This conceptual article presents a model of severe, chronic combat-related PTSD based on several years of longitudinal clinical observations of Vietnam veterans. The model describes a repeating cycle of decompensation that profoundly disrupts the veteran’s life. There appear to be “stages” of decompensation that can be described clinically and may be distinct physiologically. The stages describe a wide range of functioning, from adaptive to totally dysfunctional. PTSD core symptoms, as well as several other dimensions of clinical functioning, such as affect regulation, defenses, ego states, interactions with the environment, capacity for self-destruction/suicide and capacity for attachment and insight are described for each stage. Clinical and research implications are discussed.  相似文献

Certain unconditioned stimuli (UCS) in flavor avoidance learning sometimes become ineffective after pairings with relatively stronger UCS. This failure of avoidance learning (avfail) has been demonstrated only with rodents. The present investigations were conducted to determine whether avfail might also occur with avian species, the food selection of which is guided primarily by visual cues. In Experiment 1, starlings were given pairings of methiocarb (a relatively weak UCS) and LiCl (a relatively strong UCS). In Experiment 2, red-winged blackbirds were given pairings of two concentrations of methiocarb (relatively weak and relatively strong UCS, respectively). Pairings were followed by a conditioning trial (UCS gavage in the presence of a color cue) and two-choice tests. Conditioned avoidance was always observed except when methiocarb preceded LiCl and when the low preceded the high methiocarb dose in preconditioning pairings. Experiment 3 demonstrated that UCS habituation could not account for the results of Experiments 1 and 2. The data reflect avfail in the visual modality, and a biological implication of the results is that birds may not learn strong avoidance of aposematic prey containing varied levels of toxicant.  相似文献

A new preliminary model of emotional disorders, derived from basic tenets of emotion theory and new developments in cognitive science, is presented. It is suggested that tightly organized basic emotions stored in memory fire inappropriately on occasion. In individuals who are vulnerable both biologically and psychologically, these emotions may become the focus of anxiety or dysthymia in that the emotions themselves are experienced as uncontrollable and threatening with adequate coping being difficult or impossible. Early experiences with lack of control over one's environment as well as biological vulnerabilities may well determine whether or not one becomes anxious/dysthymic over the experience of basic emotions in an inappropriate context. This model is illustrated in the context of panic disorder and then extended to depression (sadness/distress), stress (anger), and mania (excitement).  相似文献

Functional analyses were conducted to identify reinforcers for noncompliance exhibited by 6 young children. Next, the effects of rationales (statements that describe why a child should comply with a caregiver-delivered instruction) were evaluated. In Experiment 1, 3 participants received the rationales immediately after the therapist's instruction. In Experiment 2, 3 additional participants received more practical rationales immediately before the therapist's instruction. The results indicate that rationales were ineffective for all 6 children. A guided compliance procedure increased compliance for 1 child; contingent access to preferred items with or without response cost increased compliance for the other participants. Although levels of problem behavior varied within and across participants, they were often higher in the rationale and guided compliance conditions.  相似文献

Advance notice of an upcoming instruction was evaluated to increase compliance among 3 children (4 to 5 years old) who exhibited noncompliance. Asceticism seems an “invisible” Protestant work ethic (PWE) dimension that has largely been ignored in research. This study, building directly on existing knowledge, investigated the possibility that deference to authority was central to asceticism and the possible ethical implications of this. In 3 samples of employed persons, results demonstrated that highly ascetic individuals were highly authoritarian, were low in advanced moral reasoning, and regarded ethically questionable activities benefiting organizations (but not individuals) as relatively acceptable. However, “hard workers,” another PWE dimension, regarded all ethically questionable activities as inappropriate, regardless of the beneficiary, and were less clearly authoritarian. Significant relationships involving the PWE may sometimes be undetectable when combining distinct dimensions, and asceticism merits attention as a distinct PWE component.  相似文献

This study introduces a component of adaptability, namely, a shift in the relative emphasis among different job tasks, to the measurement of employee job performance. Using a multiyear longitudinal data set on the performance of professional basketball players, the results indicate that (a) refocusing one's efforts among different job tasks exists in this context, (b) refocusing accounts for variation in performance, and (c) refocusing increases the odds of continued employment the following year. These findings have implications for understanding temporal change in job performance.  相似文献

We compared differential reinforcement plus escape extinction to noncontingent reinforcement plus escape extinction to treat food selectivity exhibited by a young child with autism. The interventions were equally effective for increasing bite acceptance and decreasing problem behaviors. After a 3‐step guided compliance procedure (vocal prompt, vocal plus model prompt, vocal prompt plus physical guidance) did not increase compliance, we evaluated 2 modifications with 4 preschool children who exhibited noncompliance. The first modification consisted of omission of the model prompt, and the second modification consisted of omitting the model prompt and decreasing the interprompt interval from 10 s to 5 s. Each of the modifications effectively increased compliance for 1 participant. For the remaining 2 participants, neither modification was effective; differential reinforcement in the form of contingent access to a preferred edible item was necessary to increase compliance. Problem behavior varied across participants, but was generally higher during guided compliance conditions and lower during differential reinforcement conditions.  相似文献
