The issues involved in deciding whether to use artificial methods of delivering hydration and nutrition are often very difficult for patients, families, and health care providers. Once private and personal matters, these decisions now frequently involve the judicial system. Five years ago, Hospice of Washington recognized the need for a written policy and wrote the one published here. Its goal is to respect individual preferences and family concerns while addressing the nutrition and hydration needs of dying patients. The policy sets parameters on the issue, provides basic information, and encourages crafting the most fitting resolution to each situation.  相似文献

Abstract. Resolution of the entropy‐evolution problem was a significant issue for Pierre Teilhard de Chardin throughout his scientific career. Although never truly satisfied with his solution, he proposed that all energy must be psychic and contain two components. Tangential energy is related to physical energy. Radial energy in some way accounts for increasing complexity and consciousness in evolution. Analysis of developments in thermodynamics, statistical mechanics, and information theory show that Gibbs free energy contains both calorimetric and noetic components, thus validating Teilhard's intuition.  相似文献

Previous research has documented altered autonomic nervous system (ANS) reactivity to laboratory-based social stress tasks in children with social anxiety disorder (SAD). It is unclear, however, whether these alterations are caused by the unfamiliar and possibly threatening lab environment or whether they generalize to other, more representative contexts. Sympathetic and parasympathetic autonomic functioning was assessed in the home (minimizing environmental threat) during a supine baseline phase and two physical activation phases (orthostatic stress, stair stepping) in children (9–13 years) with SAD (n?=?27) and healthy control children (n?=?27). Relative to controls, children with SAD showed tonic autonomic hyperarousal as indicated by higher heart rate and electrodermal activity during the supine baseline phase. Further, there was evidence for stronger cardiac and vascular sympathetic reactivity (T-wave amplitude, pulse wave transit time) to moderate physical activation in children with SAD. Higher autonomic arousal during rest was related to measures of trait social anxiety and general psychopathology. Groups did not differ on parasympathetic parameters. Our results extend previous laboratory findings and provide the first evidence for alterations in children with SAD during basal autonomic regulation and in the absence of explicit social evaluative threat. They may further help to clarify conflicting study results from previous laboratory studies. The findings underline the importance of psychophysiological assessment using different environments and tasks to elucidate the physiological bases of SAD.  相似文献

Background Collaborative learning is recognized as an effective learning tool in the classroom. In order to optimize the collaborative learning experience for children within a collaborative partnership, it is important to understand how to match the children by ability level, and whether assigning roles within these dyads is beneficial or not. Aims The current study investigated the effect of partnering children with different task‐specific abilities and assigning or not assigning helping roles within the dyads on the quality of talk used in a collaborative learning task. Sample The participants in this study comprised 54 year 6 pupils from a Western Sydney government primary school (boys=26, girls=28). The ages ranged from 10 years 10 months to 12 years 4 months with a mean age of 11 years 4 months. Method The children were formed into 27 single sex dyads of low–middle‐ and low–high‐ability partnerships. In half of each of these dyads the higher ability partner was asked to help the lower ability partner, which was compared with just asking partners to work together. The quality of talk used by the dyads while working collaboratively on the problem‐solving task was analysed using a language analysis framework developed by Mercer and colleagues (e.g. Littleton et al., 2005 ; Mercer, 1994, 1996 ). Results Results of this study found that children who worked collaboratively in the low–middle‐ability dyad condition demonstrated significantly more high‐quality exploratory talk than those in the low–high‐ability dyad condition. Although there was no significant difference between dyads who were assigned roles and those who were asked to work together, there was an interaction trend which suggests that low–high‐ability dyads, who were given the roles of helper and learner, showed more exploratory talk than dyads who were asked just to work together. Conclusion Mercer's re‐conceptualization of Vygotsky's Zone of Proximal Development (ZPD) in terms of the Intermental Development Zone (IDZ), which is reliant on constructive challenging discourse, can potentially provide a platform upon which all learners in the classroom can benefit from collaborative learning experiences.  相似文献

The ability to form associations between choice alternatives and their contingent outcomes is an important aspect of learning that may be sensitive to hippocampal dysfunction in memory disorders of aging such as amnestic mild cognitive impairment (MCIa), or early Alzheimer disease. In this preliminary study we examined brain activation using functional magnetic resonance imaging (fMRI) in 12 healthy elderly participants and nine patients with MCIa during an associative learning task. Using a high-field 3.0-Tesla MRI scanner, we examined the dynamic neural response during associative learning over trials. The slope of signal attenuation associated with learning was analyzed for differences between groups within an a priori defined hippocampal region. Results indicated dynamic signal attenuation associated with learning in the healthy elderly sample, but not in MCIa. The absence of an associative learning effect in the MCIa sample reaffirms an important link between the learning difficulties that are commonly encountered in MCIa and the mesial temporal region.  相似文献

Abstract

Motor control is classically described as relying on two components: anticipatory control (feedforward processing) and online control (feedback processing). Here we aimed to unveil the developmental steps of both feedback and feedforward control in 5–10 years old children, using a simple and ecological task. We manipulated object’s weight in a reach-to-displace paradigm. When the weight was known before lifting it, anticipatory processes were quantifiable during the reaching phase. Conversely, an unknown weight triggered online corrections during the displacing phase. Movement kinematics revealed that children anticipate this objet property as young as 5 y-o. This anticipation becomes adequate around 7 y-o and is paralleled by poor online corrections. This simple yet relevant paradigm should allow quantifying deviations from neurotypical patterns in disorders of motor control.  相似文献

ABSTRACT

Multiple experimental, neuropsychological, and individual differences studies indicated that faces are processed (e.g. perceived and memorized) differently from non-face objects. It was suggested that face specificity is a result of configural processing which is different from a rather part-based processing of other objects. In this research, we investigated the specificity of face cognition in terms of processing style. To this aim, we estimated Time-on-Task Effects (ToTEs) that allow inferring the level of automaticity of face and house processing. We collected data from 219 participants by applying four perception and recognition tasks. Generalized linear mixed-effects modelling was used to estimate fixed and random effects. Random slopes were interpreted as individual differences in ToTEs. Findings suggested that in the majority of tasks, face processing was dissociable from the processing of non-face objects. Thereby, these results offer a new perspective on the nature of differences in face and object processing.  相似文献
