Multiple chemical sensitivity (MCS) is a term used to describe a disorder characterized by a vast array of somatic, cognitive, and affective symptoms, the cause of which is attributed to exposure to extremely low levels of a variety of chemicals. Upon examination of the patient with a diagnosis of MCS, objective physical findings and consistent laboratory abnormalities are typically nonexistent. The concept of MCS has ignited considerable controversy in the fields of toxicology, immunology, allergy, psychology, and neuropsychology. Central to the controversy is the disagreement over the extent to which the manifestation of MCS is mediated by psychological factors. Because of the large number of neuropsychological symptoms associated with a diagnosis of MCS, neuropsychologists are increasingly receiving referrals for the assessment of these patients. It is important, therefore, that neuropsychologists become aware of the variety of clinical issues that must be taken into account when assessing an individual with a diagnosis of MCS. The theoretical and research literature on individuals with a diagnosis of MCS is reviewed here.  相似文献

Suboptimal effort is a threat to the validity of neuropsychological evaluations. Numerous papers have been devoted to this subject and a large number of measures have been developed in an attempt to detect suboptimal effort. To date, however, the clinical literature has focused almost exclusively on identifying suboptimal effort in adults, whereas suboptimal effort among children has not been addressed thoroughly in the clinical neuropsychological literature. The present study investigated whether or not already established effort measures could be used with children. The Test of Memory Malingering (TOMM) and the Rey-15-item test were administered to 128 children in two sites, the USA and Cyprus. The results indicated that the TOMM has the potential to be used as a measure for identifying children who do not put forth maximal effort during neuropsychological evaluations. In contrast, the Rey-15-item test does not appear to be a promising measure of effort for use with children, especially younger children.  相似文献

Lingual vibrotactile thresholds were obtained at a frequency of 250 Hz by using an ascending-continuous series of stimulation during five sessions. Each of three adult male Ss attended a control session which included a prescribed period of constant tongue positioning, a pre-experimental session where the mean of six thresholds was obtained as a basis of further testing, and three experimental sessions. In the experimental sessions vibrotactile thresholds after exposure to vibratory stimulation of varying levels of intensity and duration were obtained. The findings of the control session reflected a slight pattern of decreased lingual sensitivity indicative of the influential factor of constant maintenance of tongue positioning. Results from the experimental sessions demonstrated shifts toward progressively decreased lingual sensitivity after exposure to vibratory signals of increased levels of intensity and duration. A recovery phase to near-normal sensitivity followed each stimulus presentation.  相似文献

Causal effect modeling with naturalistic rather than experimental data is challenging. In observational studies participants in different treatment conditions may also differ on pretreatment characteristics that influence outcomes. Propensity score methods can theoretically eliminate these confounds for all observed covariates, but accurate estimation of propensity scores is impeded by large numbers of covariates, uncertain functional forms for their associations with treatment selection, and other problems. This article demonstrates that boosting, a modern statistical technique, can overcome many of these obstacles. The authors illustrate this approach with a study of adolescent probationers in substance abuse treatment programs. Propensity score weights estimated using boosting eliminate most pretreatment group differences and substantially alter the apparent relative effects of adolescent substance abuse treatment.  相似文献

Lyme disease is currently the most common vector-borne illness in the United States. The disease is multisystemic, and chronic disease, in particular, may be associated with neuropsychological deficits. However, to date, only a few empirical studies exist, which examine the neuropsychological sequelae associated with chronic Lyme disease. A review of the literature shows that the deficits observed in adults with chronic Lyme disease are generally consistent with the deficits that can be seen in processes with primarily frontal systems involvement. These observations are generally consistent with neuroradiologic findings. The clinical presentation in chronic Lyme disease and the nature of the neuropsychological deficits are discussed, as are several central issues in understanding neuropsychological functioning in chronic Lyme disease, such as the impact of chronic illness, response to treatment, and the relationship between neuropsychological performance and depression, fatigue, and neurological indicators of disease.  相似文献

This article devises a Bayesian multivariate formulation for analysis of ordinal data that records teacher classroom performance along multiple dimensions to assess aspects characterizing good instruction. Study designs for scoring teachers seek to measure instructional performance over multiple classroom measurement event sessions at varied occasions using disjoint intervals within each session and employment of multiple ratings on intervals scored by different raters; a design which instantiates a nesting structure with each level contributing a source of variation in recorded scores. We generally possess little a priori knowledge of the existence or form of a sparse generating structure for the multivariate dimensions at any level in the nesting that would permit collapsing over dimensions as is done under univariate modeling. Our approach composes a Bayesian data augmentation scheme that introduces a latent continuous multivariate response linked to the observed ordinal scores with the latent response mean constructed as an additive multivariate decomposition of nested level means that permits the extraction of de-noised continuous teacher-level scores and the associated correlation matrix. A semi-parametric extension facilitates inference for teacher-level dependence among the dimensions of classroom performance under multi-modality induced by sub-groupings of rater perspectives. We next replace an inverse Wishart prior specified for the teacher covariance matrix over dimensions of instruction with a factor analytic structure to allow the simultaneous assessment of an underlying sparse generating structure. Our formulation for Bayesian factor analysis employs parameter expansion with an accompanying post-processing sign re-labeling step of factor loadings that together reduce posterior correlations among sampled parameters to improve parameter mixing in our Markov chain Monte Carlo (MCMC) scheme. We evaluate the performance of our formulation on simulated data and make an application for the assessment of the teacher covariance structure with a dataset derived from a study of middle and high school algebra teachers.  相似文献
