The establishment of counselling and psychotherapy as “evidence‐based” interventions is contingent on the effective implementation of randomised controlled trials (RCTs) in real‐world clinical settings. This paper identifies some of the pitfalls that led to the termination of a practical RCT in a community mental health setting. It reflects upon the significance of the lessons learned by drawing on similar challenges that are documented in the wider literature. Issues surrounding staff turnover, counsellor engagement and procedural clarity are discussed, as is the conflict between the time required for research procedures and counsellors’ clinical time constraints. In our recommendations, we recognise the decisive role of collaboration and counsellor motivation in research partnerships, and the significance of clinical relevance, communication, training and research co‐production in achieving them. Time commitment is identified as the pivotal challenge in the formation and maintenance of collaborative research, but one that also must be overcome for the advancement of the counselling and psychotherapy field.  相似文献

Does national context shape the relationship between religious attendance and women's gender ideologies? Although previous studies have examined gender and religion within a single national context (and often in a single faith community), few have done so using a cross-national comparative perspective. This has left a significant gap in our understanding of how gender and religion operate in distinctive national contexts. Relying on survey data collected in 37 countries, this study analyzes how the relationship between religious attendance and women's gender ideologies is conditioned by their country's gender inequality. The findings indicate that while women's religious attendance is, on average, negatively related to egalitarian gender ideologies, this association is conditioned by religious affiliation. The relationship is also contingent on the level of gender inequality experienced, and grows weaker in countries with more prominent gender inequality. This study reveals the mechanisms that contextualize the intersection of gender and religion.  相似文献

Individual differences in emotional functioning, pain appraisal processing, and perceived social support may play a relevant role in the subjective experience of pain. Due to the paucity of data regarding individuals with Rheumatoid Arthritis (RA), the present study aimed to examine pain intensity, emotional functioning (psychological distress and alexithymia), pain appraisal (pain beliefs, pain catastrophizing, and pain-related coping strategies) and social support, and their relationships with the health-related quality of life (HRQoL) in patients with RA. Data were collected from 108 female patients diagnosed with RA. Clinically relevant levels of depressive and anxiety symptoms assessed by the HADS subscales were present in 34% and 41% of the patients, respectively, and about 24% of them exhibited the presence of alexithymia. The results of hierarchical multiple regression analyses showed that pain intensity, alexithymia, the maladaptive beliefs regarding the stability of pain and the coping strategy of guarding explained 54% of the variance in the physical component of HRQoL (p < 0.001). Depression subscale of the HADS, alexithymia, the coping strategy of resting, and the rumination factor of pain catastrophizing significantly explained 40% of the variance in the mental component of HRQoL (p < 0.001). The present findings provide evidence regarding the importance of emotional functioning and pain appraisal in the negative impact of RA on patients’ quality of life. These findings provide additional evidence for the biopsychosocial model of chronic pain, further supporting the complex interaction between emotional, cognitive, and behavioral processes in patients with chronic pain.  相似文献

The authors present the first part of a study on children with BHS (Breath Holding Spells) syndrome. These children were chosen in order to examine a “pathological” situation in infancy that was as close as possible to a “normal” one, and in which obvious somatic symptoms were associated with emotional states. Children with BHSS are not considered to have serious psychological disturbances, even if they are constantly referred to as intolerant of separation, stubborn, oppositional, or easy to temper tantrum. A reduced ability to tolerate frustration is evident in these children both from the patients' history and clinical observation. Parents usually relate BHS to situations in which the child is denied something or gets scolded. The children were observed during three sessions, both with and without the mother, according to a grid used at our Center. Patients are presented to illustrate certain recurrent features in play and interaction with mother. The data suggest that these children stagnate in an area of incomplete mental representation; something makes them oscillate between two states; in one of them, the mental representation of the object and of the bond with it is possible; in the other, the mental representation of the separation, which entails the recognition of the object, is felt as dangerous.  相似文献

In 2 studies, the authors examined self-esteem, persistence, and rumination in the face of failure. Study 1 manipulated degree of failure and availability of goal alternatives. When an alternative was available, high self-esteem (HSE) participants persisted more than low self-esteem (LSE) participants after a single failure, but less after repeated failure. When no alternative was available, no self-esteem differences in persistence emerged. LSE participants ruminated more than HSE participants. Study 2 examined persistence and rumination for 10 personal goals across an academic year. HSE participants were better calibrated (higher within-subject correlations between perceived progress and persistence across goals), had higher overall levels of persistence, higher grade point averages, and lower levels of rumination than LSE participants. Although traditional views that emphasize the tenacious persistence of HSE individuals need revision, HSE people appear more effective in self-regulating goal-directed behavior.  相似文献

Eight experts in Rational Emotive Behavior Therapy (REBT) provided personal examples of their own successes and failures in applying REBT to themselves. The experts actively talked to themselves both rationally and irrationally. Understandably, there were far more shoulds, oughts, musts, and have to's in the narratives in which the experts described when they failed to use REBT than when they succeeded in using REBT. Research employing the Iowa Gambling Task (IGT) has frequently shown that learning is impaired in various clinical populations. However, precisely what constitutes "unimpaired" control group learning remains unclear. In order to understand some of the possible factors underlying variability in control group IGT performance, the present study sought to manipulate features of the task to intentionally disrupt learning. Specifically, the present study investigated the effects of time constraints on emotion-based learning during automated administration of the IGT. For two groups of participants, a time-constraint of either 2-s or 4-s was implemented during the critical decision making period, while a control group received no time constraint. We also evaluated participants' subjective experience after every block of 20 trials. Results demonstrated that the 2-s group differed significantly from the control group. Subjective experience measures revealed rapid development of awareness of the advantageous and disadvantageous decks among all three groups. Overall, our findings demonstrate, for the first time, the effects of decision-phase time constraints on emotion-based learning and indicate that the IGT reward/punishment schedules are to some extent cognitively penetrable.  相似文献
