reflect that in many cases, the patient’s best interest might not always mean pursuing maximal technological options
available. This article reflects on the multitude of critical issues that health care providers have to face while caring

Prior evidence has shown that aversive emotional states are characterised by an attentional bias towards aversive events. The present study investigated whether aversive emotions also bias attention towards stimuli that represent means by which the emotion can be alleviated. We induced disgust by having participants touch fake disgusting objects. Participants in the control condition touched non-disgusting objects. The results of a subsequent dot-probe task revealed that attention was oriented to disgusting pictures irrespective of condition. However, participants in the disgust condition also oriented towards pictures representing cleanliness. These findings suggest that the deployment of attention in aversive emotional states is not purely stimulus driven but is also guided by the goal to alleviate this emotional state. 相似文献

To ascertain the beneficial role of spiritual counseling in patients with chronic heart failure. This is a pilot study evaluating the effects of adjunct spiritual counseling on quality of life (QoL) outcomes in patients with heart failure. Patients were assigned to “religious” or “non-religious” counseling services based strictly on their personal preferences and subsequently administered standardized QoL questionnaires. A member of the chaplaincy or in-house volunteer organization visited the patient either daily or once every 2 days throughout the duration of their hospitalization. All patients completed questionnaires at baseline, at 2 weeks, and at 3 months. Each of the questionnaires was totaled, with higher scores representing positive response, except for one survey measure where lower scores represent improvement (QIDS-SR16). Twenty-three patients (n = 23, age 57 ± 11, 11 (48 %) male, 12 (52 %) female, mean duration of hospital stay 20 ± 15 days) completed the study. Total mean scores were assessed on admission, at 2 weeks and at 3 months. For all patients in the study, the mean QIDS-SR16 scores were 8.5 (n = 23, SD = 3.3) versus 6.3 (n = 18, SD = 3.5) versus 7.3 (n = 7, SD = 2.6). Mean FACIT-Sp-Ex (version 4) scores were 71.1 (n = 23, SD = 15.1) versus 74.7 (n = 18, SD = 20.9) versus 81.4 (n = 7, SD = 8.8). The mean MSAS scores were 2.0 (n = 21, SD = 0.6) versus 1.8 (n = 15, SD = 0.7) versus 2.5 (n = 4, SD = 0.7). Mean QoL Enjoyment and Satisfaction scores were 47.2 % (n = 23, SD = 15.0 %) versus 53.6 % (n = 18, SD = 16.4 %) versus 72.42 % (n = 7, SD = 22 %). The addition of spiritual counseling to standard medical management for patients with chronic heart failure patients appears to have a positive impact on QoL. 相似文献

Studies on cognitive effects of positive emotions have associated positive emotions to broadened attention. Given the widely investigated relationship between self-focused attention and mood, it is important to investigate the effect of positive mood on visuospatial attention for self-related information. We used a performance-based measure to assess fluctuations in attentional broadening from self-related contrasted to not-self-related information. In Experiment 1, we checked that the self-related versus not-self-related stimuli did not evoke differential attention effects in general. In Experiment 2, we manipulated mood and found that an increase in positive mood was associated with a relative broadening of attention for self-related information. These results suggest that the meaning of the target of attention provides an interesting dimension for further investigation into the relation between positive emotions and attentional broadening. 相似文献

While it is common wisdom that a salient visual event draws attention, experimental research provided mixed support for this hypothesis. The present experiment seeks evidence that a singleton draws attention to the degree that its feature is novel or unexpected. Two visual search experiments were conducted where an irrelevant colour singleton is presented on each pre-critical trial to familiarize participants with the presence of the singleton. In the critical trial of Experiment 1, the singleton was presented in a novel colour without prior announcement. The singleton was gazed at significantly earlier and longer in the critical trial, as compared to pre-critical trials. This result is consistent with predictions from the expectancy discrepancy hypothesis that colour novelty is sufficient to capture attention. Experiment 2 tested the alternative explanation that a surprising event mainly leads to a breakdown of the previously acquired attentional set, which in turn causes a reorientation towards perceptual salience. An unannounced change of the background colour in the critical trial while the singleton colour remained unchanged did not induce an attentional capture by the singleton like in Experiment 1. Adaptive decision making relies on learning from feedback. Because feedback sometimes can be misleading, optimal learning requires that knowledge about the feedback’s reliability be utilized to adjust feedback processing. Although previous research has shown that feedback reliability indeed influences feedback processing, the underlying mechanisms through which this is accomplished remain unclear. Here we propose that feedback processing is adjusted by the adaptive, top-down valuation of feedback. We assume that unreliable feedback is devalued relative to reliable feedback, thus reducing the reward prediction errors that underlie feedback-related brain activity and learning. A crucial prediction of this account is that the effects of feedback reliability are susceptible to contrast effects. That is, the effects of feedback reliability should be enhanced when both reliable and unreliable feedback are experienced within the same context, as compared to when only one level of feedback reliability is experienced. To evaluate this prediction, we measured the event-related potentials elicited by feedback in two experiments in which feedback reliability was varied either within or between blocks. We found that the fronto-central valence effect, a correlate of reward prediction errors during reinforcement learning, was reduced for unreliable feedback. But this result was obtained only when feedback reliability was varied within blocks, thus indicating a contrast effect. In this contribution, the author engages in a conversation with Christopher Southgate on the relationship between social evil and what is called natural “evil.” Theologically, this centers around an understanding of creation and fall. It is argued that Southgate typically treats soteriology and eschatology as themes pertaining to an evolutionary theodicy, whereas an adequate ecotheology would discuss the problem of natural suffering under the rubric of the narrative of God's economy.
