The outcome of a randomized controlled trial of cognitive behavior therapy in addition to treatment as usual (CBT plus TAU) compared with TAU alone (TAU) in one hundred and six participants meeting diagnostic criteria for borderline personality disorder is described. We anticipated that CBT plus TAU would decrease the number of participants with in-patient psychiatric hospitalizations or accident and emergency room contact or suicidal acts over twelve months treatment and twelve months follow-up, compared with TAU. We also anticipated that CBT plus TAU would lead to improvement in a range of secondary outcomes of mental health and social functioning compared to TAU. Of the 106 participants randomized, follow-up data on 102 (96%) was obtained at two years. Those randomized to CBT were offered an average of 27 sessions over 12 months and attended on average 16 (range 0 to 35). We found that the global odds ratio of a participant in the CBT plus TAU group compared with the TAU alone group having any of the outcomes of a suicidal act, in-patient hospitalization, or accident and emergency contact in the 24 months following randomization was 0.86 (95% confidence interval [CI] 0.45 to 1.66, p = 0.66). The corresponding global odds ratio, excluding accident and emergency room contact, was 0.75 (95% CI 0.37 to 1.54, p = 0.44). In terms of the number of suicidal acts, there was a significant reduction over the two years in favor of CBT plus TAU over TAU, with a mean difference of -0.91 (95% CI -1.67 to -0.15, p = 0.020). Across both treatment arms there was gradual and sustained improvement in both primary and secondary outcomes, with evidence of benefit for the addition of CBT on the positive symptom distress index at one year, and on state anxiety, dysfunctional beliefs and the quantity of suicidal acts at two year follow-up. A 3-year longitudinal study assessed the effects of studying in the social sciences versus commerce on sociopolitical orientation. Results reveal field-specific changes in attitudes. Commerce students (n= 34) became, over time, more favorable toward “capitalists,” less favorable toward “unions,” and less likely to attribute poverty and unemployment to systemic factors. In contrast, social science students (n= 57) maintained liberal attitudes and became less likely to attribute poverty and unemployment to internal dispositions. Beliefs about internal and external causes of poverty and unemployment, while unrelated in 1st year, become negatively related in 3rd year but only among social science students. Measures taken in 3rd year to assess the influence of peers, professors, and courses suggest that peers have a generally conservative effect, even in the social sciences, while professors and courses have a liberal effect only in the social sciences. 相似文献

We address how listeners perceive temporal regularity in music performances, which are rich in temporal irregularities. A computational model is described in which a small system of internal self‐sustained oscillations, operating at different periods with specific phase and period relations, entrains to the rhythms of music performances. Based on temporal expectancies embodied by the oscillations, the model predicts the categorization of temporally changing event intervals into discrete metrical categories, as well as the perceptual salience of deviations from these categories. The model's predictions are tested in two experiments using piano performances of the same music with different phrase structure interpretations (Experiment 1) or different melodic interpretations (Experiment 2). The model successfully tracked temporal regularity amidst the temporal fluctuations found in the performances. The model's sensitivity to performed deviations from its temporal expectations compared favorably with the performers' structural (phrasal and melodic) intentions. Furthermore, the model tracked normal performances (with increased temporal variability) better than performances in which temporal fluctuations associated with individual voices were removed (with decreased variability). The small, systematic temporal irregularities characteristic of human performances (chord asynchronies) improved tracking, but randomly generated temporal irregularities did not. These findings suggest that perception of temporal regularity in complex musical sequences is based on temporal expectancies that adapt in response to temporally fluctuating input. 相似文献

This article discusses risk and resilience factors that may affect military families, with a focus on frequent relocation, deployment, exposure to combat and PTSD, and postdeployment reunion as possible risk factors influencing child psychosocial and academic outcome. Research findings are presented as supporting a theoretical pathway that suggests that the effects of military life on child outcome may follow an indirect pathway involving parental stress and psychopathology, rather than military life directly affecting children of military parents. The proposed pathway also serves to highlight the need for further research on understudied resilience factors and provides suggestions for interventions that may benefit military families. 相似文献

The present research addressed whether auditory feedback associated with a learned action sequence can activate the action
representation of that sequence. Nonpianist participants learned to perform two melodies at a piano keyboard repeatedly during
a trial. The participants heard feedback either from the melody they were performing (normal feedback) or from the other learned
melody (termed alternate feedback). An additional tone functioned as an instruction cue to either switch melodies or continue the current melody.
Following the instruction cues, participants typically paused just before switching, and paused similarly during trials with
a continue cue. Participants paused longer after a continue cue when they experienced alternate rather than normal feedback.
This effect was specific to instruction cues positioned at weak metrical accents—positions at which participants were less
likely to switch overall. Feedback did not influence timing on switch trials. These findings indicate that influences of auditory
feedback can activate learned action sequences, leading to longer latencies associated with cue evaluation. 相似文献

How is visual object perception limited by divided attention? Whereas some theories have proposed that it is not limited at all (unlimited capacity), others have proposed that divided attention introduces restrictive capacity limitations or serial processing (fixed capacity). We addressed this question using a task in which observers searched for instances of particular object categories, such as a moose or squirrel. We applied an extended simultaneous-sequential paradigm to test the fixed-capacity and unlimited-capacity models (Experiment 1). The results were consistent with fixed capacity and rejected unlimited capacity. We ascertained that these results were due to attention, and not to sensory interactions such as crowding, by repeating the experiment using a cuing paradigm with physically identical displays (Experiment 2). The results from both experiments were consistent with theories of object perception that have fixed capacity, and they rejected theories with unlimited capacity. The nature of capacity limits (if any) in visual search has been a topic of controversy for decades. In 30 years of work, researchers have attempted to distinguish between two broad classes of visual search models. Attention-limited models have proposed two stages of perceptual processing: an unlimited-capacity preattentive stage, and a limited-capacity selective attention stage. Conversely, noise-limited models have proposed a single, unlimited-capacity perceptual processing stage, with decision processes influenced only by stochastic noise. Here, we use signal detection methods to test a strong prediction of attention-limited models. In standard attention-limited models, performance of some searches (feature searches) should only be limited by a preattentive stage. Other search tasks (e.g., spatial configuration search for a "2" among "5"s) should be additionally limited by an attentional bottleneck. We equated average accuracies for a feature and a spatial configuration search over set sizes of 1-8 for briefly presented stimuli. The strong prediction of attention-limited models is that, given overall equivalence in performance, accuracy should be better on the spatial configuration search than on the feature search for set size 1, and worse for set size 8. We confirm this crossover interaction and show that it is problematic for at least one class of one-stage decision models. 相似文献
