Using mixed methodology, the authors examined facilitators to psychological thriving—or positive changes after adversity—in the context of ethnic discrimination among Latina/o adults. Several themes emerged from qualitative interviews (n = 10), including proving others wrong, ethnic identity, and community involvement. Quantitative analyses (n = 97) indicated that U.S. identity moderated the relationship between ethnic microaggressions and thriving such that high levels of U.S. identity were associated with lower thriving. Implications and future directions are discussed. Usando una metodología mixta, los autores examinaron los facilitadores de la prosperidad psicológica (o cambios positivos tras la adversidad) en el contexto de la discriminación étnica en personas latinas adultas. Varios temas surgieron de las entrevistas cualitativas (n = 10), entre los que se incluyen demostrar que otros estaban equivocados, la identidad étnica y la implicación de la comunidad. Objective: A minority of patients show sub-optimal outcomes after weight loss surgery. Research has identified predictors of poor outcomes focusing on the patients’ perspective. No research has explored surgeons’ accounts. Design: Ten Bariatric surgeons were interviewed using a critical incident approach to explore their explanations for sub-optimal outcomes in the context of a real-life case. Data were analysed using thematic analysis. Results: Three main themes were developed: ‘Challenges to success’ highlighting the role of psychosocial issues, poor adherence and patient non-disclosure; ‘Ideal world solutions’ describing who should identify and address psychosocial issues; and ‘Real world compromise’ reflecting the impact of limited resources and weighing up risk between carrying out versus not carrying out surgery. Transcending these themes was the notion of ‘responsibility’ with surgeons balancing the role of the patient, themselves and the health care system. Conclusion: Some surgeons concluded that if they had known before surgery what they know now, they may not have operated. All emphasised that they could only know what was disclosed by the patient, that they were not convinced that not operating would have resulted in better outcomes in the longer term and many felt that they were ‘damned one way or the other’. 相似文献

The quality of father–child interactions has become a focus of increasing research in the field of child development. We examined the potential contribution of father–child interactions at both 3 months and 24 months to children's cognitive development at 24 months. Observational measures of father–child interactions at 3 and 24 months were used to assess the quality of fathers’ parenting (n = 192). At 24 months, the Mental Developmental Index (MDI) of the Bayley Scales of Infant Development, Second Edition (N. Bayley, 1993 ) measured cognitive functioning. The association between interactions and cognitive development was examined using multiple linear regression analyses, adjusting for paternal age, education and depression, infant age, and maternal sensitivity. Children whose fathers displayed more withdrawn and depressive behaviors in father–infant interactions at 3 months scored lower on the MDI at 24 months. At 24 months, children whose fathers were more engaged and sensitive as well as those whose fathers were less controlling in their interactions scored higher on the MDI. These findings were independent of the effects of maternal sensitivity. Results indicate that father–child interactions, even from a very young age (i.e., 3 months) may influence children's cognitive development. They highlight the potential significance of interventions to promote positive parenting by fathers and policies that encourage fathers to spend more time with their young children. 相似文献

Summer camp programming has a rich history of promoting childhood development. In the care of children with specific childhood psychiatric disorders, the incorporation of targeted cognitive-behavioral principles provides an opportunity to marry targeted evidence-based practices with broader development, in particular social, emotional, and fine- and gross-motor development. This union is synergistic, providing the practitioner with an opportunity to employ cognitive-behavioral practices in an environment that may overcome common barriers to effective interventions outside the scope of the targeted illness. In this paper, the authors describe the preliminary findings concerning a weeklong, 25-hour summer camp program targeting childhood obsessive–compulsive disorder (OCD). Nine children ages 9–12 years participated in this pilot program. Child and parent feedback alike suggested strong treatment acceptability and efficacy in targeting both core symptoms of OCD as well as associated developmental deficits. The camp’s implementation of exposure and response prevention enables an opportunity to report on the capability of employing these strategies in a summer camp setting. In conjunction with an evidence-based treatment program for childhood OCD, a summer camp program specifically targeted for children with OCD presents a valuable tool for improving child welfare and reducing functional impairments. 相似文献

A community survey in 4,426 adults was undertaken as part of the World Mental Health Survey Initiative reporting the prevalence and risk factors for suicide‐related outcomes in Colombia. Lifetime prevalence estimates of suicide ideation, plans, attempts, and risk factors for suicide‐related outcomes were assessed. Retrospective reports of disorders age‐of‐onset were used to study associations between primary mental disorders and first onset of suicide‐related outcomes. Risks of plans and attempts were highest with ideation early age‐of‐onset and within the first year. The highest risk for ideation and attempt among ideators occurred in the 18–29 age group. After first employment (defined as the first paid job accepted by the respondent) and presence of mental disorders constituted risk factors. Impulse‐control disorders were strongest diagnostic predictors. 相似文献

At various points in the trajectory through the criminal justice system, a person may be encouraged by the hope of legal benefit to consent to medical treatment. This benefit may consist of diversion from prosecution, a favorable sentence, or parole. This form of legal encouragement has been referred to as legal leverage, quasi-coercive, or quasi-compulsory treatment. In this article, we analyze interviews with 15 men convicted of sexual offenses to explore their reactions to two hypothetical scenarios involving men consenting to a range of treatments intended to reduce risk of recidivism. In particular, we explore their reactions to quasi-coercive treatment using both real and hypothetical forms of biological therapy (e.g., drugs, brain stimulation, surgery), as opposed to psychological counselling. Here, we consider the extent to which these individuals perceive the situation to be coercive, the factors affecting these perceptions, and the ways in which physicians may mitigate perceived coercion. We found there is usually some degree of coercion identified when treatment consent is given in exchange for potential legal benefit, although this fact alone did not necessarily render the practice unacceptable. The degree of concern expressed over this potential coercion was related to the invasiveness and/or permanence of the treatment, and all participants highlighted the necessity of obtaining fully informed consent in the context of legally motivated treatment offers. 相似文献

This article reports on a collective effort to position ethics policies within the context of a specific discipline – Applied Language Studies (ALS). Through a discussion of challenges to ALS-specific pedagogical and research practices, this article highlights (1) the need for consistency across institutional Research Ethics Boards in the application of general principles of ethics review, and (2) the recognition of local considerations that are informed by disciplinary approaches not envisioned in current ethics policies. Ethics policies that are driven by substantive ethical intent will recognize pedagogical practices, research methodologies, and epistemological values and traditions that mark a discipline. 相似文献
